We have evaluated 19 children who were exposed to valproic acid (VPA) in utero to look for manifestations of a fetal valproate syndrome (FVS), as proposed by Di Liberti et al. [1984]. We found no consistent alterations of pre- or postnatal growth with exposure to VPA monotherapy. Postnatal growth deficiency and microcephaly were present however, in two thirds of children exposed to VPA in combination with other anticonvulsants. Developmental delay or neurologic abnormality was found in 71% of those exposed to VPA monotherapy, and in 90% of those exposed to VPA and other anticonvulsants. Cranio

The clinical features of the multiple pterygium syndrome are multiple congenital joint contractures, multiple skin webs, camptodactyly, vertebral anomalies, short stature, ptosis, and antimongoloid eye slant. We present 11 new cases to show the evolution of the full phenotype from birth and to confirm autosomal recessive inheritance. We emphasise morbidity secondary to respiratory impairment and that conductive deafness may be part of the syndrome.

A child with ambiguous genitalia was born after an uncomplicated pregnancy. Laparotomy revealed intraabdominal hypoplastic testes containing normal appearing Leydig cells; germ cells were present in the left gonad, not in the right. The karyotype was 46,XY in blood leukocytes and in fibroblasts cultured from the gonads; there was no evidence of mosaicism. Endocrinologic study revealed no disorder of steroidogenesis. Androgen receptors were not studied. Serologic evaluation of blood leukocytes revealed the presence of H-Y antigen, but there are reasons to believe that less H-Y antigen was prese

Antimicrobial resistance (AMR) poses an escalating global threat, yet surveillance and mechanistic understanding remain anchored in individual resistance genes, an approach that is inadequate for capturing the ecological, genomic, and metabolic complexity through which resistance emerges and spreads. In this opinion article, we argue that genome-scale metabolic models (GEMs) offer a principled framework for advancing AMR research beyond this gene-centric paradigm. Strain-resolved GEMs embed resistance determinants within metabolic networks, with the potential to generate quantitative, context-

PURPOSE: To utilise MR phenotyping in accurately predicting paediatric thalamic tumour type via qualitative MR analysis and quantitative diffusion histogram analysis, with the aim of optimising pre-surgical planning and patient outcomes.
METHODS: Patients with tumours epicentred in the thalamus were identified through an electronic patient record search (2000 to 2024). Radiological features evaluated included tumour margins, homogeneity, cystic components, T1 and T2 signal, contrast enhancement, and haemorrhage. Histopathological tumour type, WHO grade, and molecular subtype were recorded. Tu

The impact of the metabolic microenvironment on epigenetically plastic cancer cells underpins phenotypic heterogeneity, a major cause of metastasis and therapy resistance. Nutrient limitation is a key microenvironmental stress, and can cause cells to transition from proliferative to invasive phenotypes, however, whether cancer cells have the capacity to delay phenotype switching remains unknown. Here, using melanoma as a model, we reveal that the ability to buffer glucose availability by accumulating and mobilizing glycogen can determine cancer cell phenotypic transitions. While proliferating …

The coexistence of chronic obstructive pulmonary disease (COPD) and bronchiectasis is increasingly recognized as a distinct clinical phenotype that presents unique diagnostic and therapeutic challenges. This overlap syndrome is characterized by heightened airway inflammation, frequent bacterial colonization, and a greater burden of respiratory symptoms, leading to poorer clinical outcomes. In this review, we explore the pathophysiological mechanisms underlying COPD-bronchiectasis overlap, emphasizing the role of chronic infection, impaired mucociliary clearance, and structural lung damage in d…

Mixed phenotype acute leukemias (MPALs) are a heterogeneous group of acute leukemias that show differentiation along more than one lineage. MPAL are rare and account for <5 % of all acute leukemias and demonstrate an inferior prognosis compared with standard acute lymphoblastic or myeloid leukemias. Historically, due to the limited understanding of its underlying pathogenesis, there were no well-established classification schemes, leading to difficulty in both diagnosis and treatment. With the advent of new nomenclature and algorithms, including the European Group for the Immunological Charact…

BACKGROUND AND OBJECTIVES: β-mannosidosis is an ultra-rare lysosomal storage disorder caused by a deficiency of β-mannosidase, which catalyz … iew previously reported patients to broaden the spectrum of clinical features and identify genotype-phenotype correlations.
RESULTS: Developmental regression, dysphagia, obsessive-compulsive-like behavior, and erythromelalgia are newly described features associated with β-mannosidosis among the 6 patients from our cohort. Nystagmus in association with β-ma …

Multiple endocrine neoplasia type 2 (MEN2) is an autosomal dominant hereditary cancer syndrome caused by germline variants in the REarranged … anglioneuromas of the intestines. Specific RET mutations suggest a predilection toward a particular phenotype and clinical course with strong genotype-phenotype correlation. Based upon these genotype-phenotype correlations, RET mutations are stratified into three risk levels, i.e., highest, high, and moderate risk, based on the age of onset and the penetran …

Neuronal ceroid lipofuscinoses (NCLs) are a group of autosomal recessive neurogenetic disorders caused by mutations in 14 different genes. CLN6 disease manifests as variant late-infantile NCL (vLINCL) or as an adult variant. In childhood, symptoms include speech delay, vision loss, cognitive and motor decline, seizures, and early death. An in-depth characterization of a naturally occurring Cln6 mutant mouse (Cln6nclf) is presented, with implications for translational research. The expanded phenotype provides data showing early death, vision loss, and motor deficits in male and female Cln6nclf …

Dilated cardiomyopathy (DCM) is a rare disease in children and a leading cause of heart failure. There are numerous causes of DCM including genetic causes leading to isolated or syndromic presentations, with a wide variety of implicated genes. Among them, PPP1R13L is associated with a recessive syndrome leading to cardiac anomalies with skin, teeth, and hair abnormalities. Fifteen patients have been described so far. We report a patient born to unrelated parents with early-onset and progressive DCM, skin appendage anomalies, and an anorectal anomaly. Her late brother shared the same phenotype.…
