The state of the musculo-venous pump of lower extremities was studied in 72 patients with trophic ulcers caused by varicose disease. The methods used were: phlebotonometry, phleboplethysmography, US dopplerography, distal ascending and retrograde-femoral, popliteal and talocrural phlebography. This is a prospective study involving seventy-two consecutive admissions of patients with fractures of the proximal femur over a period of eleven months. It reviews the incidence, pattern and clinical course of deep vein thrombosis in this group of patients. In a group of 70 patients, 40 undergoing the insertion of a Mobin-Uddin umbrella and 30 the placing of an Adams-DeWeese clip, the authors noted a 6% recurrent embolism rate within a follow up period of 14 months to 5 years. This was fatal in 1.5% of cases. There was 10.9% recurrent phlebitis and 9.4% sequelae of severe stasis. Mobin-Uddin umbrellas gave the most complications and should be reserved for the worst cases. Indications for the Adams-DeWeese clip can be extended to surgical patients with a thrombo-embolic risk and to dangerous clots visualised by phlebography.

A prospective and randomized study of 100 patients with fracture of the femoral neck was carried out. The aim was to compare the thromboembolic prophylactic effect of dextran 70 and a sulphated polysaccharide, PZ 68. From 1973 to 1977 a total of 63 patients with thrombosis of the deep veins in leg and pelvis and in the V. cava inferior was treated by operative thrombectomy. Prior to the operation all patients were submitted to phlebography. The mean age of all was 52 years. One patient died from pulmonary embolism during thrombectomy, there was no further hospital mortality. Because of low mortality incidence and rarely postthrombotic complications we prefer the operative treatment of deep vein thrombosis. The preoperative cavography helps to avoid pulmonary embolism during thrombectomy.

PURPOSE: Persistent sciatic vein (PSV) is a rare anomaly most often associated with Klippel-Trenaunay Syndrome (KTS). Magnetic resonance imaging (MRI) and extended use of phlebography have increased the frequency of diagnoses. To evaluate our experience, we reviewed patients in whom PSV was diagnosed at our institution.

Sixty patients who were scheduled for total hip replacement were randomly allocated to receive either epidural blockade or neurolept anaesthesia. All patients received dextran 70.
