Psoriatic arthritis and the association with cardiometabolic disease: a narrative review.

UNLABELLED: Psoriatic arthritis (PsA) is associated with a higher burden of cardiometabolic disorders, such as hypertension, dyslipidemia, diabetes, obesity, and cardiovascular disease (CVD), compared with the general population. These comorbidities are associated with the severity of disease, and adversely affect treatment outcomes in PsA. Comorbidities lead to increased physician visits and medications for patients and make the selection and maintenance of therapies challenging for physicians. Moreover, CVD is a leading cause of mortality in PsA. Therefore, optimal management of PsA should i

Exploring the use and usefulness of living guidelines for consumers: international online survey of patients' and carers' views.

BACKGROUND AND OBJECTIVE: Living guidelines contain continually updated, and potentially changing, clinical recommendations. The implications of living guidelines for consumers (eg, patients, carers, and people with lived experience) - particularly how living guidelines should be developed and disseminated - are yet to be established. The objective of this study was to explore consumers' views about how best to support the use and usefulness of living guidelines to consumers. METHODS: This study used a qualitative (online survey) design. We invited consumers who were familiar with guidelines

Transdiagnostic Assessment of Mental Health and Sleep Registry: Protocol for a Cross-Sectional Study.

BACKGROUND: Mental disorders impose a substantial burden on individuals and society that cannot be fully captured by diagnostic labels or clinial indicators alone. Accordingly, the need for approaches that comprehensively assess patients' subjective experiences and daily functioning is growing. Patient-reported outcomes (PROs) enable multidimensional assessment from the patient's perspective and are increasingly important in mental health research. However, prior studies have frequently assessed these domains in isolation using different instruments and assessment schedules, limiting individua

From zero-COVID to global alignment: transnational pressures and China's pandemic communication transformation.

BACKGROUND: Public health crises are governed not only through policies but also through talk. Government press conferences are ritualized arenas where authorities construct meaning, claim competence, and manage domestic and international legitimacy. China's abrupt transition from "zero‑COVID" to a strategy of coexistence provides a critical case for examining how transnational pressures-from the World Health Organization, diplomatic partners, markets, and global media-shape official communication over time. MAIN BODY: This study analyzes 154 central government press‑conference transcripts (F

INDEX study: intravenous dexamethasone palmitate for prophylaxis of postoperative nausea and vomiting: a study protocol for a randomised controlled study.

INTRODUCTION: Dexamethasone is commonly used to prevent postoperative nausea and vomiting (PONV). However, PONV is an unresolved complex problem; even with preoperative dexamethasone administration, 14.0%-26.3% of patients undergoing minimally invasive endoscopic procedures still experience PONV within 24 hours postoperatively. Compared with dexamethasone, dexamethasone palmitate has a long-lasting anti-inflammatory effect, two to five times that of traditional water-soluble dexamethasone, with fewer adverse events. This trial aims to assess the efficacy and safety of preoperative dexamethason

Understanding the pathophysiologic pathways that underlie obesity and options for treatment.

INTRODUCTION: Obesity is a chronic, multifactorial condition with devastating health consequences. It was thought that obesity could be controlled with discipline and lifestyle changes, but we now know that the underlying pathophysiology is a dysregulation of the body's energy balance system, controlled by a complex interplay of neural, hormonal, and metabolic pathways. Recognizing obesity as a chronic disease places a greater responsibility on all health care professionals to screen and identify patients at risk and develop long-term tailored treatment plans. AREAS COVERED: This narrative re

Perceptions of Hispanic Patients With Advanced CKD About Communication and Engagement Regarding Kidney Transplantation: A Qualitative Study.

RATIONALE & OBJECTIVE: Barriers in access to kidney transplantation (KT) for Hispanic patients with advanced chronic kidney disease (CKD) are often attributed to spoken languages different from those of their health care providers. Effective communication, however, is not limited to language, and involves consideration of social context and background; if successful, it can lead to better engagement. Understanding patients' perspectives on communication and engagement with their providers regarding KT may inform clinical practice, and this was the objective of the present study. STUDY DESIGN:

What we know, don't know, and need to know about patient authorship: a scoping review, evidence gap analysis, and research roadmap.

BACKGROUND: Patient authorship is an innovation, challenging a centuries-old medical publishing model. To guide best practices for patient authorship, a strong evidence base is required. The objectives of this research were to: (1) Identify 'what we know' about patient authorship via a scoping review. (2) Describe 'what we don't know' about patient authorship via an evidence gap analysis. (3) Propose 'what we need to know' about patient authorship via a research roadmap. METHODS: This patient-led, voluntary, research project was conducted according to best practice guidelines. Using a registe

Associations between periodontal inflammation and OHIP-14 scores: A cross-sectional study.

BACKGROUND: More evidence is needed to assess the association between periodontitis and Oral Health-Related Quality of Life (OHRQoL) quantitatively. We hypothesized that periodontal inflammation is correlated with a worsening in quality of life and aimed to test this in this study. METHODS: A total of 1307 participants in King's Oral Biobank answered the Oral Health Impact Profile-14 (OHIP-14) questionnaire (measurement of OHRQoL) during a dental visit. The associations between periodontal inflammation, measured as periodontal inflamed surface area (PISA) and bleeding on probing (BOP), period

Bone turnover marker rhythmicity: implications for osteoporosis treatment and fracture healing.

ABSTRACT: Bone turnover is a dynamic process with markers of bone turnover displaying varying degrees of rhythmicity. The objective of this narrative review was to synthesize current evidence on the regulation of bone turnover marker (BTM) rhythmicity and to evaluate how these rhythms may influence clinically relevant outcomes in osteoporosis treatment and fracture healing. Structured literature searches were conducted in PubMed to identify studies that examined BTM rhythmicity, intrinsic versus extrinsic factors that influence this rhythmicity, and timing effects of osteoporosis medications a

Visualizing Preosteoarthritis: Updates on UTE-Based Compositional MRI and Deep Learning Algorithms.

Osteoarthritis (OA) is heterogeneous and involves structural changes in the whole joint, such as cartilage, meniscus/labrum, ligaments, and tendons, mainly with short T2 relaxation times. Detecting OA before the onset of irreversible changes is crucial for early proactive management and limit growing disease burden. The more recent advanced quantitative imaging techniques and deep learning (DL) algorithms in musculoskeletal imaging have shown great potential for visualizing "pre-OA." In this review, we first focus on ultrashort echo time-based magnetic resonance imaging (MRI) techniques for di

The challenge of ultra-rarity: Dual diagnosis of Lafora disease and developmental encephalopathies linked to TRIO and SHANK3 pathogenic variants.

We report two cases of dual genetic diagnoses involving Lafora disease (LD) and co-occurring neurodevelopmental disorders caused by pathogenic variants in TRIO and SHANK3, respectively. LD is an ultra-rare, autosomal recessive, severe form of progressive myoclonus epilepsy affecting previously healthy children or adolescents. In both patients, the presence of developmental delay, intellectual disability, and behavioral abnormalities was consistent with a primary genetic disorder-TRIO-related neurodevelopmental disorder in one, and Phelan-McDermid syndrome in the other. However, the onset of ep

Urethral Prolapse Case Report: Surgical and Social Considerations in Senegal.

We present three cases of urethral prolapse in prepubertal females in Senegal who presented with vulvar bleeding. Careful gynecologic and urologic physical exams were performed and revealed urethral origin and prolapse. Conservative versus surgical approaches were taken in different patients, but ultimately, each patient received a urethral meatoplasty. Surgical excision of these masses yielded a full recovery in the patients. A careful review of the literature was then undertaken and showed that surgical excision or ligation of the prolapse is preferable to more conservative treatment. The ca
