Detecting plasma cell precursors in autoimmune hepatitis.

BACKGROUND AND PURPOSE: The assessment of patients with temporal lobe epilepsy (TLE) traditionally focuses on the hippocampal formation. These patients, however, may present structural abnormalities in other brain areas. Our purpose was to develop a method to measure the combined volume of the human piriform cortex and cortical amygdala (PCA) by using MR imaging and to investigate PCA atrophy. METHODS: The definition of anatomic landmarks on MR images was based on histologic analysis of 23 autopsy control subjects. Thirty-nine adults with chronic TLE and 23 age-matched control subjects were s

Staying alive: regulation of plasma cell survival.

On describing the catastrophic effect of the plague during the Peloponnesian War, Greek historian Thucydides (c ∼450 BC) made the prescient observation that the "same man was never attacked twice - never at least fatally". This is probably the first description of the mammalian immune systems' remarkable ability to elicit a pathogen-specific response that potentially protects the host for its lifetime. This protection is largely mediated by plasma cells (PCs) that produce copious quantities of antibodies for extended periods of time, even after pathogen clearance. Here, I review the requiremen…

Kidney disease associated with plasma cell dyscrasias.

Plasma cell dyscrasias are frequently encountered malignancies often associated with kidney disease through the production of monoclonal immunoglobulin (Ig). Paraproteins can cause a remarkably diverse set of pathologic patterns in the kidney and recent progress has been made in explaining the molecular mechanisms of paraprotein-mediated kidney injury. Other recent advances in the field include the introduction of an assay for free light chains and the use of novel antiplasma cell agents that can reverse renal failure in some cases. The role of stem cell transplantation, plasma exchange, and k

Genetic aberrations and survival in plasma cell leukemia.

Plasma cell leukemia (PCL) is an aggressive and rare hematological malignancy that originates either as primary disease (pPCL) or as a secondary leukemic transformation (sPCL) of multiple myeloma (MM). We report here the genetic aberrations and survival of 80 patients with pPCL or sPCL and make comparisons with 439 cases of MM. pPCL presents a decade earlier than sPCL (54.7 vs 65.3 years) and is associated with longer median overall survival (11.1 vs 1.3 months; P<0.001). 14q32 (IgH) translocations are highly prevalent in both sPCL and pPCL (82-87%); in pPCL IgH translocations almost exclusive

CT features of abdominal plasma cell neoplasms.

The aim of this study was to describe the CT features of abdominal plasma cell neoplasms. We reviewed CT imaging findings in 11 patients (seven men, four women; mean age 62 years) with plasma cell neoplasms and abdominal involvement. Helical CT of the entire abdomen and pelvis was performed following intravenous administration of contrast material. Images were analyzed in consensus by two radiologists. Diagnoses were made from biopsy, surgery and/or clinical follow-up findings. Multiple myeloma was found in seven patients and extramedullary plasmacytoma in four patients. All patients with mult

Are patch tests useful in plasma cell vulvitis?

OBJECTIVE: To evaluate eventual contact sensitization in 7 patients with plasma cell vulvitis (PCV). STUDY DESIGN: Seven women with PCV underwent patch tests with the Italian standard series and with additional batteries of allergens (perfumes, preservatives, emulsifiers, medicaments and corticosteroid series). RESULTS: One patient experienced a positive reaction t o balsam of Peru and wool alcohol. CONCLUSION: Patch testing is not normally necessary in the management of PCV. However, when a patient with PCV complains of burning or pruritus after application of topical products, patch tests

[Primary plasma cell leukemia. A fulminant case].

Primary plasma cell leukemia is a rare variant of malignant monoclonal gammapathies with de novo presentation in the leukemic phase. Only few series enrolling more than 20 patients have been reported in the literature. The authors deal with a patient who had a fulminant presentation of primary plasma cell leukemia with fatal outcome. Diagnosis of primary plasma cell leukemia was established based on Kyle's criteria, which include an absolute plasma cell count greater than 2 x 10(9)/L or a relative plasma cell number comprising greater than 20% of peripheral blood cells. The patient died within

Intracranial plasma-cell granuloma: a case report.

A 57-year-old woman presented with right-sided ptosis and diplopia. CT scans showed a tumor in the right cerebellopontine angle attached to the tentorium. Subtotal resection was conducted prior to local irradiation (20 Gy). Histological examination of the resected specimen indicated a diagnosis of plasma-cell granuloma. Diffuse thickening of the tentorium and falx were seen on follow-up MR imaging six years after the operation. Only eleven cases of intracranial plasma-cell granuloma have been reported in the literature. This paper reports a new case with unusual recurrence on neuroimagings.

Sjögren's syndrome plasma cell panniculitis and hidradenitis.

We present a 42-year-old woman with primary Sjögren's syndrome and a polyclonal gammopathy who presented with pretibial petechiae, purpura, and tender indurated plaques. The indurated plaques revealed a lobular plasma cell panniculitis, and thus Sjögren's syndrome should be added to the short list of collagen vascular diseases that can present as plasma cell panniculitis. Her biopsies also demonstrated intense perieccrine plasma cell infiltrates that may account for Sjögren's syndrome-associated hypohidrosis. We also observed occasional vascular occlusion of vessels with an amorphous, eosinoph

[Extraosseus plasma cell lymphoma with ileal localization].

The paper reports a case of extraosseous plasmacytic lymphoma in an ileal localisation in a patient with multiple myeloma. After having underlined the relative rarity of tumours of the small intestine and their diagnostic and clinical characteristics, the paper describes the case in question. The unique nature of the case depends on the extreme rarity of cases of isolated secondary forms in patients with active systemic pathologies reported in the literature.

[Plasma cell granuloma of the chest and lung in childhood].

Plasma cell granuloma is a benign, non-neoplastic lesion rarely found in children. It occurs mainly in the chest and lungs, the right lung mostly. Most cases are asymptomatic and are discovered incidentally on routine chest X-ray, although there may have been atypical upper respiratory symptoms. The X-ray findings, as well as those of other imaging modalities, are nonspecific, making the exact localization and diagnosis of the lesion difficult, as demonstrated in the cases of 2 boys aged 8 and 9 years, respectively. Locating the right diaphragm and its relation to the large lesion by various i

Cacogeusia in amyloidosis associated with plasma cell dyscrasia.

Cacogeusia, being the patients' main complaint, is reported as a new symptom of amyloidosis, probably caused by a peripheral sensory neuropathy. A case is presented of a patient with weight loss, dysphagia, macroglossia and taste disturbances, due to amyloidosis associated with a plasma cell dyscrasia. The importance of oral manifestations in amyloidosis is discussed.

Subglottic plasma cell granuloma: report of a case.

A case of subglottic and tracheal plasma cell granuloma masquerading as tumor invasion by thyroid malignancy was successfully treated using CO2 laser. A 55-year-old woman with a history of inspiratory stridor of almost 10 months duration was admitted to our clinic, and examinations revealed a mass in the subglottic space and trachea obstructing the airway. CT scan findings was suggestive of tumor invasion from a thyroid gland malignancy, however, histopathological examination of a biopsy specimen showed it to be plasma cell granuloma. Cases of plasma cell granuloma of the larynx and trachea ar

Plasma cell pneumonia induced by Legionella pneumophila.

We report a case of severe Legionella pneumonia in which the histopathologic features of an open lung biopsy specimen were characterized by a monotypic plasma cell infiltrate and absence of granulocytes or other acute inflammatory cells. The clinical course was otherwise typical for a severe case of Legionella pneumonia with complete functional and roentgenographic recovery following a prolonged convalescence. We speculate on possible reasons for this patient's unusual histopathologic features.

Plasma fibrinolytic activity in sickle cell disease.

Changes in plasma fibrinolytic activity, as measured by euglobulin lysis time (ELT) and serum fibrinogen degradation products (FDP) were investigated in 70 Nigerians with homozygous (HbSS) sickle cell disease (SCD), (50 in stable state and 20 in crisis state) and 75 age-matched non-sicklers. All had normal hemoglobin genotype (HbAA). The levels of ELT and FDP were significantly higher in sicklers in steady state than non-sicklers, but significantly lowered in sicklers in crisis than both non-sicklers and sicklers in stable state. Disturbances of fibrinolytic mechanism in both steady and crisis

Plasma fibrinogen levels in sickle cell disease.

Plasma fibrinogen levels were measured, by a clotweight method, in 25 patients with homozygous sickle cell disease (HbSS) during the steady state and in 36 age matched non-sicklers. Mean fibrinogen levels in the sicklers was 5.3 +/- 2.0 g/l (range 3.0-11.5 g/l) compared with 3.1 +/- 1.0 (range 2.0-5.5 g/l) for non-sicklers (p less than 0.001). There was no significant sex difference between males and females in either group. We suggest the raised fibrinogen seen in SCD may be an added factor in the severity of vascular occlusion during SCD crisis.
