Malignant pleural mesothelioma presenting as low back pain: diagnosed by bone scan coordinating with F-18 FDG PET/CT.

STUDY DESIGN: Case report. OBJECTIVE: We report malignant pleural mesothelioma (MPM) discovered in a Tc-99m MDP bone scan as a photopenic lesion in a 64-year-old man presenting with low back pain and diagnosed with F-18 fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT). SUMMARY OF BACKGROUND DATA: Malignant pleural mesothelioma, an uncommon neoplasm with a poor prognosis, arises from mesothelial cells of the pleura. Typically, the patient presents with either chest pain or symptoms derived from a pleural effusion such as dyspnea, or both. Most cases of MPM ar

Thymic carcinoma with dissemination: a retrospective analysis of ten patients.

OBJECTIVE: Thymic carcinoma is a rare mediastinal neoplasm with frequent pleural or pericardial dissemination. We retrospectively studied ten such cases and analyzed factors that influenced the survival of the patients. METHODS: Ten thymic carcinoma patients with dissemination have been treated since 1987. The clinical and pathological data were retrospectively reviewed. RESULTS: Pretreatment tumor biopsy was performed and demonstrated squamous cell carcinomas in nine and small cell carcinoma in one. In six of ten patients pleural or pericardial dissemination was clinically evident (cT4). Th

Novel platinum agents and mesenchymal stromal cells for thoracic malignancies: state of the art and future perspectives.

INTRODUCTION: Non-small cell lung cancer and malignant pleural mesothelioma represent two of the most intriguing and scrutinized thoracic malignancies, presenting interesting perspectives of experimental development and clinical applications. AREAS COVERED: In advanced non-small cell lung cancer, molecular targeted therapy is the standard first-line treatment for patients with identified driver mutations; on the other hand, chemotherapy is the standard treatment for patients without EGFR mutations or ALK rearrangement or those with unknown mutation status. Once considered an ineffective thera

[Note on the complications of pleural needle biopsy: neoplastic seeding of the wall (apropos of 300 effusions and 440 puncture biopsies)].

Out of 440 pleural biopsies done with Abrams or Castelain needle, performed over 11 years in 300 pleural effusions, only 14 incidents of accidents occurred. There were 9 minor ones (intrapleural bleeding, subcutaneous emphysema, pneumoserosa by aspiration). Neoplastic graft along the biopsy tract was observed in 5 cases, of which 4 were primitive malignant mesotheliomas and one a primitive appearing neoplastic pleurisy. The risk of parietal neoplastic seeding runs high in malignant mesothelioma, considering that 11 such cases were included in this series of biopsied patients. This risk is not

Objective Thoracoscopic Criteria in Differentiation between Benign and Malignant Pleural Effusions.

BACKGROUND: Thoracoscopy is the "gold standard" diagnostic modality for investigation of suspected pleural malignancy. It is postulated that meticulous assessment of the pleural cavity may be adequate to indicate malignancy through the macroscopic findings of nodules, pleural thickening, and lymphangitis. We attempted to critically assess this practice, by precisely defining objective macroscopic criteria which might differentiate benign from malignant pleural diseases according to intrapleural pattern and anatomical location, and thereby to explore the predilection of abnormalities to specifi

Giant Intrapulmonary Solitary Fibrous Tumour With a Possible Liver Metastasis: A Rare Case.

Intrapulmonary solitary fibrous tumours (SFTs) are exceptionally rare mesenchymal neoplasms, particularly when presenting as giant thoracic masses. We report a case of a 57-year-old woman with progressive chest pain and dyspnoea who initially presented with pleural effusion and hypoglycaemia and received empirical anti-tuberculosis therapy without improvement. Contrast-enhanced CT revealed a left lower lobe mass enlarging to 11 × 16.5 × 15.2 cm on repeat imaging, with rightward cardiac displacement, pericardial effusion and a hepatic nodule suspicious for metastasis to the liver. Core needle b

Pulmonary Kaposi's Sarcoma and Its Complications in the HAART Era: A Contemporary Case-Based Review.

The early years of the acquired immunodeficiency syndrome (AIDS) epidemic introduced the global medical community to Kaposi's sarcoma (KS), a heretofore seldom encountered angiosarcomatous neoplasm associated with human herpesvirus-8. At that time, clinicians treating these KS patients were routinely exposed to the pulmonary manifestations of this malignancy, including characteristic airway lesions, peribronchovascular opacities, and the typically hemorrhagic pleural effusions. They also witnessed uncommon complications of pulmonary KS such as chylous effusions, diffuse alveolar hemorrhage, an

Synchronous case of follicular lymphoma and Langerhans cell sarcoma in the same lymph node.

Langerhans cell sarcoma (LCS) is a very rare histiocytic and dendritic cell neoplasm originating from Langerhans cells. There are case reports of histiocytic and dendritic cell neoplasms synchronously or sequentially observed in patients with malignant lymphoma. We present a case in which LCS and follicular lymphoma (FL) grade 3a were observed within the same lymph node. A 66-year-old male visited our hospital with a general malaise. Pleural effusion and systemic lymph adenopathy were detected. Biopsy of an inguinal lymph node was performed. The lymph node had regions with follicular structure

Hemorrhagic intramedullary solitary fibrous tumor of the conus medullaris: case report.

The differential diagnosis of spinal tumors is guided by anatomical location and imaging characteristics. Diagnosis of rare tumors is made c … performed and a diagnosis of solitary fibrous tumor (SFT) was made. SFTs are mesenchymally derived pleural neoplasms, which rarely present at other locations of the body, but have been increasingly described to occur as primary neoplasms of the spine and CNS. The authors believe that this case is unique in its rare location at the level of the conus, and also that …

On the diagnosis of malignant pleural mesothelioma: A necropsy-based study of 171 cases (1997-2016).

BACKGROUND: Malignant pleural mesothelioma (MPM) diagnosis is known to be difficult. We report on the diagnostic elements available in life in an MPM necropsy case series and describe the frequency of non-neoplastic asbestos-related diseases as biological exposure indices. METHODS: We reviewed pathologic and clinical records of an unselected series of autopsies (1977-2016) in patients with MPM employed in the Monfalcone shipyards or living with shipyard workers. We assessed the consistency with autopsy results of diagnoses based on, respectively, radiologic, cytologic, and histologic findings

Pleuropulmonary blastoma in an adult: an initial case report.

BACKGROUND: Pleuropulmonary blastoma (PPB) is a unique dysontogenetic neoplasm of childhood. Its primitive, sarcomatous features are analogous to those of other dysembryonic or dysontogenetic tumors, such as Wilms tumor, hepatoblastoma, neuroblastoma, and embryonal rhabdomyosarcoma. PPB typically presents in young children, most younger than 5 years, as a pulmonary and/or pleural-based tumor with cystic, solid, or combined cystic and solid features. These neoplasms are characterized histologically by primitive mesenchymal or a mixture of primitive and sarcomatous components and generally have

Pleural solitary fibrous tumour with brain metastasis: an aggressive tumour and pathologic conundrum.

Solitary fibrous tumours of the pleura are a rare finding; those with brain metastases are rarer still. Here, we present the evolution of a pleural solitary fibrous tumour in a 70-year-old male treated surgically, and subsequent brain metastasis requiring emergent craniotomy and excision. The patient received adjuvant radiotherapy to the brain and had no recurrence of brain metastases; however, 1 year surveillance imaging demonstrated metastases to the lungs, liver and spleen for which he received chemotherapy but eventually succumbed to the disease process. Solitary fibrous tumours are most o

Thymoma--report of 71 cases and a review.

Of the 71 cases of thymoma reviewed, 31 were benign and 40 were malignant. Associated syndromes were common--myasthenia gravis 51% (36/71), red cell aplasia 3%, hypogammaglobulinemia 4%, mucocutaneous candidiasis 1%, and a number of other autoimmune diseases and neoplasms. These were almost equally distributed among benign and malignant tumors. A plain radiograph showing diffuse involvement of the mediastinum always indicates invasive (malignant) thymoma, while a localized pattern usually suggests a benign lesion (25 of 39 cases). Computerized tomography or MRI often gives additional informati

Sparganosis in the monotremes Tachyglossus aculeatus and Ornithorhynchus anatinus in Australia.

Sparganosis in the echidna, Tachyglossus aculeatus, is characterized by large subcutaneous masses that may be confused with neoplasms. Plerocercoids, believed to be Spirometra erinacei, were found in the ventral or lateral subcutaneous tissues of three echidnas. In each case there was a prominent fibrous mass < or = 12 cm in diameter enclosing plerocercoids, which were surrounded by an intense inflammatory infiltrate dominated by plasma cells and lymphocytes, with focal suppuration. Multiple, small (< or = 2 mm) plerocercoids were present in the lungs and pleural cavity of a fourth echidna tha

[Unusual case of mesothelioma of the tunica vaginalis associated with prostatic adenocarcinoma].

Malignant mesothelioma of the tunica vaginalis testis, is a very rare neoplasm with highly aggressive biological behaviour. It usually occurs in patients aged between 55 and 75 years. A testicular mass is always observed, often accompanied with hydrocele. The response to chemotherapy and radiotherapy is poor. Initial aggressive surgery is necessary. The median survival, without surgical treatment is 23 months. A rare case of malignant mesothelioma of the tunica vaginalis testis, observed in a patient affected by prostate neoplasm is reported. A radical retropubic prostatectomy was performed. T

Sarcomatoid renal cell carcinoma presenting with cannonball pulmonary metastases.

A woman in her 30s without prior medical history presented with 4 months of progressive back pain that evolved to paraplegia, followed by dyspnoea and haemoptysis. Imaging revealed numerous, well-circumscribed pulmonary nodules consistent with cannonball metastases, a loculated right pleural effusion, bilateral renal masses, pancreatic nodules and osteolytic L5 collapse with cord compression. The lung lesion biopsy demonstrated a poorly differentiated neoplasm, which on immunohistochemistry was most consistent with metastatic sarcomatoid carcinoma of renal origin. She required intubation, pleu

First report of pleural mesothelioma treated with cytoreductive surgery and hyperthermic intrathoracic chemotherapy in Kazakhstan: a case report.

BACKGROUND: Malignant pleural mesothelioma (MPM) is a rare but aggressive neoplasm arising from the mesothelial lining of the pleura.Despite advancements in systemic chemotherapy and targeted therapies, the prognosis for MPM remains poor, with a median survival of 8-14 months. Standard treatment modalities include surgery in combination with radiotherapy and/or chemotherapy. Hyperthermic Intrathoracic Chemotherapy (HITOC) has emerged as a promising adjunct to surgery. CASE PRESENTATION: A 35-year-old Asian male was diagnosed with Stage II malignant pleural mesothelioma (T2N0M0). The patient u

Malignant ovarian teratoma in a Lusitano filly: Diagnostic challenges and clinical significance.

An 8-month-old Lusitano filly (a young female horse) was referred with a five-day history of abdominal distension, respiratory distress, and hyporexia. Clinical and ultrasonographic examinations revealed ventral edema, anemia, leukocytosis, hypoalbuminemia, and bilateral pleural effusion. The filly subsequently developed progressive clinical deterioration, including persistent hyporexia, scant fecal output, and acute colic with nasogastric reflux, followed by sudden respiratory decompensation, collapse, and death. Post-mortem examination revealed a large, multilobulated abdominal mass involvin

Clinical and biologic prognostic factors in malignant pleural mesothelioma.

Malignant pleural mesothelioma is an extremely aggressive neoplasm of the pleura mainly attributable to asbestos exposure. Conventional medical, physical, and surgical treatments and their combinations are basically ineffective and just a few subjects experience some benefit. No definite guidelines can be provided in patient selection and therapeutic strategies. Currently, malignant pleural mesothelioma therapy is guided by clinical stage and patient characteristics, which are quite unreliable, rather than by the histological or molecular features of the tumor. In the present review the impact
