Meckel's cave epidermoid with trigeminal neuralgia: CT findings.

An epidermoid tumor of Meckel's cave was found in a middle-aged woman with trigeminal neuralgia. On CT the lesion had negative attenuation numbers of fat and extended from an expanded Meckel's cave through the porous trigeminus into the ambient and cerebellopontine angle cisterns. Surgical excision provided relief of the patient's trigeminal neuralgia.

Gas-CT cisternography for detection of small acoustic nerve tumors.

Gas-CT cisternography is a simple and accurate procedure for detection of small acoustic nerve tumors. Review of one of the largest series in a single institution found that 98% of the studies clearly showed the presence or absence of tumors. The diagnostic pitfalls of the small number of studies in which significant errors were made, or could have been made, are discussed. It is concluded that certain situations appear to call for extra caution: (a) when the filling defect does not show a convex surface, (b) when the amount of cisternal gas is marginal, and (c) when the canal is small. In suc

Empty sella syndrome in a boy with mucopolysaccharidosis type VI (Maroteaux-Lamy).

An 11-year-old boy with mucopolysaccharidosis type VI is presented. Because of optic atrophy, a pneumencephalogram was performed which revealed a relatively large hydrocephalus internus and a very wide, empty sella that filled with air. A Spitz-Holter valve was inserted into a lateral ventricle. STH, TSH, and cortisol response to stress, and thyroid function were normal. LH and prolactin were measurable but close to the lower normal limit. Prolactin did not rise during surgical stress. Those analyses, together with the clinical aspect, rule out severe pituitary malfunction.

The cerebellopontine angle and internal auditory canal: neurovascular anatomy on gas CT cisternograms.

We reviewed 103 normal gas CT cisternograms to delineate the appearance of normal neurovascular structures in the cerebellopontine angle (CPA) and internal auditory canal (IAC). Cranial nerves VII and VIII were identified in the CPA in 97% of cases, either separately (53%) or as a bundle (44%). Intracanalicular branches of the VIIIth cranial nerve were identified in 20% of cases, and cranial nerve V was visualized in the CPA in 14%. The characteristic vascular loop, usually the anterior inferior cerebellar artery, was visible in 35% of cases, and, in 22% of visualized cases, was in an intracan

Idiopathic communicating hydrocephalus: the prognostic significane of ventricular size after shunting.

A description is given of the dramatic, sustained recovery of 3 demented and neurologically disable patients with idiopathic communicating hydrocephalus after they underwent ventriculo-atrial shunting. In those patients there was no significant change in ventricular size after the ventriculo-atrial shunting. 1 of the patients also had cortical atrophy. Hypertensive vasculopathy and multiple infarcts may be the explanation in 2 of the patients. Reveiw of the literature results in the following conclusions: 1) the CAT scan alone, perhaps combined with intrathecal "Amipaque' cisternography, provi

[Neuroradiology and the chiasmatic angle syndrom (author's transl)].

Many diagnostic procedures are available. The main methods are briefly reviewed. Standard X-rays of the skull are always needed. In doubtful cases polytomography is essential. Bone scintigraphy reveals early signs of changes in the bony macrostructures. Brain scintigraphy is still very useful in observing 1. the blood-flow in both intracranial and intraorbital regions, and 2. the cranio-orbital distribution of the radiotracer. Brain scintigraphy is particularly important in detecting meningiomas and vascular lesions. The dynamic study and the comprehensive survey it gives are still not replace

[The diagnostic value of pneumotomoencephalography in patholigic processes in the region of the sella turcica].

The pneumotomoencephalographic procedure applied by the authors in pathological processes in the region of sella turcica is indicated in cases when during pneumocisternography the air fails to fully fill the chiasmal cisternae and it is impossible to obtain a clearcut idea about the presence and the size of the tumour. In such instances an additional introduction of 8--10 ml of air into the ventricular system permits it with a changed position of the patient's head to distinctly see on the tomogram the condition of the anterio-inferior portion of the III ventricle, which is the first to suffer

Supratentorial extracerebral cysts in infants and children.

Twelve cases of supratentorial extracerebral cysts in infants and children are reported. Eight were located in the Sylvian fissure, two in the interhemispheric fissue, and two over the convexity of the cerebral hemispheres. Irrespective of their precise location these cysts, in their common, uncomplicated form, give rise to a clinical syndrome different from that recorded in older patients, with a symmetrical macrocrania of a severe degree unassociated with any neurological signs or abnormalities in psychomotor development. Extensive unilateral transillumination of the skull is common (six cas
