Airway obstruction during pneumonectomy using a single lumen tube: A case report.

RATIONALE: Endotracheal intubation is an essential step for airway management during general anesthesia. When surgeons carry out thoracic surgery such as pneumonectomy, they usually request lung isolation to secure a clear surgical view. A double lumen endotracheal tube is used for lung isolation in routine thoracic surgeries. PATIENT CONCERNS: A 56-year-old man was previously diagnosed with left Aspergilloma, a tuberculosis destroyed lung, and diabetes mellitus. According to his chest x-ray and chest computed tomography, his left lung was nearly collapsed, and the result of a pulmonary funct…

Low Thoracic Skeletal Muscle Area Predicts Morbidity After Pneumonectomy for Lung Cancer.

BACKGROUND: Sarcopenia represented by low psoas muscle area is associated with increased hospital length of stay (LOS), postoperative complications, and mortality. We studied whether thoracic skeletal muscle area (TSMA) derived from computed tomography (CT) predicts morbidity after pneumonectomy for lung cancer. METHODS: Consecutive patients who underwent pneumonectomy for lung cancer from 2005 to 2017 were retrospectively analyzed. TSMA was defined as the sum of muscle area at the level of the eighth and the 12th thoracic vertebral bodies on preoperative CT. Patients were stratified into sex…

A Cranial-Sided Approach for Repeated Mitral Periprosthetic Leak After Right Pneumonectomy.

A 72-year-old man presented with worsening dyspnea on effort. He underwent right pneumonectomy 40 years ago, then mitral valve replacement through a right thoracotomy 8 years ago with repeat surgery to repair a periprosthetic valve leak; the mediastinum was displaced to the right, and the heart was rotated counterclockwise. Transthoracic echocardiography showed periprosthetic valve leak recurrence near the left atrial appendage. We repaired the periprosthetic valve leak through a median sternotomy. Transecting the main pulmonary artery allowed us to widely open the cranial-sided left atrium. W…

Multimedia article. Entirely thoracoscopic pneumonectomy using the prone position: a new technique.

BACKGROUND: Reports on video-assisted pneumonectomy have remained scarce, despite early demonstration of its technical feasibility. A totally videothoracoscopic pneumonectomy was first reported by Conlan and Sandor. The patient in this report was positioned in the full lateral position. In this video, we report a totally videothoracoscopic left-side pneumonectomy with the patient in prone position. METHODS: A 49-year-old man was admitted to our hospital for a bifocal cancer of the left lower lung lobe (LLL) and the cervical esophagus. The preoperative workup included a chest computed tomograp…

[Cardiac arrhythmia after lung cancer surgery--an analysis of 140 pneumonectomy cases].

Among 808 surgically resected lung cancer cases at Shanghai Chest Hospital during the period of January 1991 to December 1992, 140 of them (17.3%) underwent pneumonectomy (47 right, 93 left). In the 140 cases: 124 male, 16 female; aged 27-74 yrs, pre-operative abnormal EKG was found in 38 cases. Postoperatively, arrhythmia occurred in 76 cases, with an incidence of 54.3%. Of the 76 arrhythmias, atrial fibrillation was seen in 7 patients, atrial premature beats in 2, supraventricular tachycardia in 1 and sinus tachycardia in 66. The results indicate that cardiac arrhythmia is a common complicat…

Bronchial stump disruption following inadvertent right mainstem intubation 9 years after pneumonectomy.

An 82-year-old man, with a history of right pneumonectomy 9 years prior to admission, was emergently intubated for respiratory failure resulting from rapid atrial fibrillation and congestive heart failure. Postintubation chest radiograph demonstrated the tube to be in the right bronchial stump. Over the next 48 hours gas exchange remained poor, endotracheal suctioning produced copious serosanguinous secretions, and chest radiographs showed increasing air, with air fluid levels, in the previously opacified right hemithorax. Bronchoscopy demonstrated air bubbling from the posterior wall of the r…

[Esophago-thoracic fistula after pneumonectomy and long-term main bronchus stump fistula].

A woman patient admitted for treatment, who had reached the age of 65 years, had a previous history of an open pulmonary tuberculosis in 1946 that had been treated at that time by means of collapsotherapy and phrenico-exeresis. Pleuropneumonectomy was performed in 1976 because of a residual cavity of a thoracic empyema. Since 1978 the patient suffered from a fistula of the bronchus treated by postural therapy without achieving a cure. In 1989 a fistula formed between oesophagus and pneumonectomy cavity. Clinically this was associated with an increasing reduction of performance and a suddenly i…

Induction chemoradiotherapy to avoid pneumonectomy in centrally located non-small cell lung cancer.

OBJECTIVES: Pneumonectomy is associated with substantial perioperative morbidity and long-term cardiopulmonary function impairment. Therefore, avoiding pneumonectomy whenever oncologically feasible is an important goal in managing centrally located non-small cell lung cancer. This study evaluated the feasibility and oncologic outcomes of induction chemoradiotherapy administered to avoid pneumonectomy. METHODS: We retrospectively reviewed patients with centrally located non-small cell lung cancer initially considered candidates for pneumonectomy but underwent induction chemoradiotherapy to red…

Bronchial stump re-resection for carcinoid tumor recurrence after left pneumonectomy: Case report.

Carcinoid tumors of the lung are rare, low-grade neoplasms within the neuroendocrine tumor spectrum. They are classified as typical and atypical carcinoids. Typical carcinoids are predominantly centrally located and rarely metastasize, whereas atypical carcinoids more commonly occur in peripheral locations and have a higher propensity for distant metastasis. Most patients benefit from curative surgical treatment. In this case report, we describe a patient who previously underwent left pneumonectomy for a typical carcinoid tumor and subsequently developed a local recurrence requiring re-resecti…

Nutrition management of a patient following emergent pneumonectomy due to chest wall trauma.

Emergent total pneumonectomy is a rare surgical intervention for patients with severe chest trauma. Patients who survive the immediate postoperative period experience prolonged, complex hospitalizations. The purpose of this case study is to review the nutrition care provided to a patient who survived total pneumonectomy and the supporting evidence. John Doe (JD) is a man aged 28 years who presented to a level I trauma center with penetrating chest trauma. He required multiple operative interventions, resulting in a partial right and total left pneumonectomy. JD's hospitalization was complicate…

Central airway carcinoid tumor mimicking chronic asthma and necessitating pneumonectomy: A case report.

INTRODUCTION: Central airway tumors can occasionally be misdiagnosed as a chronic disease. We present a case of a central airway carcinoid t … e tumor to allow some patency to the left lung. After stabilization, he subsequently underwent left pneumonectomy. He recovered well and was discharged home on postoperative day 2. On surveillance 2.5 years after pneumonectomy, he has resumed bodybuilding and has no evidence of recurrent disease. DISCUSSION: Proximal airway tumors can mimic asthma. Careful man …

A pneumonectomy model to study flow-induced pulmonary hypertension and compensatory lung growth.

In newborns, developmental disorders such as congenital diaphragmatic hernia (CDH) and specific types of congenital heart disease (CHD) can lead to defective alveolarization, pulmonary hypoplasia, and pulmonary arterial hypertension (PAH). Therapeutic options for these patients are limited, emphasizing the need for new animal models representative of disease conditions. In most adult mammals, compensatory lung growth (CLG) occurs after pneumonectomy; however, the underlying relationship between CLG and flow-induced pulmonary hypertension (PH) is not fully understood. We propose a murine model …

Caustic ingestion leads to pneumonectomy and right colonic interposition, a unique case report.

INTRODUCTION: Corrosive ingestion forms serious problem, with various outcomes depending on the time of diagnosis and treatment. We report here a case with rare and dangerous complications. PRESENTATION OF CASE: A two-year-old girl came to our hospital, complaining of solids' dysphagia. Retrieving her medical history showed that she had ingested a corrosive liquid accidentally. Without knowing the nature of the ingested agent, the local doctor removed it, using nasal gastric tube. This procedure induced vomiting, which in turn led to more damage. She stayed in the area's hospital for 40 days

[Pneumonectomy for Non-small Cell Lung Cancer: Predictors of Operative Mortality and Survival].

Surgery is the most effective way to cure non-small cell lung cancer currently. Although sleeve lobectomy, with less reduction of respiratory function and a lower mortality rate, would not compromise oncological results in well-selected patients with central lung cancer, pneumonectomy is still necessary when complete resection could not be achieved by other surgical procedures. Cardiopulmonary complications are the most common complication after pneumonectomy. Fully understanding the related complications after pneumonectomy can help surgeons make a diagnosis timely, and further take relevant …

Combined approach using uniportal video-assisted thoracoscopic surgery in left tracheal sleeve pneumonectomy.

Left tracheal sleeve pneumonectomy is rare and challenging surgical procedure with high postoperative complication and mortality rates. We describe the least invasive single-stage surgical approach using uniportal video-assisted thoracoscopic surgery (VATS) currently successfully applied for a left centrally located bronchogenic carcinoma with tracheal involvement. This case shows how uniportal VATS can be effectively used in challenging cases for reducing invasiveness without compromising patient safety nor oncological principles of complete surgical resection.
