[Ultrasonographic visualization of a Baker-cyst as cause of a peroneal nerve palsy in a patient with rheumatoid arthritis].

With this report we present a patient with rheumatoid arthritis in whom a popliteal cyst could be diagnosed quickly and conclusively by ultrasonography as the possible reason for peroneal palsy. The sonographic investigation of the knee joint revealed an effusion. Within the popliteal fossa (dorsal longitudinal and transverse section) a cyst measuring 54 x 21 x 51 mm in a typical dorsomedial position in between the heads of the gastrocnemius muscles was found. Besides extensive synovectomy, we completely removed the cyst. The common peroneal nerve was entrapped by the Baker's cyst. The complet…

Ultrasound evaluation of the popliteal space. Comparison with arthrography and physical examination.

Ultrasound scanning techniques were used to examine the popliteal space in 102 knees. In 30 of 34 knees in which arthrograms were also obtained, the information obtained from both tests was the same. In two instances in which there were different results, the presence of palpable popliteal cysts was confirmed by ultrasound scans but not by arthrography, and in two other cases, small asymptomatic cysts were seen on arthrograms but not on ultrasound scans. In the 68 other knees, ultrasound scans were helpful in the differential diagnosis of popliteal cysts, popliteal artery aneurysms, thrombophl…

Adventitial cystic disease of the popliteal artery.

We describe a patient with adventitial cystic disease of the popliteal artery with intermittent claudication involving the right calf during exercise. Magnetic resonance imaging (MRI) and computed tomography (CT) revealed a cystic lesion that encircled and compressed the popliteal artery. Resection of the cyst involving a segment of the affected popliteal artery and interposing an autologous vein graft resolved the symptoms, and the postoperative course was uneventful. The cyst was histologically similar to a ganglion.

Calf pain and cystic adventitial disease of the popliteal artery.

In a 61 year-old women, suffering a left calf pain for more 18 months, a diagnosis of cystic adventitial disease of the popliteal artery was confirmed by scanner. At the operation, we found an intramural cyst of the popliteal artery. A total resection of the cyst and of the artery (resection and end to end anastomosis) was performed. Adventitial cystic disease represents 0, 1% of the vascular diseases). The popliteal artery seems to be the most frequent site.

Episodic intermittent claudication associated with a Baker's cyst.

Baker first described synovial cysts in the leg in connection with diseases of the knee joint. Whilst the majority are asymptomatic, some present as a localised swelling behind the knee, and others either compress the popliteal vein or rupture and can be misdiagnosed as deep vein thrombosis; transmitted pulsation through the cyst can suggest the presence of a popliteal aneurysm. This paper documents a case of intermittent claudication of the lower limb secondary to compression of the popliteal artery by a Baker's cyst.

Sonography of popliteal masses.

Sonography of the popliteal space was performed in 28 patients with a history of a popliteal mass, verified by physical examination in all but 7 patients. The final diagnosis was established by needle biopsy or surgery. Three main types of lesions were found sonographically. A thin-walled echo-free cyst was present in 16 patients. The diagnosis in all of these cases was Baker's cyst. Two patients had a mixed cystic and solid lesion, which was found to be a Baker's cyst, with a loose body in 1 case and a soft-tissue sarcoma with hematoma in the other. Three patients showed solid lesions subsequ

[A swelling in the knee cavity, not caused by a Baker's cyst but by a nerve sheath tumour].

In a woman, aged 30 years, who presented with a right popliteal mass, a Baker's cyst (popliteal cyst) was diagnosed. Five years later she developed symptoms that were attributed to compression of the peroneal nerve by the mass. Because of the troublesome nature of these symptoms, it was decided to excise the mass. Immediately after the operation, a complete loss offunction ofthe common peroneal nerve was apparent. Histopathologic examination revealed an ancient schwannoma (nerve sheath tumour). The most important reason for not having diagnosed the schwannoma was that it was not considered in …

[The pseudothrombophlebitic syndrome: a new diagnostic approach].

The clinical manifestation of popliteal or Baker's cysts, either intact or complicated due to rupture or dissection, can be a thrombophlebitis (pseudothrombophlebitis), greatly difficulting the differential diagnosis and the management of these patients, being their treatment completely different. The doppler echography is a reliable, quick and low-cost method which allow to differentiate between this two types of pathology and the magnetic resonance (MR) is the ideal technique to assess the complications of popliteal cysts. We present a case of pseudothrombophlebitis due to compression of the…

Nonoperative management of lower extremity claudication caused by a Baker's cyst: case report and review of the literature.

A Baker's or popliteal cyst is a synovial cyst in the popliteal fossa arising from the knee joint. The majority of patients develop a popliteal mass that is asymptomatic, but in a small percentage of patients, complications and symptoms occur; these may not only encompass the popliteal veins and arteries but may also include cyst leakage, infection, hemorrhage, and compartment syndrome. Severe lower limb ischemia caused by a Baker's cyst is extremely rare, having been reported only six times since 1960; all patients were treated with surgical intervention. We report the case of a 29-year-old m…

Successful endovascular management of popliteal artery adventitial cystic disease with expanded polytetrafluoroethylene stent graft reinforced with interwoven nitinol stent.

Popliteal adventitial cystic disease has traditionally been treated with open surgery of cyst excision, patch repair of artery, or interposition graft. Endovascular angioplasty or stenting has demonstrated poor mid- to long-term results because high-viscosity tension of the adventitial cyst continues to exert extrinsic compression, resulting in early elastic recoil and disease recurrence. To our knowledge, this is the first report in the literature of an effective endovascular strategy using a popliteal stent graft reinforced with an interwoven nitinol stent to treat popliteal cysts, with favo…

Magnetic resonance arthrographic demonstration of knee joint communication in isolated popliteal artery cystic adventitial disease.

Cystic adventitial disease is a rare, nonatherosclerotic condition most commonly affecting the popliteal artery in the popliteal fossa. We describe the case of a 44-year-old man with episodic calf claudication in whom duplex ultrasound and computed tomography angiography showed cystic lesions surrounding the popliteal artery with mild luminal narrowing, consistent with cystic adventitial disease. Conventional magnetic resonance (MR) imaging identified a popliteal artery adventitial cyst with suspected knee joint connection. Fluoroscopically guided intra-articular gadolinium injection in the kn

[A man with a swelling of the knee].

A 72-year-old male presented with a dorsomedial swelling of the knee. X-rays showed isolated ossificated cartilage particles. The particles, which were found to be in a popliteal cyst, were surgically removed and analyzed by the pathologist. The diagnosis was synovial osteochondromatosis . Isolated synovial osteochondromatosis inside a popliteal cyst is uncommon.

Endoscopic Resection of Recalcitrant Tuberculous Bursitis of the Popliteal Fossa.

Tuberculosis of the musculoskeletal system is uncommon and constitutes 1% to 5% of all forms of tuberculosis. In total, 30% of skeletal tuberculosis involves the joints, the knee being the third most commonly affected after the spine and the hip. Knee tuberculosis commonly presents as synovitis or arthritis, and infected Baker's cyst is a rather rare presentation. It is believed to result from the propagation of tuberculosis of the knee joint into the cyst, as approximately one half of the Baker's cyst communicates with the knee joint and it appears as swelling in the medial aspect of the popl

[The diagnosis of cystic degeneration of the adventitia (author's transl)].

The clinical picture of cystic degeneration of the adventitia is illustrated by two cases. This usually occurs in a portion of an artery near a joint, most commonly in the popliteal artery. The diagnosis is made in the presence of irregular, intermittent claudication associated with an "hour glass" stenosis of the popliteal artery on the arteriogram, without displacement of the vessel. The aetiology is probably related to herniation of the synovia from the adjacent joint capsule; this explains not only the variable pressure within the cyst in the arterial wall, but also the connection found at

Epidermal inclusion cyst of the knee.

Epidermoid cysts are asymptomatic, slowly enlarging, firm to fluctuant, dome-shaped lesions. Epidermoid cysts frequently appear on the trunk, neck, face, and scrotum, behind the ears and in the palmoplantar region. We review all the cases of epidermoid cyst of the knee and present a case of non-traumatic-induced epidermal cyst in the popliteal fossa of a 66-year-old male with 10-year history of right knee swelling, which appeared firstly as a small mass 2 × 2 cm in the popliteal aspect of the knee. The mass was increasing gradually in size until 2 years ago when it increased suddenly to gain t

Cystic adventitial disease of the popliteal artery: is there a consensus in management?

Cystic adventitial disease (CAD) is a well described disease entity that commonly affects the popliteal artery, presenting as a rare cause of non-atherosclerotic claudication. The traditional surgical approaches are cyst resection and bypass, or cyst evacuation or aspiration. We report the case of a 58-year-old female with CAD of the popliteal artery treated successfully with cyst resection and bypass using an autologous graft. We reviewed the literature over the last 25 years on management and outcomes of CAD of the popliteal artery. We identified a total of 123 cases; most cases were treated

Transmission and molecular characterisation of wild measles virus in Romania, 2008 to 2012.

PURPOSE: This study aimed to evaluate the efficacy and safety of arthroscopic internal drainage for popliteal cysts using the figure-of-four position. METHODS: Between January 2016 and June 2024, 61 patients with symptomatic popliteal cysts underwent arthroscopic internal drainage with the knee positioned in a figure-of-four, while intra-articular lesions were managed with synovial debridement, meniscal repair/formation, and removal of loose bodies. Preoperative magnetic resonance imaging confirmed the presence of popliteal cysts and concomitant intra-articular lesions. Operation time and int…

Detecting Baker's cyst: venous duplex scanning in ambulatory setting.

Popliteal cyst, also called Baker's cyst, is a popliteal fossa enlargement filled with synovial fluid. Baker's cysts can be symptomatic and cause considerable pathologies such as thrombophlebitis, compartment syndrome and even nerve entrapment. It is the most common nonvascular pathology seen in the popliteal fossa but clinically indistinguishable from deep vein thrombosis. The aim of the present study was to evaluate venous duplex scanning in detecting and distinguishing complicated Baker's cyst and deep vein thrombosis in outpatient setting. Medical records of all patients undergoing venous …

An unusual loose body in the knee.

In a 30-year-old man, the movement of a loose body between the knee joint and a popliteal cyst caused difficulty in locating the loose body. The first roentgenogram showed a posteromedial opacity, which was thought to be a sesamoid. On a subsequent film, a loose body appeared in the intercondylar notch and the posteromedial opacity was absent. At arthroscopy, the loose body could not be found within the joint and a peroperative radiograph showed that the opacity had returned to its original position. The loose body was visualized with the arthroscope inserted via a posteromedial portal and pas…
