Treatment of postmenopausal osteoporosis with odanacatib.

INTRODUCTION: The market of anti-osteoporosis drugs has been declining in recent years, possibly in part due to the publicity around adverse events observed with bisphosphonates. Also, the proportion of patients with clinical fracture who receive adequate treatment remains low. So there are still unmet needs in this field. Odanacatib is a cathepsin K inhibitor currently being developed for the treatment of postmenopausal osteoporosis that could be an advance in this context. AREAS COVERED: Odanacatib is a bone resorption inhibitor, but it preserves some degree of bone formation, which differe

Recognition and treatment of postmenopausal osteoporosis.

Osteoporosis is a common but often silent condition among older adults. The characteristic pathological changes associated with osteoporosis may go unnoticed until a fracture occurs or a bone density test is performed. Although osteoporosis occurs in men and among premenopausal women, it is most prevalent among postmenopausal women who are the focus of this article. Within the subpopulation of postmenopausal women, there are major differences in risk status, disease severity, and response to treatment. Two individual examples are presented to highlight these differences and to illustrate appro

An overview on the treatment of postmenopausal osteoporosis.

Osteoporosis is a worldwide health problem related to the aging of the population, and it is often underdiagnosed and undertreated. It is related to substantial morbidity, mortality and impairment of the quality of life. Estrogen deficiency is the major contributing factor to bone loss after menopause. The lifetime fracture risk at 50 years of age is about 50% in women. The aim of the treatment of osteoporosis is to prevent fractures. Non-pharmacological treatment involves a healthy diet, prevention of falls, and physical exercise programs. Pharmacological treatment includes calcium, vitamin D

An unusual cause of postmenopausal bleeding.

A 68-year-old postmenopausal woman presented to the gynaecology clinic with a 1-week history of vaginal bleeding. She was investigated to rule out local and systemic causes including genital malignancy. The investigations were negative for genital malignancy and her symptoms settled spontaneously. The bleeding was attributed to a corticosteroid treatment for shoulder joint pain. A short literature review of this rare side effect is discussed.

Prevention and treatment of postmenopausal osteoporosis.

In the beginning, that is from the 1960's, when a link between menopause and osteoporosis was first identified; estrogen treatment was the standard for preventing bone loss, however there was no fracture data, even though it was thought to be effective. This continued until the Women's Health Initiative (WHI) study in 2001 that published data on 6 years of treatment with hormone therapy that showed an increase in heart attacks and breast cancer. Even though the risks were small, 1 per 1500 users annually, patients were worried and there was a large drop off in estrogen use. In later analyses t

[Denosumab for treatment of postmenopausal osteoporosis].

BACKGROUND: Treatment with bisphosphonates reduces the risk of new fractures and is the treatment of choice for osteoporosis. Denosumab inhibits bone resorption via a different mechanism than bisphosphonates, and is a new option in the treatment of osteoporosis. In this paper we give an overview of the mode of action and clinical effects. MATERIAL AND METHODS: The paper is based on a non-systematic literature search in Pubmed/Medline. RESULTS: Denosumab is a human monoclonal antibody to receptor-activated nuclear factor kappa B (RANKL), a member of the TNF family that is formed in the osteob

Postmenopausal osteoporosis and tooth loss.

OBJECTIVE: The aim of this study was to determine relation between tooth loss and general body bone mineral density in postmenopausal female who were seeking for prosthetic treatment. MATERIAL AND METHODS: There were included 79 women in this study (age from 49-81 years, mean age 62.9 years) with partial tooth loss. For all patients bone mineral density measurements for lumbar spine and both femoral necks by dual energy X-ray absorptiometry (Lunar DEXA DPX-NT, GE Medical Systems) were performed. Based on DEXA results patients were divided into 3 groups: normal bone density (T-score ≥-1.0), os

How to manage postmenopausal osteoporosis?

Osteoporosis is characterized by reduced bone mass and disruption of bone architecture, resulting in increased fracture risk. Several therapeutic agents are now available to treat postmenopausal osteoporosis and prevent fractures. Combined calcium and vitamin D supplementation reduce the relative risk of non-vertebral fractures by about 18%. Hormone replacement therapy (HRT) should not be prescribed for osteoporosis in women who do not experience menopausal symptoms. The marked benefits of raloxifene on the reduction in invasive breast cancer and vertebral fracture risk are partially counterba

Obesity and fractures in postmenopausal women.

Low body mass index (BMI) is a recognized risk factor for fragility fracture, whereas obesity is widely believed to be protective. As part of a clinical audit of guidance from the National Institute of Health and Clinical Excellence (NICE), we have documented the prevalence of obesity and morbid obesity in postmenopausal women younger than 75 years of age presenting to our Fracture Liaison Service (FLS). Between January 2006 and December 2007, 1005 postmenopausal women aged less than 75 years with a low-trauma fracture were seen in the FLS. Of these women, 805 (80%) underwent assessment of bon

Denosumab for the management of postmenopausal osteoporosis.

View the National Osteoporosis Foundation Clinician's Guide Postmenopausal osteoporosis is a major concern to public health. Fractures are the major clinical consequence of osteoporosis and are associated with substantial morbidity, mortality, and health care costs. Despite the availability of screening and treatment guidelines, osteoporosis diagnosis and treatment remain low. Health care providers may consult guidelines in the clinical management of their patients with osteoporosis, including those from the National Osteoporosis Foundation, and the new fracture risk assessment tool from the W

Ospemifene use in postmenopausal women.

BACKGROUND: Selective estrogen receptor modulators (SERMs) are structurally different compounds that interact with intracellular estrogen receptors in target organs as estrogen agonists and antagonists. These drugs have been intensively studied over the past decades and have proven to be a highly versatile group for the treatment of different conditions associated with menopause, including hormone-responsive cancer and osteoporosis. However, currently available SERMS are also responsible for side effects such as thromboembolic disorders, or gynecological symptoms (especially vaginal dryness an
