Gabapentinoid Pharmacology in the Context of Emerging Misuse Liability.

This article will review the epidemiology and pharmacology of gabapentinoids (gabapentin and pregabalin) relevant to their emerging misuse potential and provide guidance for clinical and regulatory management. Gabapentinoids are γ-aminobutyric acid analogues that produce their therapeutic effects by inhibiting voltage-gated calcium channels and decreasing neurotransmitter release. Recently gabapentinoid prescribing and use have increased tremendously. Although traditionally thought to possess a favorable safety profile, gabapentinoid misuse has also risen significantly. Gabapentinoid misuse ge

Comparing Medical Comorbidities Between Opioid and Cocaine Users: A Data Mining Approach.

BACKGROUND: Prescription drug monitoring programs (PDMPs) are instrumental in controlling opioid misuse, but opioid users have increasingly shifted to cocaine, creating a different set of medical problems. While opioid use results in multiple medical comorbidities, findings of the existing studies reported single comorbidities rather than a set, and furthermore, those findings are often conflicting because of the lack of controlling for other substances in the analysis when combined use of substance creates synergistic effects. On the other hand, the findings from cocaine use are mainly relate…

The Effect of Nonsteroidal Anti-Inflammatory Drug Contraindications on Opioid Use, Length of Stay, and Patient-Reported Outcomes After Lumbar Fusion.

OBJECTIVE: To assess opioid use, length of stay (LOS), and patient-reported outcome measures (PROMs) among patients undergoing lumbar fusion with contraindications to nonsteroidal anti-inflammatory drugs (NSAIDs) compared to those without any contraindications. SUMMARY OF BACKGROUND DATA: Nonsurgical patients with NSAID contraindications have an increased association with opioid use disorder. There is a lack of research examining long-term pain medication trends among surgical patients with NSAID contraindications, specifically before and after lumbar fusion. METHODS: Patients aged 18 years

Real or fake? Sourcing and marketing of non-prescribed benzodiazepines amongst two samples of people who regularly use illicit drugs in Australia.

INTRODUCTION: There is concern around non-prescribed benzodiazepine use, particularly with increasing detections of counterfeit products containing high-risk novel compounds. The aims of this study were to investigate how and which non-prescribed benzodiazepines are being sourced; forms, appearance and packaging; and awareness of risks associated with non-prescribed benzodiazepines. METHODS: Data were collected from a sample of Australians who inject drugs or use ecstasy and/or other illicit stimulants on a monthly or more frequent basis, and who reported past 6-month use of non-prescribed be

Development and validation of a 'dilute and shoot' LC-MS/MS method in urine suitable for screening and confirmation analyses in a clinical setting.

Urine toxicological analysis serves as a significant tool in both clinical and forensic contexts, facilitating the diagnosis of acute intoxications, determination of causes of death, monitoring of substance use in occupational settings, and identification of drug-facilitated crimes. In this regard, the dilute-and-shoot method, when integrated with liquid chromatography-tandem mass spectrometry (LC-MS/MS), represents a promising analytical approach due to its efficacy, reliability, and wide-ranging applicability. This study presents the development and validation of an LC-MS/MS method that simu

Forecasting drug utilization and expenditure in a metropolitan health region.

BackgroundNew pharmacological therapies are challenging the healthcare systems, and there is an increasing need to assess their therapeutic value in relation to existing alternatives as well as their potential budget impact. Consequently, new models to introduce drugs in healthcare are urgently needed. In the metropolitan health region of Stockholm, Sweden, a model has been developed including early warning (horizon scanning), forecasting of drug utilization and expenditure, critical drug evaluation as well as structured programs for the introduction and follow-up of new drugs. The aim of this

Opioids in headache treatment. Is there a role?

There may be a population of patients subject to frequent headache and in whom optimal analgesic effect is obtained only by frequent but controlled use of opiate drugs and in whom adverse drug effects are minimal. It is emphasized again that the reality is that there are currently a large amount of opioids being prescribed for headache patients because of patients' demands. One of the major considerations for physicians prescribing such treatment is familiarity with the legal guidelines. The federal law requires physicians to register if they are to maintain or detoxify with opioids addicts de

Does Postoperative Physical Therapy Impact Pain, Opioid Consumption, and Clinical Outcomes After Single-Level Lumbar Fusion?

STUDY DESIGN: Retrospective cohort. OBJECTIVE: To determine the impact of physical therapy (PT) on opioid consumption, surgical outcomes, and patient-reported outcome measures (PROMs) following single-level lumbar fusion. SUMMARY OF BACKGROUND DATA: Physical therapy following lumbar fusion surgery is variably prescribed, with the goals of pain relief and return to activity. However, existing evidence on the efficacy of postoperative PT after spine surgery is heterogeneous and generally low quality. Furthermore, the impact of PT on opioid use following lumbar fusion has been minimally studied

Stage-of-change Assessment Predicts Short-term Treatment Engagement for Opioid Use Disorder Patients Initiated on Buprenorphine.

INTRODUCTION: The emergency department (ED) is an effective setting for initiating medication for opioid use disorder (MOUD); however, predicting who will remain in treatment remains a central challenge. We hypothesize that baseline stage-of-change (SOC) assessment is associated with short-term treatment retention outcomes. METHODS: This is a longitudinal cohort study of all patients enrolled in an ED MOUD program over 12 months. Eligible and willing patients were treated with buprenorphine at baseline and had addiction medicine specialist follow-up arranged. Treatment retention at 30 and 90

Comparison of Postoperative Opioid Use After Anterior Cervical Diskectomy and Fusion or Posterior Cervical Fusion.

INTRODUCTION: Posterior cervical fusion (PCF) and anterior cervical diskectomy and fusion (ACDF) are two main surgical management options for the treatment of cervical spondylotic myelopathy. Although ACDF is less invasive than PCF which should theoretically reduce postoperative pain, it is still unknown whether this leads to reduced opioid use. Our objective was to evaluate whether PCF increases postoperative opioid use compared with ACDF. METHODS: We retrospectively identified all patients undergoing 2-level to 4-level ACDF or PCF at a single center from 2017 to 2021. Our state's prescripti

Opioid consumption in the first 30 days after surgery was independently associated with new persistent opioid use.

INTRODUCTION: Previous studies suggest that new persistent opioid use (NPOU) after surgery was associated with larger perioperative opioid prescriptions, but the association between NPOU and postoperative opioid consumption is unknown. METHODS: This retrospective study included opioid naïve individuals aged 18-64 who underwent surgical procedures across 70 Michigan hospitals between July 1, 2018 and November 15, 2021 and were prescribed opioids at discharge. We used clinical and patient-reported opioid consumption data from the Michigan Surgical Quality Collaborative, a statewide surgical reg

Adherence to national pediatric antibiotic prescription guideline for pneumonia treatment of children 2-59 months: a retrospective chart review at a regional hospital in Rwanda.

BACKGROUND: Inappropriate use of antibiotics is a major driver of antimicrobial resistance and can lead to adverse healthcare outcomes. Treatment guidelines promote rational prescribing and help minimize healthcare costs. We aimed to assess adherence to the national pediatric treatment guideline for pneumonia among healthcare providers at a major rural hospital in Rwanda. METHODS: A retrospective chart review was conducted to audit clinical records of children aged 2–59 months hospitalized with pneumonia between January 2022 to January 2024. Data on the classifications of pneumonia, prescribed

Holmium Laser Enucleation of Prostate: What is the True Rate of Postoperative Opioid Use?

OBJECTIVES: To determine if patients were obtaining opioids after HoLEP from other sources - despite our opioid-free postoperative pathway - we utilized a national prescription drug monitoring program (PDMP) to review all patients who underwent HoLEP at our institution. METHODS: We performed a retrospective review of all HoLEPs completed by two fellowship-trained surgeons. We utilized a national PDMP to determine the true rate of postoperative opioid use. The primary outcome was filling of an opioid prescription within 31 postoperative days. Student t-tests and chi-square tests were used to c

Effects of the Communities That HEAL intervention on initiation, retention, and linkage to medications for opioid use disorder (MOUD): A cluster randomized wait-list controlled trial.

Medications for opioid use disorder (MOUD) can reduce opioid use and overdose deaths. This study examined whether the Communities That HEAL (CTH) intervention increased MOUD initiation, retention, and linkage. The HEALing Communities Study was a multi-site, 2-arm, parallel, community-level, cluster-randomized, unblinded, wait-list controlled trial conducted in 67 communities (n = 34 intervention, n = 33 control). Using Prescription Drug Monitoring Programs and Medicaid claims data, we compared mean community-level rates of MOUD outcomes during the 1-year comparison period (July 2021-June 2022)…

Unforeseen uses of oral contraceptive pills: Exploratory study in Jordanian community pharmacies.

BACKGROUND: The use of oral contraceptive pills (OCPs) as a birth control method is very common worldwide. OCPs have many other labeled non-contraceptive indications, and as a result there is an associated risk of improper use, as with any other medications. This study was designed to assess the unforeseen improper uses of OCPs observed by community pharmacists in Jordan. METHOD: A cross-sectional study design was conducted using a self-administered survey. A convenience sample (n = 380) of Jordanian community pharmacists, were recruited through social media resources. The survey included mul
