The authors report a retrospective study collecting ten cases of priapism over fifteen years. Clinical diagnosis is evident. Idiopathic priapism is most frequent of all the medical treatments available, no one seems to be efficient. Among surgical procedures draining the venous blood from the cavernous bodies, spongiocavernous shunt (Al-Ghorab) has been a simple, fast and effective method in the last five patients. Functional result depends on the age and especially on the precocity of the surgical treatment. Therefore priapism is an andrologic emergency.

We report 16 cases of priapism in 13 of 404 patients who received intracavernous injection of vasoactive drugs to induce erection. In 10 patients the complication presented following injection of papaverine and phentolamine in combination. Despite reducing the dose of both agents, one patient had another episode of priapism. One patient who had been receiving papaverine alone in increasing doses with no results, developed priapism after the first injection of the combined treatment modality and again after the dose of both drugs had been reduced. Priapism may be primary (idiopathic) or secondary to sickle cell anemia, trauma, leukemia, drugs, venous thromboembolic diseases, and other less common disorders. This study concerns 21 patients with priapism treated during a period of ten years. Nine patients (43%) had sickle cell anemia. The authors demonstrate the management of erectile impotence by using Papaverin or the combination of Papaverin-Regitin and its overdosing. The overdosing should be put in mind when using these drugs. The authors also demonstrate three types of priapisms. We present a rare case of priapism in a child, ten years old, in association with Fabry's disease. The child had a history of disseminated nodular enlargement, crises of fever, intermittent pain in the extremities and ten hours persistent painful erection of the penis. During a 4-year period 7 patients with priapism were managed successfully with a non-incision treatment program. Three were treated by hypotensive anesthesia with sodium nitroprusside, 2 by the Winter procedure (creating a temporary cavernospongiosum shunt) and 2 by combining hypotensive anesthesia and the Winter procedure. Priapism is a very rare problem in childhood. Although sickle cell disease and leukemia may produce persistent erection, juvenile priapism is most commonly idiopathic. The physiology and pathology of erection are reviewed, and the pathomechanisms of diseases which may produce priapism are described. If red cell sickling and leukemia are excluded, immediate surgical intervention by either corporosaphenous or corporospongiosum shunts is recommended for resolution of priapism and preservation of potency. Since 1952 we have seen nine patients with priapism leading to a diagnosis of chronic granulocytic leukemia (CGL) and a tenth patient who gave a history of priapism when CGL was diagnosed as a result of other symptoms. Seven patients had had one or more transient episodes of prolonged erection before the diagnosis of CGL was established. Two cases of priapism treated with fibrinolysin (streptokinase) are presented. Comparisons are made between medical and surgical treatment in the literature. It is concluded that fibrinolysin may be useful in some cases of early priapism, but should be chosen only after serious consideration of the circumstance that its use precludes early surgery.
