Benefits of implementing pain-related disability and psychological assessment in dental practice for patients with temporomandibular pain and other oral health conditions.

BACKGROUND: Evidence in the field of dentistry has demonstrated the importance of pain-related disability and psychological assessment in the development of chronic symptoms. The Diagnostic Criteria for Temporomandibular Disorders offer a brief assessment for the diagnostic process in patients with orofacial pain (Axis II). The authors describe relevant outcomes that may guide general oral health care practitioners toward tailored treatment decisions and improved treatment outcomes and provide recommendations for the primary care setting. METHODS: The authors conducted a review of the literat

Three kinds of proactive risk analyses for health care.

BACKGROUND: In health care, proactive risk assessment usually takes the form of Failure Mode and Effects Analysis (FMEA). An applied research firm, four community hospitals, and a community health care alliance in south-central Washington State--as members of the Tri-Cities Patient Safety Coalition (TCPSC)--used proactive risk assessment methods such as event tree analysis (ETA) and hazard identification to assess the risk of adverse events associated with a process or system. CASE STUDIES: (1) ETA of a Mental Health Issue: An assessment was performed to understand the risk presented by emerg

Instruments to measure outcomes of post-intensive care syndrome in outpatient care settings - Results of an expert consensus and feasibility field test.

BACKGROUND: There is no consensus on the instruments for diagnosis of post-intensive care syndrome (PICS). We present a proposal for a set of outcome measurement instruments of PICS in outpatient care. METHODS: We conducted a three-round, semi-structured consensus-seeking process with medical experts, followed each by exploratory feasibility investigations with intensive care unit survivors (n1 = 5; n2 = 5; n3 = 7). Fourteen participants from nine stakeholder groups participated in the first and second consensus meeting. In the third consensus meeting, a core group of six clinical researchers

Time-driven activity-based costing of total knee replacements in Karachi, Pakistan.

INTRODUCTION: Osteoarthritis of the knee has been identified as the most common disability in Pakistan. Total knee replacement (TKR) surgery is the curative treatment for advanced osteoarthritis of the knee; however, cost remains one of the barriers to effective and timely service delivery. OBJECTIVE: We conducted a time-driven activity-based costing (TDABC) analysis of TKR to identify major cost drivers and areas for process improvement. METHODS AND ANALYSIS: We performed a prospective TDABC analysis of patients who underwent bilateral TKR at The Indus Hospital (TIH) during a 14-month perio

Addressing the quality and cost of cholecystectomy at a safety net hospital.

BACKGROUND: The aim of this study was to evaluate the effects of safety-net burden on outcomes of a common, urgent operation like cholecystectomy. METHODS: We identified all cholecystectomies performed from 2005 to 2011 in the California State Inpatient Database and separated them into three cohorts based on the performing hospital's safety-net burden. Hierarchical multivariable regression analyses were performed with outcomes including laparoscopy, advanced disease, morbidity, length of hospitalization, and cost. RESULTS: Safety-net hospitals had similar rates of laparoscopy, overall advanc

Implementation of simple telehealth to manage hypertension in general practice: a service evaluation.

BACKGROUND: Hypertension is common and conveys significant risk of morbidity and mortality. However, inadequate control of hypertension is common. Following a successful local use of a simple telehealth intervention ('Florence') for the diagnosis and management of hypertension, the Advice & Interactive Messaging (AIM) for Health simple telehealth programme was launched across England in March 2013. Four protocols were developed to diagnose and monitor blood pressure (BP). The aim of this service evaluation was to identify the extent to which predefined service outcomes, regarding ascertainment

Robot-assisted vs. conventional laparoscopic rectopexy for rectal prolapse: a comparative study on costs and time.

PURPOSE: Laparoscopic rectopexy has become one of the most advocated treatments for full-thickness rectal prolapse, offering good functional results compared with open surgery and resulting in less postoperative pain and faster convalescence. However, laparoscopic rectopexy can be technically demanding. Once having mastered dexterity, with robotic assistance, laparoscopic rectopexy can be performed faster. Moreover, it shortens the learning curve in simple laparoscopic tasks. This may lead to faster and safer laparoscopic surgery. Robot-assisted rectopexy has been proven safe and feasible; how

[Parent satisfaction with a child and family-centered treatment program for primary headache in childhood and adolescence].

Patient satisfaction represents a significant outcome criterion in the context of systemic psychotherapeutic therapy research. This study investigated parent satisfaction with a psychosocial treatment program (comprising three components: child group therapy, parents' evenings, systemic family sessions) for pediatric primary headache (diagnosed according to IHS criteria). 10 weeks after the end of the treatment program, the parents were sent a questionnaire containing open questions and ten-point numerical rating scales. The sample comprised n=48 families. The return rate was 89%. The qualitat

Evaluation of perinatal care management programs: an integrated review.

Nursing care management has become a popular method to integrate health care systems with goals of decreasing costs and improving quality. As high-risk pregnancies and newborn intensive care unit (NBICU) costs generate some of the highest costs in health care, care management has been a strategy introduced in perinatal medicine to accomplish the same goals. Consistent with other areas of nursing, perinatal care management currently has no agreed upon model of practice or method of evaluating how and whether the goals have been achieved. The purpose of this project was to evaluate various perin

Design and implementation of a stratified care model for refugee women's behavioral health in an obstetrical care setting.

INTRODUCTION: In December 2022, hospital-based obstetrical providers identified significant behavioral health (BH) needs among recently arrived Haitian refugee patients. Demand for BH services exceeded capacity, and services were not culturally tailored for perinatal Haitian women. This necessitated a reimagining of the BH care model. This quality improvement article describes the process of designing and implementing an embedded BH care model into a refugee-serving obstetrical clinic. METHOD: An intervention mapping (IM) approach guided this study. A needs assessment (IM Phase 1; January 202

Appropriateness of Care Measures: A Novel Approach to Quality.

The clinical judgment of a physician is one of the most important aspects of medical quality, yet it is rarely captured with quality measures in use today. We propose a novel approach using individualized physician benchmarking that measures the appropriateness of care that a physician delivers by looking at their practice pattern in a specific clinical situation. A prime application of our novel approach to appropriateness measures is the surgical management of peripheral artery disease and claudication. We discuss 4 potential consensus metrics for the treatment of claudication that explore a

Follow-up of abnormal results in an urban community-based breast cancer screening program.

OBJECTIVE: Determine the rates of follow-up for abnormal mammograms and see how they compare with rates of similar programs elsewhere. DATA: Records from 1990-2000 of the Breast Cancer Awareness Program were analyzed and reviewed to determine follow-up rates. RESULTS: The follow-up rate for all 11 years was 85%; during the last 7 years the rate has been 90%. CONCLUSION: Follow-up rates were as good and generally better than those reported in the literature for similar programs. The rates, though good to begin with, improved with the more rigorous training and requirements of the Wisconsin W
