Misplaced endotracheal tubes by paramedics in an urban emergency medical services system.

STUDY OBJECTIVE: To determine the incidence of unrecognized, misplaced endotracheal tubes inserted by paramedics in a large urban, decentralized emergency medical services (EMS) system. METHODS: We conducted a prospective, observational study of patients intubated in the field by paramedics before emergency department arrival. During an 8-month period, emergency physicians assessed tube position at ED arrival using a combination of auscultation, end-tidal carbon dioxide (ETCO(2)) monitoring, and direct laryngoscopy. RESULTS: A total of 108 intubated patients were studied. On arrival in the E

Ethical guidelines in fetal medicine.

The ethical guidelines in the clinical practice of fetal medicine are discussed, largely from the point of view of early prenatal diagnosis. The discussion concentrates on several specific aspects including counselling, good procedure, prenatal screening, disclosure of results including fetal sex, experimentation and assessment of new technologies and health education. The emphasis is placed on the importance of informed consent, choice and understanding on the part of the patient, and the professional competence of the doctor and other professionals she may meet.

The medical competence of health care providers in sub-Saharan Africa: Evidence from 16 127 providers across 11 countries.

Despite a consensus that quality of care is critically deficient in low-income countries, few nationally representative studies provide comparable measures of quality of care across countries. To address this gap, we used nationally representative data from in-person administrations of clinical vignettes to measure the competence of 16 127 health care providers across 11 sub-Saharan African countries. Rather than large variations across countries, we found that 81% of the variation in competence is within countries and the characteristics of health care providers do not explain most of this va

The Metamorphosis of NAR to CRNA: The Caterpillar to Butterfly Analogy.

The journey from nurse anesthesiology resident (NAR) to certified registered nurse anesthetist (CRNA) is a profound transformation, one that extends beyond skill acquisition to the evolution of professional identity. Using the metaphor of a caterpillar's metamorphosis into a butterfly, this article explores how academic rigor, clinical immersion, and mentorship shape resilience, adaptability, and confidence in future CRNAs. Grounded in growth mindset theory, it highlights how embracing challenges, seeking feedback, and reframing setbacks as opportunities for learning build both competence and

[White-collar criminals--a homogenous offender population? Reflections on typical and atypical "white-collar criminals"].

Little has been written in German scientific literature on the personality structure of white-collar criminals. Often, the relevance of this level of investigation has downright been denied. Conventional psychopathology does not seem to be an appropriate approach to these character problems since there are not only deficits but also competences to be found which are useful while making a professional career. The author points out the inhomogeneity of this offender population and presents a case report of an atypical white-collar criminal. Over and above that, he introduces two psychological co

Investigating the relationship between clinical competence and the incidence of needle-stick injuries (NSIs) and their contributing factors in nurses: a descriptive cross-sectional study in Southern Iran.

BACKGROUND: Needle-stick injuries (NSIs) pose a substantial occupational hazard, exposing healthcare professionals to potentially infectious diseases. Nurses' clinical competence plays a crucial role in preventing and mitigating the incidence of NSIs. This study aimed to investigate the relationship between clinical competence and the incidence of NSIs, as well as the factors contributing to these injuries, among nurses in Fars Province, southern Iran, from March 2023 to May 2023. METHODS: This descriptive cross-sectional study included 264 nurses selected through convenience sampling. All pa

Contextualizing the digital competence framework for nursing education: Findings from a Delphi.

AIM: To identify key digital competencies for higher education nursing faculty, explore challenges in digital integration, and provide recommendations for systematic faculty development. BACKGROUND: Digital competencies are essential for nursing faculty to deliver high-quality education in an increasingly digitalised healthcare context. Despite their importance, faculty members often encounter challenges in integrating digital technologies into teaching, clinical education, and simulation, highlighting the need for structured and context-specific faculty development. DESIGN: A mixed-methods,

Financial management roles of nurse managers in selected public hospitals in KwaZulu-Natal province, South Africa.

BACKGROUND: The public health sector in South Africa has been facing severe financial cutbacks and financial constraints in recent times. Implementing a UDL Framework: A Study of Current Personnel Preparation Practices.

Young adults with intellectual disability (ID) continue to experience the least successful postschool outcomes among transition-aged youth ( Sanford et al., 2011 ). Experts disagree on the most effective approach to improve outcomes such as employment, postsecondary education, and community living. In 2015, the National Goals Conference brought together educational researchers to set an agenda to guide the field in terms of research, practice, and policy ( Thoma, Cain, & Walther-Thomas, 2015 ). One of their recommendations, based on promising research and practices, urged the field to identify

Are we exacerbating students' learning disabilities? an investigation of preservice teachers' attributions of the educational outcomes of students with learning disabilities.

While claims of the importance of attribution theory and teachers' expectations of students for student performance are repeatedly made, there is little comprehensive research identifying the perceptions preservice teachers have of students with learning disabilities (LD). Accordingly, 444 Australian preservice primary school teachers were surveyed using vignettes and Likert-scale questions, to ascertain their responses to students with and without LD. It was found that preservice primary school general education teachers held a negative attribution style towards students with LD. Preservice p

Coordination dynamics of the horse-rider system.

The authors studied the interaction between rider and horse by measuring their ensemble motions in a trot sequence, comparing 1 expert and 1 novice rider. Whereas the novice's movements displayed transient departures from phase synchrony, the expert's motions were continuously phase-matched with those of the horse. The tight ensemble synchrony between the expert and the horse was accompanied by an increase in the temporal regularity of the oscillations of the trunk of the horse. Observed differences between expert and novice riders indicated that phase synchronization is by no means perfect bu

Recognition of special qualifications in neurological surgery.

In recent years, one-half of the 23 recognized specialty boards in medicine have begun formally to recognize subspecialization with some type of certification process. One such board, the American Board of Internal Medicine, examines and certifies in 11 subspecialty areas while the American Board of Pathology does so in nine, the American Board of Pediatrics in eight, the American Board of Obstetrics and Gynecology in four, and the American Board of General Surgery in three. Arguments for and against similar recognition of subspecialization within neurosurgery are reviewed. The present positio

Pharmacy Education and Transition to Practice in Indonesia.

Pharmacy education in Indonesia is expected to prepare pharmacists for a wide range of roles in a health system where medicines are central to prevention and treatment. Yet workforce studies describing capability gaps, uneven distribution, and variation in service delivery across sectors and regions suggest that ensuring new graduates are ready for practice remains a persistent challenge. This evidence-informed narrative review describes the pharmacist education pipeline in Indonesia and maps how competency expectations are specified, taught and assessed across the main stages of education and

Dental professionalism and influencing factors: patients' perception.

BACKGROUND: Professionalism was recognized as a fundamental competency of medical/dental practice at the beginning of the twenty-first century. Organizations and experts published several definitions of professionalism, but research on patients' perception of professionalism has been limited. To address this gap in literature, this study explored dental patients' perception of dental professionalism, and then compared their perception with that of dental professionals. METHODS: Of the 800 questionnaires that were distributed to dental patients in public and private clinics and hospitals, 504
