Prospective study of individuals with long-term urinary catheters colonized with Proteus species.

OBJECTIVES: To characterize the variability in the times catheters take to block with encrustation in patients who have Proteus in their urinary flora, and to identify factors responsible for modulating the rate of catheter encrustation and blockage. PATIENTS AND METHODS: Twenty patients were followed prospectively for > or = 12 weeks, with a bacteriological analysis on weekly urine samples. The pH of the voided urine samples and the pH at which crystals formed in them (the nucleation pH) were determined. Catheters were collected and examined for bacterial biofilm and crystal deposition. RES…

Proteus syndrome in southern Africa: natural history and clinical manifestations in six individuals.

Six individuals with Proteus syndrome have been investigated; 2 were adults and the others ranged in age from 2 to 11 years. They had a wide spectrum of manifestations and severity together with the hitherto unreported anomalies of penile hypertrophy, macro-orchidism, goiter, and failure of breast development. These findings were associated with normal endocrine function. Bizarre digital overgrowth, hemihypertrophy, thickened palms and soles, exostoses of the skull, and multiple hamartomata were common anomalies. Early overgrowth of limbs and digits occurred in several instances but the ultima…

Multidisciplinary management promoting wound healing in a patient with Proteus syndrome: a case report.

INTRODUCTION: Proteus syndrome is caused by AKT1 gene mutations. It manifests during childhood and results in asymmetric overgrowth of skin, bone, central nervous system, and adipose tissues and has a poor survival rate. Currently, there are no targeted drug treatments for this condition. Due to its rarity, the nonspecific symptoms and complications of Proteus syndrome often go unrecognized and, thus, untreated by clinicians. CASE DESCRIPTION: This case report documents the diagnosis and treatment of a 14-year-old patient with Proteus syndrome who developed severe soft tissue infection second…

Management of CPAP Follow-up by Telemonitoring in Obstructive Sleep Apnea: The PROTEUS Project.

PURPOSE: CPAP is the standard treatment for obstructive sleep apnea (OSA), but as many as 50% of patients discontinue its use, within three years. The PROTEUS project aims to enhance CPAP adherence through telemonitoring. PATIENTS AND METHODS: OSA patients undergoing CPAP treatment since July 2018, had an in-person reassessment after three months, followed by quarterly telemonitoring by providers, who intervened in cases of poor adherence (less than 4 h·night-1 or less than 70% of days), excessive mask leakage, or elevated residual apnea-hypopnea index (rAHICPAP). RESULTS: A total of 486 (~8…

Genomic and Biological Characterization of a Novel Proteus mirabilis Phage with Anti-Biofilm Activity.

The emergence of multidrug-resistant (MDR) Proteus mirabilis poses a significant threat in porcine farming and public health, highlighting the need for alternative biocontrol agents. This study aimed to isolate and characterize a lytic bacteriophage with therapeutic potential against MDR P. mirabilis. Using the clinical MDR P. mirabilis strain Pm 07 as host, a bacteriophage, vB_Pmc_P-07 (P-07), was successfully isolated from fecal and sewage samples via an enrichment protocol. Phage P-07 forms plaques surrounded by a distinct translucent "halo," suggesting the production of depolymerase. It ac…

Clonal Dissemination of NDM-Producing Proteus mirabilis in a Teaching Hospital in Sousse, Tunisia.

Proteus mirabilis (P. mirabilis) is an opportunistic pathogen involved in urinary tract infections as well as various nosocomial infections. Emerging resistances to beta-lactams in this species complicates potential treatment since it is intrinsically resistant to colistin. Eleven isolates of carbapenem-non-susceptible P. mirabilis were identified in Sousse Hospital, Tunisia, from January 2018 to December 2022. MICs were determined and isolates were sequenced to determine their resistomes, sequence types, virulence factors, and their clonal relationships. Susceptibility testing showed that all…

First evidence of a therapeutic effect of miransertib in a teenager with Proteus syndrome and ovarian carcinoma.

Proteus syndrome (PS) is an ultra-rare disease characterized by progressive, disproportionate, segmental overgrowth caused by a somatic gain-of-function mutation p.Glu17Lys in the oncogene AKT1. The disease has high morbidity and mortality rates due to the increased risk for patients to develop cancer and progressive overgrowth. A teenage patient with severe PS phenotype developed a pelvic recurrence of low-grade serous ovarian carcinoma (LGSOC). Taking into consideration, recent results of the use of AKT inhibitors both in PS and AKT-mutant cancers, we treated the patient on a compassionate b…

Polymorphisms of Gene Cassette Promoters of the Class 1 Integron in Clinical Proteus Isolates.

OBJECTIVE: To describe the polymorphisms of gene cassette promoters of the class 1 integron in clinical Proteus isolates and their relationship with antibiotic resistance. METHODS: Polymorphisms of the gene cassette promoter in 153 strains of Proteus were analyzed by PCR and nucleotide sequencing. Variable regions of atypical class 1 integrons were detected by inverse PCR and nucleotide sequencing. Enterobacterial repetitive intergenic consensus (ERIC)-PCR was used to analyze the phylogenetic relationships of class 1 integron-positive clinical Proteus isolates. Representative beta-lactamase g…

Development of an "early warning" sensor for encrustation of urinary catheters following Proteus infection.

Biofilm formation in long-term urinary catheterized patients can lead to encrustation and blockage of urinary catheters with serious clinical complication. Catheter encrustation stems from infection with urease-producing bacteria, particularly Proteus mirabilis. Urease generates ammonia from urea, and the elevated pH of the urine results in crystallization of calcium and magnesium phosphates, which block the flow of urine. The aim of this research is to develop an "early warning" silicone sensor for catheter encrustation following bacterial infection of an in vitro bladder model system. The in…

Use of quorum sensing antagonists to deter the formation of crystalline Proteus mirabilis biofilms.

Proteus mirabilis biofilms are a major cause of urinary catheter blockage. Antibiotic-impregnated catheters used to prevent catheter blockage have achieved limited success. Research has examined the efficacy of quorum sensing inhibitors against Pseudomonas aeruginosa biofilms, but there are few reports of the effects of these compounds against crystalline P. mirabilis biofilms. This study examined the effect of two quorum sensing antagonists, p-nitrophenyl glycerol (PNPG) and tannic acid, against crystalline P. mirabilis biofilms. Tannic acid and PNPG were observed to inhibit the quorum sensin…

Comparison of PCR-based molecular markers for the characterization of Proteus mirabilis clinical isolates.

Proteus mirabilis is one of the most important pathogens associated with complicated urinary tract infections (acute pyelonephritis, bladder infections, kidney stones) and bacteremia, affecting patients with anatomical abnormalities, immunodeficiency, and long-term urinary catheterization. For epidemiological purposes, various molecular typing methods, such as pulse-field gel electrophoresis (PFGE) or ribotyping, have been developed for this pathogen. However, these methods are labor intensive and time-consuming. We evaluated the discriminatory power of several PCR-based fingerprinting methods…

Kasabach-Merritt syndrome caused by giant hemangiomas of the spleen in patients with Proteus syndrome.

Proteus syndrome is a rare hamartomous disorder with highly variable manifestations of disproportionate overgrowth of body parts, epidermal nevi and vascular malformations. We describe two patients who had typical clinical and imaging Proteus syndrome features. One of most characteristic findings in these two patients was that they had giant hemangiomas of the spleen that caused Kasabach-Merritt syndrome, demonstrating clinical bleeding and disseminated intravascular coagulation laboratory alterations. The present two cases are the first report of this complication in Proteus syndrome. The pat…

A new gastrointestinal finding in Proteus syndrome: report of a case of multiple colonic hemangiomas.

A patient with Proteus syndrome presented with lower gastrointestinal bleeding due to multiple colonic hemangiomas, a finding which has not been described previously in this syndrome. The patient was a 20-year-old man with features fulfilling the diagnostic criteria for Proteus syndrome. He fulfilled both general criteria (mosaic distribution of the lesions, progressive course and sporadic occurrence) and specific criteria (including epidermal nevus, disproportionate overgrowth of limbs and vascular malformations). Fiberoptic colonoscopy revealed multiple hemangiomas, 0.5-1 cm in diameter, on …

Chemical, biological, and structural properties of stable Proteus L forms and their parent bacteria.

Proteus L forms were disrupted by osmotic shock, and the sedimentable material present in the homogenate was further fragmented in a Sorvall pressure cell. The pressure cell was also used for disrupting normal Proteus cells. The homogenates obtained were fractionated by differential centrifugation. Purified endotoxins were isolated from the major fractions by phenol extraction. Material extracted with phenol from the membrane fraction of the L forms was about as toxic and pyrogenic on a weight basis as the typical enterobacterial endotoxins isolated from cell walls of normal bacteria. The yiel…
