Psychotic disorder and extrapyramidal symptoms associated with vitamin B12 and folate deficiency.

Vitamin B12 and folate deficiency causing neuropsychiatric and thrombotic manifestations, such as peripheral neuropathy, subacute combined degeneration of cord, dementia, ataxia, optic atrophy, catatonia, psychosis, mood disturbances, myocardial infarction and portal vein thrombosis are well known. This present report highlights an unusual presentation of vitamin B12 deficiency-psychotic disorder, extrapyramidal symptoms in a 12-year-old boy. His symptoms responded to parenteral vitamin B12 therapy. So with this report we emphasized that serum vitamin B12 and folate levels should be measured,

Mortality in offspring of parents with psychotic disorders: a critical review and meta-analysis.

OBJECTIVE: Mortality risk in offspring of parents with psychotic disorders is a sensitive and important topic, but evidence on which to base plans for preventive services is limited. The authors synthesized evidence for mortality risk among offspring of parents with psychotic disorders and examined potential modifiers of risk such as offspring age and parental diagnosis. METHOD: Electronic reference and citation databases were searched. Secondary analyses were carried out to generate relative risk estimates and perform post hoc statistical power calculations. A meta-analysis of the associatio…

Major depressive disorder with psychotic features in an aviator after head trauma.

We present a case in which a Naval aviator suffered an unusual sequence of neuropsychiatric symptoms after head trauma. He demonstrated subtle deficits on several measures 1 mo after the trauma. Repeat testing at 8 mo showed apparent recovery, and the patient was cleared to return to flight status. Unbeknownst to medical staff, the patient was still experiencing difficulties. He was found in a severely debilitated state and exhibiting psychotic features 12 mo after his initial trauma. The patient was hospitalized in a psychiatric ward, and with prolonged inpatient and outpatient treatment, he

Family history study of posttraumatic stress disorder with secondary psychotic symptoms.

OBJECTIVE: A family history approach was used to determine if posttraumatic stress disorder (PTSD) with secondary psychotic symptoms was associated with a familial vulnerability to schizophrenia and other psychoses. METHOD: Family history methods were used to compare rates of familial psychopathology in the first-degree relatives of three proband groups: 1) patients with DSM-IV PTSD with secondary psychotic symptoms, 2) patients with DSM-IV PTSD without psychotic symptoms, and 3) healthy matched comparison subjects. RESULTS: PTSD with secondary psychotic symptoms was not associated with an e

[Persisting and late onset psychotic disorder due to consumption of ecstasy (MDMA)].

Let us present a case of recurrent paranoid psychotic episodes in a patient with a history of abuse of <<ecstasy>> (MDMA: 3, 4-methylenedioxymethamphetamine), that persisted after a long withdrawal time, that, in our opinion, after an exhaustive differential diagnosis, may be attributed to that consumption. The carachteristics of this case are formally and naturally equivalent to those referred to in other papers about paranoid psychosis after abuse of MDMA, in which the sudden appearance, and the symptomatolgy of an intense feeling of threat and physical violence outstands, accompanied by abn

Dopamine antagonists in tic-related and psychotic spectrum obsessive compulsive disorder.

Serotonin uptake inhibitors (SUIs) have been established as the first-line pharmacotherapy of obsessive compulsive disorder (OCD). However, approximately one half of patients who receive an adequate trial with these agents remain clinically unchanged. The addition of drugs that enhance serotonin (5-HT) neurotransmission, such as lithium and buspirone, to ongoing treatment in SUI-refractory patients has generally proved to be an ineffective strategy. The addition of dopamine antagonists to the regimens of SUI-resistant patients appears to be a useful approach for OCD patients with a comorbid ch

[Behavioral study on reduction of psychotic and behavioral disorders induced by antitussive and expectorant].

Previously, we reported that psychotic and behavioral disorders induced by repeated abuse of a commercial preparation of antitussive and expectorant (BRON-W Solution, SS Pharmaceut.) was mainly due to caffeine (CAF). In order to reduce such problem, we assessed effects of a preparation containing theophylline (THEO) as the substitution of CAF, and dihydrocodeine (2HCOD), methylephedrine (MEPH) and chlorpheniramine (CPA) on ambulatory activity and discrete lever-press avoidance response in mice. It is well known that THEO is less potent in the central action than CAF, but its relaxant action on

Sleep disturbances as an early indicator of psychotic disorders in adolescents and young adults.

Sleep disturbances are increasingly recognized as clinically relevant markers in the early detection of psychotic disorders among adolescents and young adults. These disturbances, including insomnia, circadian rhythm disruptions, parasomnias, and fragmented sleep, affect up to 70% of individuals at clinical high risk for psychosis, rising to 80% in fully developed schizophrenia. This review synthesizes current evidence on the relationship between sleep problems and early psychotic symptoms, emphasizing their potential as observable and possibly modifiable indicators of illness onset. Neurobiol…

Association of Concussion With an Increased Risk of Psychotic Disorders: A Retrospective Cohort Study.

BACKGROUND: Head trauma has a well-documented association with increased incidence of psychiatric disorders, especially depression, anxiety, and post-traumatic stress disorder (PTSD). However, the relationship between traumatic brain injury (TBI) and subsequent psychotic disorders remains poorly characterized. Despite their relative rarity, psychotic disorders are highly debilitating, making clarification of this association clinically important, given the substantial public health burden of mild traumatic brain injury (mTBI). Whether this risk reflects mechanisms specific to head injury or tr…

Diagnosis of methamphetamine-induced psychotic disorder: Findings of an expert consensus panel.

AIM: We define criteria for methamphetamine-induced psychotic disorder (MIPD) to aid in accurate and reliable diagnosis. METHOD: An expert panel was recruited and engaged in an iterative consensus process. A literature search supported this work. The a priori level for consensus was considered ≥80% of voting panellists. RESULTS: The final expert panel included 22 physicians from different backgrounds and practice environments. The panel produced two consensus diagnoses: (1) acute MIPD and (2) persisting MIPD, which is further separated into subacute and chronic timeframes. Although certain c

Brief psychotic disorder related to areca nut use: a case report.

BACKGROUND: Areca Nut (AN) is the fourth most commonly abused drug after nicotine, ethanol, and caffeine, due to its psychoactive properties provided by bioactive substances. Although previous studies have demonstrated AN's anxiolytic-like activity and potential benefits in ameliorating symptoms of depression and schizophrenia, there remains limited awareness regarding its association with brief psychotic disorder. CASE PRESENTATION: This case report presents the clinical profile of a 30-year-old male patient with a history of betel nut chewing for the past 2 years, who exhibited sudden onset

Clinicodemographic correlates of psychotic features in bipolar disorder - a multicenter study in China.

BACKGROUND: Psychotic symptoms are prevalent in patients with bipolar disorder (BD). However, nearly all previous studies on differences in sociodemographic and clinical factors between patients with (BD P +) and without (BD P-) psychotic symptoms were conducted in Western populations, and limited information is known in China. METHOD: A total of 555 patients with BD from seven centers across China were recruited. A standardized procedure was used to collect patients' sociodemographic and clinical characteristics. The patients were divided into BD P + or BD P- groups based on the presence of

The effect of treatment on insight in psychotic disorders - A systematic review and meta-analysis.

BACKGROUND: For people with a psychotic disorder lack of insight can be detrimental on their condition and recovery. For this reason, insight has been considered as a target for therapy. We conducted a systematic review of the literature on pharmacological, psychological and other treatments to test the hypothesis that these interventions could improve insight. METHODS: We performed a literature search (1970-2020) across the following databases: PubMed, EMBASE, PsychINFO, Medline and Web of Science. Within each database the following search terms and the associated Boolean operatives were use

Medication-induced Psychotic Disorder. A Review of Selected Drugs Side Effects.

BACKGROUND: Medication-induced psychotic disorder (MIPD) is a diagnostic term for a syndrome with symptoms such as hallucinations and delusions directly related to drug intake. The purpose of this review is to report and comment on the current knowledge about pathomechanisms, risk factors, symptoms, and treatment of MIPD caused by selected widely used medications. METHODS: PubMed, Scopus, and Google Scholar databases were searched for articles on MIPD published prior to January 2021 using search terms 'psychosis' OR 'psychotic disorder' AND 'side effects' combined with certain medications gro

UP'S: A Cohort Study on Recovery in Psychotic Disorder Patients: Design Protocol.

Recovery is a multidimensional concept, including symptomatic, functional, social, as well as personal recovery. The present study aims at exploring psychosocial and biological determinants of personal recovery, and disentangling time-dependent relationships between personal recovery and the other domains of recovery in a sample of people with a psychotic disorder. A cohort study is conducted with a 10-year follow-up. Personal recovery is assessed using the Recovering Quality of Life Questionnaire (ReQoL) and the Individual Recovery Outcomes Counter (I.ROC). Other domains of recovery are asses

Age at migration and the risk of psychotic disorders: a systematic review and meta-analysis.

OBJECTIVE: To conduct a systematic review and meta-analysis of the existing evidence on the association between age at migration and the risk of psychotic disorders. METHODS: Observational studies were eligible for inclusion if they presented data on the association between age at migration and the risk of psychotic disorders among first-generation migrant groups. We used two random effects meta-analyses to pool effect estimates for each stratum of age at migration relative to (i) a native-born reference category and (ii) the youngest age stratum (0 to 2 years). RESULTS: Ten studies met incl…

Adverse Childhood Experiences and Methylation of the FKBP5 Gene in Patients with Psychotic Disorders.

Altered methylation of the FKBP5 gene has been observed in various mental disorders and attributed to the effects of adverse childhood experiences (ACEs). However, the level of FKBP5 methylation has not been investigated in patients with psychotic disorders. Therefore, in this study we aimed to determine the FKBP5 methylation in patients with psychosis and controls, taking into account the effects of ACEs. Participants were 85 patients with psychotic disorders, including first-episode psychosis (FEP) patients and acutely relapsed schizophrenia (SCZ-AR) patients, as well as 56 controls. The lev…
