Vanishing Lung Syndrome, or Idiopathic Giant Bullous Emphysema, with Pneumothorax, and Subcutaneous Emphysema in a 58-Year-Old Female Smoker with Chronic Obstructive Pulmonary Disease.

BACKGROUND Vanishing lung syndrome (VLS), also known as idiopathic giant bullous emphysema, is a rare manifestation of chronic obstructive pulmonary disease (COPD) and usually occurs in middle-aged smokers. This report presents a 58-year-old female smoker with COPD and VLS who presented with spontaneous pneumothorax. The pneumothorax was managed with a chest tube and was later complicated by massive subcutaneous emphysema. CASE REPORT A 58-year-old woman with a past medical history of long-term smoking and COPD presented with worsening shortness of breath. Upon initial evaluation, she had tach

Pulmonary artery stiffness in chronic obstructive pulmonary disease (COPD) and emphysema: The Multi-Ethnic Study of Atherosclerosis (MESA) COPD Study.

PURPOSE: Chronic obstructive pulmonary disease (COPD) and particularly emphysema are characterized by stiffness of the aorta, due in part to accelerated elastin degradation in the lungs and aorta. Stiffness of the pulmonary arteries (PAs) may also be increased in COPD and emphysema, but data are lacking. We assessed PA stiffness using MRI in patients with COPD and related these measurements to COPD severity and percent emphysema. MATERIALS AND METHODS: The Multi-Ethnic Study of Atherosclerosis (MESA) COPD Study recruited 290 participants, age 50-79 years with 10 or more packyears and free of

Emphysema elevates the DETECT scores: impact on pulmonary hypertension screening and diagnosis in systemic sclerosis.

OBJECTIVES: Combined pulmonary fibrosis and emphysema (CPFE) predicts unfavourable outcomes in systemic sclerosis (SSc). CPFE and emphysema are associated with pulmonary function tests (PFTs) abnormalities. As screening algorithms for pulmonary hypertension include PFTs, we aimed to assess whether CPFE and emphysema affect the performance of the DETECT algorithm to select patients for right-heart catheterization (RHC). METHODS: SSc patients from our referral centre, with available chest tomography images to identify emphysema or interstitial lung disease (ILD) and data to calculate the DETECT

Chronic Eosinophilic Pneumonia With Overlapping Emphysema and Fibrosis: An Atypical Presentation of Recurrent Pulmonary Tuberculosis.

This case report describes an atypical presentation of recurrent pulmonary tuberculosis in a patient with chronic respiratory symptoms and eosinophilia, initially misdiagnosed as chronic eosinophilic pneumonia. Despite suggestive clinical and radiological features, a final diagnosis was only reached after prolonged microbiological culture. The case underscores the diagnostic complexity of interstitial lung diseases and emphasizes the importance of maintaining a broad differential, particularly in tuberculosis-endemic regions.

Meta-analysis: clinical features and treatments of lung cancer in combined pulmonary fibrosis and emphysema.

BACKGROUND AND AIM: There are many epidemiological pieces of evidence that show combined pulmonary fibrosis and emphysema (CPFE) patients have an increased risk of lung cancer. We conducted a systematic review of all published data to define the characteristics and treatments of lung cancer that develops in CPFE by performing a meta-analysis. METHODS: Databases(including PubMed, Medline, CNKI, VIP, etc.) were searched to find original articles that related to lung cancer in CPFE(CPFE-LC) patients and a meta-analysis was used to analyze the included 15 articles. Stata17.0 software was performe

Further evidence of a type 2 inflammatory signature in chronic obstructive pulmonary disease or emphysema.

BACKGROUND: There is increasing recognition of a type 2 (T2) inflammatory pattern in a subset of patients with chronic obstructive pulmonary disease (COPD) or emphysema, characterized by blood and airway eosinophilia. The mechanism underlying this is not well established. The recognition that CD125 (interleukin [IL]-5 receptor alpha) is expressed on some lung neutrophils and eosinophils in patients with asthma led us to speculate that CD125 may also be expressed on lung neutrophils in patients with COPD or emphysema. OBJECTIVE: To interrogate the expression of CD125 on lung neutrophils (and,

Spontaneous pneumomediastinum, emphysema, and pulmonary bullae associated with refractory Mycoplasma pneumoniae pneumonia in a child.

We report a 21-month-old child with spontaneous pneumomediastinum (PM), emphysema, and pulmonary bullae caused by Mycoplasma pneumonia (M. pneumoniae) pneumonia. The patient presented with stubborn dyspnea and wheezing although received integrated treatment, including anti-infection, anti-inflammation, and intravenous immunoglobulin. This case was unique in that pulmonary bullae kept increasing. PM and emphysema were refractory in our patient, which have been reported as benign and limited. Surgical intervention was implemented to ultimately cure the case. Clinicians should be aware of air lea

Sequential occurrence of combined pulmonary fibrosis and emphysema syndrome in a non-smoker female patient.

The combined pulmonary fibrosis and emphysema (CPFE) syndrome is a unique and an under-recognized disorder characterized by emphysema in the upper lobes and interstitial fibrosis in the lower lobes of the lung. It occurs predominantly in males and almost exclusively in smokers. This rare combination of a restrictive and an obstructive mechanical defect carries a poorer prognosis than either of the two components. We present a case of CPFE syndrome in a non-smoker female patient who developed lower lobe emphysema subsequent to development of interstitial fibrosis. The case was remarkable for th

The value of CT for disease detection and prognosis determination in combined pulmonary fibrosis and emphysema (CPFE).

BACKGROUND AND PURPOSE: Several imaging-based indices were constructed quantitatively using the emphysema index (EI) and fibrosis score (FS) on high-resolution computed tomography (HRCT). We evaluated the ability of these indices to predict mortality compared to physiologic results. Additionally, prognostic predictive factors were compared among subgroups with biopsy-proven fibrotic idiopathic interstitial pneumonia (IIP) (biopsy-proven CPFE) and in a separate cohort with subclinical CPFE. MATERIALS AND METHODS: Three chest radiologists independently determined FS. EI was automatically quanti

A systemically administered EP2 receptor agonist stimulates pulmonary angiogenesis in a murine model of emphysema.

We investigated whether systemically administered EP2 receptor agonists would stimulate angiogenesis in the emphysematous lungs of mice. Saline or porcine pancreatic elastase was intratracheally administered to C57BL/6J mice to induce the formation of emphysematous lesions, and 4 weeks later the mice were intraperitoneally injected with an EP2 receptor agonist, ONO-AE1-259 or PGE2, or saline on days 1-5 each week for 3 weeks. Intraperitoneal ONO-AE1-259 in the mice in the intratracheal saline group increased pulmonary capillary volume to 114.9% of the control value (P<0.05), and when administe

CT findings and temporal course of persistent pulmonary interstitial emphysema in neonates: a multiinstitutional study.

OBJECTIVE: The purpose of this study was to evaluate the CT appearance, management, and temporal course of persistent pulmonary interstitial emphysema in neonates. MATERIALS AND METHODS: Criteria for inclusion in the study group included neonates with a history of prematurity who required ventilation for lung disease, development of hyperexpanded radiolucent lung lesions after typical radiographic findings of pulmonary interstitial emphysema, and CT documentation of lung abnormalities. Radiographs and CT scans were reviewed for the anatomic distribution, appearance, and presence of classic li

Impaired pulmonary inflammatory responses are a prominent feature of streptococcal pneumonia in mice with experimental emphysema.

Little is known about why patients with chronic obstructive pulmonary disease are susceptible to bacterial infections. Using an animal model of pulmonary emphysema, we investigated the inflammatory responses to bacterial infection. After intratracheal infection with Streptococcus pneumoniae (10(3)-10(7) cfu/mouse), the control mice did not die. However, the mice with emphysema died in a dose-dependent manner. Bronchoalveolar lavage fluid, examined 24 hours after infection showed that the numbers of total cells and neutrophils, in addition to murine tumor necrosis factor-alpha and macrophage in…

Effects of high-frequency oscillatory ventilation on circulation in neonates with pulmonary interstitial emphysema or RDS.

OBJECTIVE: Mechanical ventilation may impair cardiovascular function if the transpulmonary pressure rises. Studies on the effects of high-frequency oscillatory ventilation (HFOV) on cardiovascular functions have yielded conflicting results. This study was done to compare alterations in left ventricular output and blood flow velocities in the anterior cerebral artery, internal carotid artery, and celiac artery using a Doppler ultrasound device before and 2 h after initiating HFOV in neonates with respiratory distress syndrome (RDS) or pulmonary interstitial emphysema (PIE). DESIGN: Prospective

Accuracy of postmortem radiography of excised air-inflated human lungs in assessment of pulmonary emphysema.

The accuracy of radiography of excised air-inflated lungs in assessing pulmonary emphysema at necropsy was evaluated in a series of 107 adults who had died in hospital by reading the radiographs and examining the pathological specimens independently. The radiographic and pathological assessments of the severity of emphysema correlated significantly (r = 0.87, p less than 0.0001). Mild emphysema was recognised radiographically in 88.7% and moderate in 94.9% of the lungs. One of 16 normal lungs (6.3%) was radiographically diagnosed as showing mild emphysema. Six out of 53 lungs (11.3%) with mild…

The Efficacy of Antifibrotics in Combined Pulmonary Fibrosis and Emphysema: A Nationwide Multicentre Retrospective Cohort Study.

INTRODUCTION: While efficacy of antifibrotics (pirfenidone and nintedanib) in idiopathic pulmonary fibrosis (IPF) is well established, its effects on combined pulmonary fibrosis and emphysema (CPFE) remain unclear. We evaluated the therapeutic efficacy of antifibrotics in CPFE patients. METHODS: This multicentre study used the Korean IPF Cohort registry, identifying 966 CPFE patients. A 6-month landmark window controlled for immortal time bias. Propensity score matching (PSM) balanced baseline characteristics for age, sex, body mass index (BMI), lung cancer, and Composite Physiologic Index (C

Analysis of body composition with bioelectrical impedance analysis in patients with severe COPD and pulmonary emphysema.

BACKGROUND: Patients with chronic obstructive pulmonary disease (COPD) often suffer from cachexia and malnutrition. Less is known about body composition and nutritional behaviour in patients with advanced COPD and pulmonary emphysema. METHODS: We performed a single-center prospective analysis of patients with COPD GOLD III/IV. Metabolic parameters, dietary and exercise behavior, lung function, exercise capacity and body composition by bioelectrical impedance analysis (BIA) were analyzed. Patients with severe emphysema (emphysema index [EI] >20%) were compared to patients with mild emphysema (…

Clinical Outcomes of Patients with Combined Idiopathic Pulmonary Fibrosis and Emphysema in the IPF-PRO Registry.

PURPOSE: To assess the impact of concomitant emphysema on outcomes in patients with idiopathic pulmonary fibrosis (IPF). METHODS: The IPF-PRO Registry is a US registry of patients with IPF. The presence of combined pulmonary fibrosis and emphysema (CPFE) at enrollment was determined by investigators' review of an HRCT scan. Associations between emphysema and clinical outcomes were analyzed using Cox proportional hazards models. RESULTS: Of 934 patients, 119 (12.7%) had CPFE. Compared with patients with IPF alone, patients with CPFE were older (median 72 vs 70 years); higher proportions were
