Spontaneous Nystagmus in the Dark in an Infantile Nystagmus Patient May Represent Negative Optokinetic Afternystagmus.

Abnormal projection of the optic nerves to the wrong cerebral hemisphere transforms the optokinetic system from its usual negative feedback loop to a positive feedback loop with characteristic ocular motor instabilities including directional reversal of the optokinetic nystagmus (OKN) and spontaneous nystagmus, which are common features of infantile nystagmus syndrome (INS). Visual input plays a critical role in INS linked to an underlying optic nerve misprojection such as that often seen in albinism. However, spontaneous nystagmus often continues in darkness, making the visual, sensory-driven

Postural Ataxia in Cerebellar Downbeat Nystagmus: Its Relation to Visual, Proprioceptive and Vestibular Signals and Cerebellar Atrophy.

BACKGROUND: The cerebellum integrates proprioceptive, vestibular and visual signals for postural control. Cerebellar patients with downbeat nystagmus (DBN) complain of unsteadiness of stance and gait as well as blurred vision and oscillopsia. OBJECTIVES: The aim of this study was to elucidate the differential role of visual input, gaze eccentricity, vestibular and proprioceptive input on the postural stability in a large cohort of cerebellar patients with DBN, in comparison to healthy age-matched control subjects. METHODS: Oculomotor (nystagmus, smooth pursuit eye movements) and postural (po

Clinical Characteristics and Longitudinal Videonystagmographic Monitoring in an Adolescent with Vimentin Antibody-Positive Cerebellar Ataxia: A Case Report.

Background. Recently, vimentin IgG has been identified as a novel astrocytic autoantibody in patients with inflammatory meningoencephalomyelitis with prominent cerebellar involvement. This antibody has been reported to be associated with a characteristic inflammatory clinical syndrome. However, the functional impact of vimentin IgG on vestibulocerebellar pathways—particularly specific oculomotor abnormalities and their dynamic evolution during immunotherapy—remains poorly understood. Case Description A 15-year-old girl developed gait instability and dysarthria following an upper respiratory in

Apathy in Alzheimer's disease: Eye movements characteristics and neurostructural basis.

BACKGROUND: We sought to evaluate the characteristics of eye movements in Alzheimer's disease (AD) patients with apathy (AD-A) and their ability to identify AD-A and explore the shared neurostructure of eye movements and apathy. METHODS: Total 32 normal controls, 36 AD-A, and 72 AD with no apathy (AD-NA) patients were recruited. Parameters of smooth pursuit, fixation, prosaccade, and antisaccade were compared among the three groups. Correlation analyses were conducted among apathy score, eye movement parameters, and gray matter volume (GMV) in AD patients. Receiver operating characteristic cu

Correlation between non-motor symptoms and eye movements in Parkinson's disease patients.

INTRODUCTION: Parkinson's disease (PD) presents with a variety of non-motor symptoms such as sleep disorders, hyposmia, pain, cognitive dysfunction, neuropsychiatric and autonomic symptoms. One of the most neglected motor symptoms is the impairment of eye movements, which occurs in 75% of PD patients. The aim of our study was to investigate the relationship between the severity of non-motor symptoms and the impairment of different types of eye movements. METHODS: We conducted a cross-sectional study with idiopathic PD patients in which non-motor symptoms were assessed using standardised scale

Utility of a novel perceptual-motor control test for identification of sport-related concussion beyond current clinical assessments.

Clinically feasible metrics, that can inform the concussion recovery decision making process by evaluating a unique domain beyond current testing domains (e.g., balance, neurocognition, symptoms, vestibular/ocular function) are still in need. The purpose of this study was to compare perceptual-motor control in adolescent athletes ≤21 days of sport-related concussion and healthy controls and evaluate the association of perceptual-motor control to the outcomes of commonly-used sport-related concussion clinical assessments. Athletes (age: 12-18 years) with sport-related concussion (n = 48) and he

Predominant cortical dysfunction in Guadeloupean parkinsonism.

Atypical parkinsonism is extremely frequent in Guadeloupe and may have an environmental cause. One-half of the patients with this tauopathy have dopa-resistant parkinsonism, tremor, subcortical dementia and abnormal eye movements suggestive of progressive supranuclear palsy (PSP). They also have hallucinations, dysautonomia, which are not characteristic of PSP. Furthermore, the oculomotor abnormalities and the tremor, which is jerky, differ from what is observed in classical PSP patients. We therefore undertook an electrophysiological study to characterize these features in greater detail. Nin

Do the clinical features in infantile-onset saccade initiation delay (congenital ocular motor apraxia) correlate with brain magnetic resonance imaging findings?

BACKGROUND: Infantile-onset saccade initiation delay (ISID) is a defect in saccade initiation. Other features may include impaired smooth ocular pursuit, developmental delay, hypotonia, and ataxia. Brain magnetic resonance imaging (MRI) can be normal or show supratentorial or infratentorial abnormalities. Our aim was to correlate the clinical features of ISID with brain MRI findings. METHODS: Detailed review of the English medical literature between 1952 and 2012 revealed 67 studies with possible ISID. Patients without a brain MRI or with inadequate information, Joubert syndrome, neurodegener

Ophthalmic features of ataxia telangiectasia-like disorder.

INTRODUCTION: Ataxia telangiectasia (AT) is a recessive neurodegenerative disease due to a faulty repair mechanism for breaks in double-stranded DNA (ATM mutation). Ophthalmic features of AT include conjunctival telangiectasia, strabismus, saccadic dysfunction with head thrusts, and convergence insufficiency. Ataxia telangiectasia-like syndrome (ATLD) is a more recently recognized condition due to homozygous mutation in MRE11, a gene also involved in the cellular repair response to double-stranded DNA breaks; ophthalmic features of ATLD are not well described. The purpose of this article is to

Central oculomotor disturbances and nystagmus: a window into the brainstem and cerebellum.

BACKGROUND: Oculomotor disturbances and nystagmus are seen in many diseases of the nervous system, the vestibular apparatus, and the eyes, as well as in toxic and metabolic disorders. They often indicate a specific underlying cause. The key to diagnosis is systematic clinical examination of the patient's eye movements. This review deals mainly with central oculomotor disturbances, i.e., those involving smooth pursuit, saccades, gaze-holding, and central types of nystagmus. METHODS: We searched the current literature for relevant publications on the diagnosis and treatment of oculomotor distur

[Oculomotor system and multiple sclerosis].

Disorders of ocular motility due to multiple sclerosis can cause significant symptoms, such as blurred vision, diplopia, oscillopsia and dizziness. The spectrum of ocular motor disorders is broad due to the multifocal nature of the disease. The most frequent disorders include dysmetric saccades, impaired smooth pursuit, internuclear ophthalmoplegia (INO), impaired vestibulo-ocular reflex, misalignment of visual axes, impaired vergence and gaze-evoked nystagmus. Patients with abnormal eye movements are more handicapped than patients without ocular motility disorders as eye movement disorders an

Bilateral pseudo-internuclear ophthalmoplegia in myasthenia gravis.

A 38-year-old male with bilateral pseudo-internuclear ophthalmoplegia (-INO) in myasthenia that could have been misdiagnosed as INO in multiple sclerosis is reported. He experienced fluctuating symptoms including double vision, imbalance, and tinnitus. His eye movements simulated bilateral INO, with a downshoot in abduction. After thymectomy, his eye movements became normal. From our case and a review of the literature, we propose that ptosis, downshoot, and fatigability are likely to be signs of pseudo-INO in myasthenia, whereas an impaired vertical smooth pursuit is unlikely. Dissociated nys

Visual-vestibular interaction in multiple sclerosis.

Visual modulation of the vestibuloocular reflex (VOR) was analyzed in 20 patients with multiple sclerosis who had no vestibular or ocular motor symptoms. Visual suppression of the VOR was impaired in 75% of patients. VOR gains in darkness were elevated in 35% of patients. Elevated VOR gain in darkness is attributed to vestibular adaptation to defective smooth pursuit. This vestibular plasticity achieved retinal image stability by matching eye velocity to head velocity when stationary targets were viewed. The high incidence of impaired visual suppression of the VOR rivals the sensitivity of oth

[Gaze control: role of the parietal cortex].

Eye movements constitute one of the most basic means of interacting with our environment, allowing to orient to, localize and scrutinize the variety of potentially interesting objects that surround us. In this review we discuss the role of the parietal cortex in the control of saccadic and smooth pursuit eye movements, whose purpose is to rapidly displace the line of gaze and to maintain a moving object on the central retina, respectively. From single cell recording studies in monkey we know that distinct sub-regions of the parietal lobe are implicated in these two kinds of movement. The middl

Predictive fixations to target bounces in an interception task.

Predictive control enables humans to anticipate future events by combining sensory feedback with internal models. In interception tasks, such mechanisms could allow the visual system to estimate future target positions, supporting timely and accurate motor responses. Here, we investigated predictive gaze behavior in a visuomotor task where participants used a joystick to intercept a moving target that rebounded within a circular arena. Eye movements were classified into fixations, smooth pursuit, and saccades using a velocity-based method. The arena's geometry constrained rebound angles and fa

From Gaussian blobs to naturalistic videos: Comparison of oculomotor behavior across different stimulus complexities.

Research on eye movements has primarily been performed in two distinct ways: (1) under highly controlled conditions using simple stimuli such as dots on a uniform background, or (2) under free-viewing conditions with complex images, real-world movies, or even with observers moving around in the world. Although both approaches offer important insights, the generalizability among eye movement behaviors observed under these different conditions is unclear. Here, we compared eye movement responses to video clips showing moving objects within their natural context with responses to simple Gaussian

An internal clock generates repetitive predictive saccades.

Previously we demonstrated the presence of a behavioral phase transition between reactive and predictive eye tracking of alternating targets. Prior studies of repetitive movements have proposed that an "internal clock" is the neural mechanism by which interval timing is achieved. In the present report we tested whether predictive oculomotor (saccade) tracking is based on an internal time reference (clock) by examining the effect of transient perturbations to the periodic pacing stimulus. These perturbations consisted of altering the timing of the stimulus (abruptly increasing or decreasing the

Pursuit eye movement deficits in autism.

Oculomotor studies provide a novel strategy for evaluating the functional integrity of multiple brain systems and cognitive processes in autism. The current study compared pursuit eye movements of 60 high-functioning individuals with autism and 94 intelligence quotient, age and gender matched healthy individuals using ramp and oscillating target tasks. Individuals with autism had normal pursuit latency, but reduced closed-loop pursuit gain when tracking both oscillating and ramp targets. This closed-loop deficit was similar for leftward and rightward pursuit, but the difference between individ
