Identification and Function of Fibrocytes in Skeletal Muscle Injury Repair and Muscular Dystrophy.

We identified and characterized the function of CD45+/collagen I+ fibrocytes in acutely injured skeletal muscle of wild-type (WT) and Ccr2-/- mice, and in quadriceps and diaphragm muscles of mdx5cv mice, a mouse model for Duchenne muscular dystrophy. Fibrocytes were not detected in peripheral blood of WT mice after acute muscle injury or mdx5cv mice. Fibrocytes were detected in acutely injured muscles and in mdx5cv quadriceps and diaphragm muscles. These cells expressed F4/80 and CCR2, and they were mostly Ly6Clo They expressed a low level of collagens but a high level of profibrotic growth fa

The Use of Hamstring Fatigue to Reduce Quadriceps Inhibition After Anterior Cruciate Ligament Reconstruction.

Arthrogenic muscle inhibition, an inability to fully activate the quadriceps muscles, has been persistently observed after anterior cruciate ligament reconstruction (ACLr) surgery. Reductions in quadriceps activation may be partly due to the flexion reflex pathway, hamstrings activation, and reciprocal quadriceps inhibition. Since central fatigue has been shown to modify hamstring excitability and change the hamstring reflex response, hamstring fatigue might alleviate quadriceps muscle inhibition by counteracting the flexion reflex. In this study, nine young adult athletes (age: M = 19.9 years…

Bioenergetics and intermuscular fat in chronic obstructive pulmonary disease-associated quadriceps weakness.

INTRODUCTION: Chronic obstructive pulmonary disease (COPD) is associated with metabolic abnormalities in muscles of the lower limbs, but it is not known whether these abnormalities are generalized or limited to specific muscle groups, nor is there an easy way of predicting their presence. METHODS: Metabolism in the quadriceps and biceps of 14 COPD patients and controls was assessed during sustained contraction using 31-phosphorus magnetic resonance spectroscopy ((31) P MRS). T1 MRI was used to measure quadriceps intermuscular adipose tissue (IMAT). RESULTS: COPD patients had prolonged quadri

Insulin resistance is associated with skeletal muscle weakness in COPD.

BACKGROUND AND OBJECTIVE: Quadriceps weakness is seen across all GOLD stages of COPD and is associated with increased morbidity and mortality. As quadriceps weakness is only weakly associated with FEV1 , mechanisms other than airflow obstruction are implicated. We tested the hypothesis that insulin resistance contributes to skeletal muscle weakness in people with stable COPD. METHODS: Fifty-one COPD patients (no exacerbations preceding 6 weeks, no rehabilitation preceding 3 months) without known diabetes mellitus underwent assessment of skeletal muscle, including measurement of quadriceps max

Respiratory and limb muscles' ability to repeatedly generate maximal isometric strength in patients with intensive care unit-acquired weakness: an observational study.

BACKGROUND: Intensive care unit-acquired weakness (ICU-Aw) is a prevalent complication in critically ill patients, affecting both limb and respiratory muscles, individually or concurrently. The precise mechanisms by which muscle weakness influences the distinct functional roles of each muscle group remain to be fully elucidated. The objective of this study was to compare the time course evolution of inspiratory and quadriceps muscles strength during repeated maximal isometric contractions in patients with limb muscles and inspiratory muscles weakness. METHODS: A single-center, observational s…

A case report: New technique of proximal lengthening for treatment of injection induced rectus femoris muscle contracture.

UNLABELLED: Contracture of quadriceps femoris muscle is an uncommon condition in clinical practice. It was suggested that intramuscular injections into the thigh for children are sometimes the cause of this condition. CASE REPORT: The case presents a new surgical technique for the treatment of isolated rectus femoris muscle contracture. The patient is a 6- year-old boy complaining of external rotation of the left leg when he walks and inability to squat. Clinical examination showed positive Ely's test which indicated isolated rectus femoris muscle contracture. The patient underwent proximal l

Abnormalities in quadriceps-hamstring strength. Relationships in polymyositis and dermatomyositis.

Muscle strength determined as torque was measured in patients with dermatomyositis and polymyositis. Patients demonstrated weakness in both quadriceps and hamstring muscle groups. The ratio of quadriceps to hamstring torque (Q:H) was 1.93 +/- 0.38 in controls and 1.60 +/- 0.62 in patients. Patients with Q:H ratios that were below this mean were older, weaker, and were treated with greater doses of prednisone. Fifty-six percent of patients studied initially had low Q:H ratios, and 64% of all patients had low Q:H ratios at some time. The Q:H ratio in patients with steroid and thyroid myopathy, h

Thigh strength and activation as predictors of knee biomechanics during a drop jump task.

PURPOSE: To examine whether normalized quadriceps and hamstring strength would predict quadriceps and hamstring muscle activation amplitudes and whether these neuromuscular factors would predict knee kinematics and kinetics during a drop jump task. METHODS: Thirty-nine females and 39 males were measured for isometric quadriceps and hamstring strength and were instrumented to obtain surface electromyography, kinematic, and kinetic measures during the initial landing of a drop jump. Multiple linear regressions first examined the relationship between thigh strength and activation then examined w

Is there a difference in knee biomechanics due to fatigue of the quadriceps versus hamstrings during landing on a single limb? a randomized comparative laboratory study.

BACKGROUND: The quadriceps and hamstring muscles play a central role in absorbing impact forces and maintaining dynamic knee stability during landing tasks. Together, these muscle groups also strongly influence knee joint kinematics and kinetics. This study aimed to compare the effects of induced fatigue of the quadriceps and hamstring muscles on the kinematics and kinetics of the knee joint during a single-leg landing in a forward-drop jump task. METHODS: Of the 108 athletes (aged 20-25 years) randomized to either the quadriceps or hamstring fatigue group, 93 completed the study and were ana

Thigh ultrasound monitoring identifies muscle atrophy in mechanically ventilated pediatric patients.

Over the last decade, ultrasonography has taken on an increasingly important role in the daily management of critically patients and has recently been proposed as a means of measuring muscle volume and architecture. This study had two main aims: to monitor for the onset of muscle atrophy in mechanically ventilated pediatric patients during stays in a pediatric intensive care unit based on quadriceps femoris muscle thickness measurements and to study whether demographic and clinical variables have an impact on muscle loss in critically children. The study followed a prospective, observational,

Skeletal muscle morphology and exercise response in congenital generalized lipodystrophy.

OBJECTIVE: Congenital generalized lipodystrophy (CGL) is an autosomal recessive genetic disorder characterized by almost complete absence of adipose tissue, muscular appearance, and severe insulin resistance since birth. We investigated whether insulin resistance in CGL patients is associated with abnormal muscle morphology and whether increased muscularity imparts increased muscle strength and exercise capacity RESEARCH DESIGN AND METHODS: We obtained quadriceps muscle biopsies to study muscle fiber types and capillary density in three African-American women (aged 17-20 years) with CGL. We a…

Diagnosis and management of quadriceps strains and contusions.

Injuries to the quadriceps muscle group occur frequently in sports and athletic activities. Muscle strains and contusions constitute the majority of these injuries. The clinical presentation and assessment of quadriceps strains and contusions are reviewed along with discussion of appropriate imaging used in diagnosis. Treatment protocols for acute injuries are reviewed including rehabilitation techniques frequently utilized during recovery. Special consideration is given to discussing the criteria for return to sports for athletes after injury. Myositis ossificans is a potentially disabling co…

Obesity and recovery of muscle strength after anterior cruciate ligament reconstruction in pediatric patients.

PURPOSE:: To explore factors influencing muscle strength after anterior cruciate ligament (ACL) reconstruction (ACLR) in pediatric patients. We hypothesized that obesity/overweight, autograft hamstring tendon, and concomitant injuries would be associated with slower muscle recovery. METHODS:: We retrospectively reviewed the records of pediatric ACLR patients during a 3-year period. Muscle recovery was defined as ≥85% of peak torque compared with the contralateral side. We categorized patients as either obese/overweight or normal weight. Statistical analysis was performed using Mann-Whitney U,

Altered Energetics of Exercise Explain Risk of Rhabdomyolysis in Very Long-Chain Acyl-CoA Dehydrogenase Deficiency.

Rhabdomyolysis is common in very long-chain acyl-CoA dehydrogenase deficiency (VLCADD) and other metabolic myopathies, but its pathogenic basis is poorly understood. Here, we show that prolonged bicycling exercise against a standardized moderate workload in VLCADD patients is associated with threefold bigger changes in phosphocreatine (PCr) and inorganic phosphate (Pi) concentrations in quadriceps muscle and twofold lower changes in plasma acetyl-carnitine levels than in healthy subjects. This result is consistent with the hypothesis that muscle ATP homeostasis during exercise is compromised i…
