BACKGROUND: Radiodermatitis in its acute and chronic presentations represents a set of specific adverse effects resulting from ionizing radiations. Skin-field cancerization is another condition associated to or independent of clinically recognizable radiodermatitis. This condition leads to the risk of later occurrence of skin malignancies.

Indications for radiotherapy of benign dermatoses have decreased markedly during the past decades. The remaining indications are reviewed. Side effects of ionizing radiation therapy are discussed in detail, with special emphasis on radiodermatitis and radiation cancer of the skin, mutagenic and genetic effects of x rays, and induction of carcinogenesis of interanl organs. Thyroid cancer is used as an example to discuss modern theories of radiocarcinogenesis and risk coefficients of various organ systems. Guide lines for radiotherapy of benign dermatoses are proposed.

Six patients with mycosis fungoides were treated with topical mechlorethamine hydrochloride for periods of two to four years. Clinical and histological studies for radiomimetic and radiodermatitis-like effects failed to demonstrate any abnormalities. The only observed changes were generalized hyperpigmentation of the skin and melanin-containing melanophages in the papillary dermis. We consider that the long-term use of topical mechlorethamine may be a safe form of therapy, but that a continuous indefinite follow-up of patients on this medication should be mandatory.

Atypical vascular proliferations (AVP) are a late complication after radiotherapy. Most cases have been reported in female breast cancer patients on the chest wall. These lesions are mostly of the lymphatic type. Herein, we report a blood vascular-type AVP in a male on the neck 60 years after radiotherapy for a benign hemangioma, which makes this case exceptional. We removed the whole chronic radiodermatitis surgically. Histopathology excluded vascular malignancies but confirmed AVP. We discuss the differential diagnoses and treatment.

We describe a patient who developed a porocarcinoma in the nail bed. His profession had exposed him to X-rays for many years, resulting in chronic radiodermatitis of several digits on both hands. He displayed an ulcer in the lateral nail fold of the right third digit which extended into the nail bed. The histology of the lesion was consistent with malignant eccrine poroma. Porocarcinoma has not been previously published in this location and this tumor must be considered in differential diagnosis of subungual malignant tumors.

Cancerous changes occur most frequently in four types of chronic acquired lesions: rare lupus scars and those of long-standing burns, rhinophymas, and radiodermatitis. Insufficient therapy is often applied because of underestimation of potential changes, fear of hemorrhage, or lack of knowledge of the extent of the lesion. The ulcerous forms and those developing in scar tissue have the poorest prognosis, and this also applies to centrofacial basal cell epitheliomas. Recurrences after radiotherapy are of bad omen, and they must be treated by wide surgical excision.

BACKGROUND: In postmastectomy radiation therapy (PMRT), some centres prescribe the use of a tissue-equivalent bolus to the skin to reduce the risk of chest wall recurrence. The addition of bolus causes an increase in the skin dose, which may lead to increased risk of radiodermatitis. A 30-year-old woman developed chronic radiodermatitis and five squamous cell carcinomas within 7 years following a 16-year period of repeated grenz irradiations given to her psoriasis. The estimated cumulative dose of grenz radiation was approximately 3,000 rads (r) to each treated area. This case, illustrating a rather underestimated carcinogenic potential of grenz rays, is interpreted in light of the available literature on the subject. The technical bases of grenz ray therapy are also briefly reviewed.

We describe a patient with angiosarcoma of the scalp arising in an area of radiodermatitis caused by X-ray epilation therapy for scalp ringworm as a child. Radiotherapy has been well documented as a causative factor in the formation of cutaneous malignancies, most notably basal cell and squamous cell carcinoma, but only rarely angiosarcoma. This is now the first reported case of angiosarcoma arising after X-ray epilation therapy for scalp ringworm, and we postulate a causative link between the two events.
