Nuclear medicine techniques have received little attention in the practice of emergency medicine, yet radionuclide imaging can provide valuable and unique information in the management of acutely ill patients. In this review, emphasis is placed on the role of these techniques in patients with bone injuries, non-traumatic bone pain and in those with pleuritic chest pain. New developments such as single photon emission computed tomography (SPECT) in myocardial infarction are outlined and older techniques such as scrotal scintigraphy are reviewed. Radionuclide techniques are discussed in a clinic…

A case of hepatoma was diagnosed by means of radionuclide hepatic blood flow study (hepatic scintiangiography) and static liver-spleen imaging. The hepatic flow study clearly showed arterial blood supply to the right lobe of the liver and provided a strong indication for the diagnosis. Sonographic examination indicated some abnormalities but was less conclusive than radionuclide studies. Subsequent contrast angiogram, computerized tomography scan, positive hepatitis B profile, very high alpha-fetoprotein test, and laparatomy confirmed the diagnosis.

A 40-year-old woman visited our hospital with a several-year history of right hypochondriac pain and vomiting after eating. She had been treated for functional dyspepsia, with no improvement in her symptoms. No gallstones were detected on imaging tests, but papillary insufficiency or dyskinesia of the gallbladder was suspected and biliary scintigraphy was performed. Biliary scintigraphy showed delayed excretion of radionuclides from the gallbladder and bile ducts into the duodenum. We initially suspected papillary dysfunction and performed endoscopic sphincterotomy, but there was no improvemen

Radiopharmaceuticals are biologically active molecules labeled with radionuclides that have advanced the possibility of the nuclear medicine. They support non-invasive, high-resolution diagnostic imaging of molecular and physiological processes in vivo. The techniques such as positron emission tomography (PET) and single-photon emission computed tomography (SPECT) utilize short-lived β⁺ and γ-emitting isotopes to generate highly sensitive, three-dimensional assessments of biological function. In therapeutic applications, radionuclides that emit β⁻ particles, α particles, or Auger electrons ena

Because AIDS patients frequently present with minimal symptomatology, radionuclide imaging with its ability to survey the entire body, is especially valuable. Gallium-67 citrate, the most commonly performed radionuclide study for localizing infection in these patients, is most useful for detecting opportunistic infections, especially in the thorax. A negative gallium scan, particularly when the chest X-ray is unremarkable, rules strongly against pulmonary disease. A negative gallium scan in a patient with an abnormal chest X-ray and Kaposi's sarcoma, suggests that the patient's respiratory dis…

Surgery for primary hyperparathyroidism is successful in 95% of patients, but ectopic glands and anatomic variations in location are causes of surgical failure. The radionuclide imaging agent, technetium (Tc)-99m-sestamibi, in conjunction with subtraction iodine-123 scanning, is a new method of preoperative localization of abnormal parathyroid glands. In a study approved by the Institutional Review Board, 22 patients with primary hyperparathyroidism underwent preoperative evaluation with high-resolution ultrasonography and Tc-99m-sestamibi/I-123 radionuclide scanning for attempted localization…

The concept of death by neurologic criteria (DNC), a medicolegal construct also known as "brain death", is over 50 years old. Ancillary examinations have a well-defined role in this diagnosis, principally when the clinical examination cannot be completely or safely performed. Radionuclide studies, which provide a combination of ease of performance and high accuracy, remain amongst the most recommended ancillary studies in current clinical guidelines. There are in fact 2 discrete categories of radionuclide studies, using hydrophilic or lipophilic radiopharmaceuticals (RPs), which have overlappi

Behçet's syndrome is a multisystem disease; however, cardiac involvement in this disorder has been relatively less recognized. Noninvasive imaging methods may reveal a cardiac disorder which has not manifested itself clinically. In this study, radionuclide ventriculography and Doppler echocardiography were performed in 24 patients with Behçet's disease to assess the left ventricular systolic and diastolic function. Patients had neither known heart disease nor cardiac symptoms. Radionuclide ventriculography was performed using 99mTc-labeled red blood cells. Ejection fraction (EF), EF fraction i

Affibody molecules, nonimmunoglobulin scaffold proteins, have a high potential as probes for molecular imaging of different molecular targets. One of the molecular targets for radionuclide diagnosis and therapy is B7-H3 (known as CD276), which is overexpressed in various cancers, whereas its expression is low in most normal organs and tissues. The visualization of expression levels of B7-H3 has been performed using Affibody molecules labeled with Tc-99m. However, radionuclide molecular imaging using PET offers several advantages such as superior sensitivity, quantitation accuracy, and better s

Among different imaging modalities, Positron Emission Tomography (PET) gained importance in routine hospital practice depending on ability to diagnose diseases in early stages and tracing of therapy by obtaining metabolic information. The combination of PET with Computed Tomography (CT) forms hybrid imaging modality that gives chance to obtain better images having higher resolution by fusing both functional and anatomical images in the same imaging modality at the same time. Therefore, better contrast agents are essentially needed. The advance in research about developing drug delivery systems

A patient with Prinzmetal's variant angina (PVA) who developed an inferolateral myocardial infarction with right-ventricle involvement was studied using sequential radionuclide imaging until 5 months after the acute event. The patient also underwent-contrast ventriculography and coronary-artery angiography. Equilibrium-gated radionuclide angiography (EGRA) revealed the localization of ventricular dysfunction, the results obtained being in agreement with haemodynamic data. We emphasize the usefulness of the parameters obtained using EGRA (regional wall motion, systolic and diastolic parameters)…

Computed transaxial tomography (CTT) has become the major imaging tool in the evaluation of any intracerebral lesion, including those due to traumatic injury. Expense and availability sometimes dictate that radionuclide brain scintigraphy (RBS) be performed in place of CTT. Certain situations exist where RBS may be used as a primary tool. These include circumstances where assessment of brain perfusion may be essential, e.g., determination of brain death. Radionuclide cisternography still remains useful in evaluating patients with normal pressure hydrocephalus and cerebrospinal fluid leakage.
