[Astigmatism correction by femtosecond laser].

Through the creation of astigmatic keratotomies, femtosecond laser can correct congenital or post-surgical astigmatism. We have been using an optimized treatment nomogram, which offers superior and more predictable results than manual incisional techniques. Femtosecond arcuate incisions allow a reduction of over 50% of the preoperative astigmatism. Nonetheless, the efficacy can likely be improved further by refining these nomograms specific to the femtosecond laser.

Evolution of Keratoconus: From Diagnosis to Therapeutics.

This review describes the evolution of the diagnosis and treatment of keratoconus from the earliest written description to present day. The first description was provided in 1736 by Benedict Duddell who described the prominent corneas of a fourteen-year-old boy. Throughout the 19th century, a variety of surgical procedures were proposed to manage the disease, such as surgically repositioning the pupil away from the cone, iris incarceration to produce a slit-like pupil, cauterization of the cone to produce a scar, and full thickness elliptical excision of the cone. Despite the ingenuity of thes

Simple device for quantifying the influence of halos after lasik surgery.

We present a simple device (called a halometer) to detect and quantify the phenomenon of halos after certain surgical procedures, such as refractive surgery. The task of the subject consists basically of discriminating, after dark adaptation, a small light source around a central high-luminance stimulus. The device, which is connected to a computer, provides a disturbance index to measure the effect of halos on the observer's vision. Tested with normal subjects and patients after LASIK (laser in situ keratomileusis) surgery, this apparatus proved sufficiently sensitive to quantify the halos in

Comparison of Long-Term Outcomes of the Lamellar and Penetrating Keratoplasty Approaches in Patients with Keratoconus.

We compared the visual and refractive outcomes, intraocular pressure (IOP), endothelial cell loss (ECL), and adverse events in keratoconus patients after deep anterior lamellar keratoplasty (DALK) and penetrating keratoplasty (PK) with the best corrected visual acuity (BCVA) below 0.3 (logMAR 0.52). This is a prospective, comparative cohort study of 90 eyes (90 patients) with a clinical diagnosis of keratoconus. Patients underwent a complete eye examination before the surgical approach, 6 and 12 months postoperatively that consisted of BCVA, refractive astigmatism (AS), central corneal thickne

A new approach of aspheric intralamellar keratoprostheses optic design made with poly(2-hydroxy ethylmethacrylate) hydrogel.

Keratoprosthesis (KPro) is a surgical procedure largely confined to end-stage corneal blindness correction, where artificial cornea substitutes the native tissue. Though the problem of bio integration was addressed partially by strategic utilization of synthetic polymers and native tissue, major challenges like optical performance and design-associated post-operative complications of KPro were overlooked. Herein, a novel intralamellar KPro design is conceptualized to address these challenges using a light-transparent poly(2-hydroxy ethylmethacrylate) (pHEMA) hydrogel with good shape memory. pH

Necrotizing scleritis as a complication of cosmetic eye whitening procedure.

BACKGROUND: We report necrotizing scleritis as a serious complication of a cosmetic eye whitening procedure that involves the use of intraoperative and postoperative topical mitomycin C. FINDINGS: This is a single case report. A 59-year-old Caucasian male with a history of blepharitis status post uncomplicated LASIK refractive surgery reported chronic conjunctival hyperemia for 15 years prior to undergoing a cosmetic eye whitening procedure. He presented to our clinic 12 months after the cosmetic eye whitening procedure with progressive bilateral necrotizing scleritis and scleral calcificatio

Intrastromal corneal ring segments: how successful is the surgical treatment of keratoconus?

This review evaluates the outcomes of intrastromal corneal ring segment (ICRS) implantation for the treatment of keratoconus considering a new grading system based on the preoperative visual impairment of the patient. Additionally, a five-year follow-up analysis of patients with stable and progressive keratoconus is performed in order to assess the long term stability of the surgical procedure. Corrected distance visual acuity decreased statistically significantly in patients with mild keratoconus (P < 0.01) but statistically significantly increased in all other grades (P < 0.05). The improvem

Review of Presbyopia Treatment with Corneal Inlays and New Developments.

Presbyopia may represent the largest segment of refractive errors that is without an established and effective refractive surgery treatment. Corneal Inlays are materials (synthetic or allogenic) implanted in the stroma of patients' corneas to improve presbyopia. These inlays, introduced into the United States in 2015 via the small-aperture corneal inlay (KAMRATM, SightLife Surgical/CorneaGen, Seattle, Washington, United States), were met with an initial wave of enthusiasm. Subsequent models like the shape-changing corneal inlay (RAINDROPTM, Revision Optics, Lake Forest, California, United Stat
