The upper airway: congenital malformations.

The upper airway extends from the nasal aperture to the subglottis and can be the site of multiple types of congenital malformations leading to anatomical or functional obstruction. This can cause severe respiratory distress. Newborns are obligate nasal breathers; therefore nasal obstruction can lead to airway compromise and respiratory distress. The etiologies are varied and include, choanal atresia, pyriform aperture stenosis, and rarely tumors such as glioma, encephalocele, teratoma, or dermoid. More common upper airway congenital anomalies include laryngomalacia, vocal cord paralysis, and

Electrical injuries.

Electrical injury is a relatively infrequent but potentially devastating form of multisystem injury with high morbidity and mortality. Most electrical injuries in adults occur in the work-place, whereas children are exposed primarily at home. In nature, electrical injury occurs due to lightning, which also carries the highest mortality. The severity of the injury depends on the intensity of the electrical current (determined by the voltage of the source and the resistance of the victim), the pathway it follows through the victim's body, and the duration of the contact with the source of the cu

[A case of periodic thyrotoxic paralysis].

Periodic thyrotoxic paralysis (TPP) is a disease that usually affects Asian men from 20 to 50 years. It is secondary to genetic mutations of ion channels for the transport of sodium and potassium. It is characterized by recurrent and brutal bouts of paresis / flaccid paralysis, without sensory involvement, in a context of hyperthyroidism. Seizures preferentially affect proximal muscles and are often associated with severe hypokalemia. However, serious repercussions are rare, except in cases of extreme weakness of the respiratory muscles or cardiac arrhythmia induced by hypokalemia. The treatme

[Hereditary hypokaliaemic periodic paralysis in a 13-year-old boy].

Hypokaliaemic periodic paralysis is a rare hereditary neuro-muscular disease caused by an error in the ion-canals in muscle cells resulting in decreased excitabiliy. It presents itself in the late childhood or teenage years with a periodic paralysis without involving respiratory and heart muscles. Our patient was a 13-year-old boy, who woke up with decreased strength in arms and legs after excessive physical activity and a high carbo-hydrate intake. Tests showed a low P-potassium level. The patient had full remission of his symptoms within 24 hours.

Unilateral laryngeal paralysis in a newborn with congenital varicella syndrome.

We report a case of a newborn with unilateral laryngeal paralysis due to congenital varicella syndrome. The patient needed neonatal resuscitation with positive pressure ventilation for severe respiratory failure. Fiberoptic bronchoscopy showed left unilateral laryngeal paralysis. From the first week of life he presented left Horner's syndrome and difficult swallowing. He was hospitalised during the first 3 months of life for recurrent aspiration pneumonia and failure to thrive. At 11 months an ophthalmic evaluation with Ret-Cam showed a chorioretinal scar in the macular region that allowed the

Comparison of unilateral arytenoid lateralization and ventral ventriculocordectomy for the treatment of experimentally induced laryngeal paralysis in dogs.

This study evaluated changes in respiratory function in dogs with experimentally induced laryngeal paralysis treated with either unilateral arytenoid lateralization or ventral ventriculocordectomy, and compared the effectiveness of these procedures. Evaluation consisted of clinical assessment and tidal breathing flow volume loop and upper airway resistance measurements. Carbon dioxide and doxapram hydrochloride were used as respiratory stimulants. Initially, all dogs improved clinically after corrective surgery. However, by the end of the study, laryngeal collapse had developed in 2 of 5 dogs

Paralysis due to renal potassium wasting : an unusual presentation of leptospirosis.

Renal involvement in leptospirosis and its association with hypokalaemia is known to occur. Hypokalaemia may lead to paralysis. Herein such a case of hypokalaemic paralysis in leptospirosis is being reported. A 45-years-old male presented with 16 hours duration of myalgia, conjunctival suffusion, progressive flaccid quadriparesis, respiratory muscle weakness and dysphasia. Forty-eight hours later, he developed fever, oliguria and abnormal liver function test. Laboratory investigations showed severe hypokalaemia and serological evidence of leptospirosis. He was treated with intravenous potasium

Renal tubular acidosis complicated with hypokalemic periodic paralysis.

Three Chinese girls with hypokalemic periodic paralysis secondary to different types of renal tubular acidosis are presented. One girl has primary distal renal tubular acidosis complicated with nephrocalcinosis. Another has primary Sjögren syndrome with distal renal tubular acidosis, which occurs rarely with hypokalemic periodic paralysis in children. The third has an isolated proximal renal tubular acidosis complicated with multiple organ abnormalities, unilateral carotid artery stenosis, respiratory failure, and consciousness disturbance. The diagnostic evaluation and emergent and prophylact

Cervical epidural anesthesia for combined neck and upper extremity procedure: a pilot study.

In a prospective pilot study, we evaluated the possibility of performing a total parathyroidectomy with parathyroid gland implantation into the forearm (a combined neck and upper extremity procedure) under cervical epidural anesthesia (CEA) at C6-7 level using ropivacaine. The indication for CEA was the patient's choice or a previous procedure on the neck with unilateral vocal cord paralysis. Anesthesia was induced by 10 mL of 0.75% ropivacaine plus 10 mug of sufentanil in 2 mL. Block onset time, success rate, analgesia, sensory block extent, changes in respiratory and hemodynamic variables, c

VP1 is the primary determinant of neuropathogenesis in a mouse model of enterovirus D68 acute flaccid myelitis.

UNLABELLED: Enterovirus D68 (EV-D68) is an emerging pathogen that can cause severe respiratory and neurologic disease [acute flaccid myelitis (AFM)]. Intramuscular (IM) injection of neonatal Swiss Webster (SW) mice with US/IL/14-18952 (IL52), a clinical isolate from the 2014 EV-D68 epidemic, results in many of the pathogenic features of human AFM, including viral infection of the spinal cord, death of motor neurons, and resultant progressive paralysis. In distinction, CA/14-4231 (CA4231), another clinical isolate from the 2014 EV-D68 outbreak, does not cause paralysis in mice, does not grow in

Laryngeal paralysis in dogs.

Laryngeal paralysis is a common cause of upper airway obstruction in older, large-breed dogs and is likely associated with a generalized polyneuropathy in most animals. Surgical therapy is frequently indicated, and UAL is currently the recommended treatment. Respiratory signs significantly improve in most patients after surgery; however, postoperative complication rates can be high, and patients have a lifelong risk of developing respiratory tract disease.

Guillain-Barre Syndrome following SARS-COVID-19 Infection: A Case Report from India.

Severe acute respiratory syndrome coronavirus 2 (SARS-COVID-19) is a novel coronavirus that started in Wuhan City in China in December 2019. It can cause acute respiratory infection. Guillain-Barre syndrome (GBS) is an autoimmune disease characterized by rapidly progressing ascending paralysis that is triggered by an infection or immune stimulation which produces an abnormal immune response that targets peripheral nerves. In most cases, it is preceded by a bacterial or viral infection. This is a case of a 36-year-old male patient from India who developed progressive acute flaccid paralysis aft

Diaphragm pacing for respiratory insufficiency.

Diaphragm pacing (DP) by electrical stimulation of the phrenic nerve offers important advantages to a highly select group of patients with respiratory paralysis. The patient wears an external radiofrequency (RF) transmitter over an implanted receiver, and a stimulating current is induced without the need for any transcutaneous wires. We review the conditions and requirements of patients who may benefit most from DP. We outline the preoperative evaluation and procedures for surgical implantation. We discuss the risk of diaphragmatic fatigue posed by initiation of DP and the use of gradual condi…

Acute flaccid rhombencephalomyelitis with radiculitis in a child with an enterovirus A71 infection seen for the first time in Denmark: a case report.

BACKGROUND: Acute flaccid myelitis is a serious condition of the spinal cord. More than 80% of patients experience a mild respiratory illness or fever consistent with a viral infection prior to acute flaccid myelitis development. Enterovirus A71 is known to circulate in Denmark, and has previously been associated with severe neurological symptoms. In this case report we describe acute flaccid rhombencephalomyelitis with radiculitis in an infant with an enterovirus infection. CASE PRESENTATION: The 8-month-old male of Asian origin presented with fever and gastrointestinal symptoms, followed by

Bilateral Diaphragmatic Nerve Paralysis Secondary to Cervical Spondylosis: A Rare Case.

Bilateral diaphragmatic nerve paralysis due to cervical spondylosis is an extremely rare condition with only one previously reported case. We present a 64-year-old Japanese male with a history of left diaphragmatic nerve paralysis who developed sudden nocturnal dyspnea. Physical examination revealed orthopnea and type 2 respiratory failure. Imaging studies showed bilateral diaphragmatic elevations and cervical spine stenosis at C3/C4 and C4/C5 levels. Pulmonary function tests demonstrated a significant reduction in vital capacity, particularly in the supine position. After excluding other pote

"C3, 4, 5 Keeps the Diaphragm Alive." Is phrenic nerve palsy part of the pathophysiological mechanism in strangulation and hanging? Should diaphragm paralysis be excluded in survived cases?: A review of the literature.

The phrenic nerve arises in the neck. It is formed from C3, C4, and C5 nerve fibers and descends along the anterior surface of the scalenus anterior muscle before entering the thorax to supply motor and sensory input to the diaphragm. Its anatomic location in the neck leaves the nerve vulnerable to traumatic injury. Phrenic nerve injury can arise as a result of transection, stretching or compression of the nerve, and may result in paralysis of the diaphragm. Consequences of diaphragm paralysis include respiratory compromise, gastrointestinal obstruction, and cardiac arrhythmias. There may be s

[The post-operative recurrent laryngeal nerve paralysis and its course (author's transl)].

Benign diseases of the thyroid may also be the cause of pre-operative paralysis of the recurrent laryngeal nerves. the frequency of palsies in uncomplicated strumectomies is approximately 4 to 5%. A markedly higher risk exists in endothoracic goitres which must be delivered by thoracotomy, in recurrencies and thyroiditis. Dissection, crush lesions, strain and nerves mistakenly pulled into a ligature are the most frequent causes of injury; haematoma, oedema, heat by diathermy, lesions caused by a scar and a neuritis of the nerve are less common. 30 to 50% of unilateral laryngeal nerve palsies a
