A review of a small series of cases with idiopathic retroperitoneal fibrosis revealed that two of the patients had been under treatment for gout prior to developing retroperitoneal fibrosis. Investigation showed that other patients in the series had hyperuricaemia. Examination of the retroperitoneal fibrotic tissue demonstrated the presence of urate crystals in unusually high numbers. OBJECTIVE: To describe our initial experience with balloon dilatation of the retroperitoneum.
METHODS: In 12 patients undergoing laparoscopic renal surgery, the retroperitoneum was dilated using the technique of balloon dilatation. This technique is described and the operative procedure performed in each patient is outlined. The postoperative recovery data and complications are also presented.
RESULTS: This procedure provided excellent exposure of the anatomic structures within the retroperitoneal space and simplified the surgical dissection by staying out of the abdominal cavity.

This ultrasonographic finding preceded the development of hematochezia and anemia, and pleural and peritoneal bleeding did not develop. We report a case of a retroperitoneal abscess presenting as necrotizing fasciitis of the thigh from direct spread over the iliac crest. In cases of necrotizing fasciitis of the thigh with no obvious source, an intra-abdominal nidus of infection may be considered.

We report an unusual cause of the pleural effusion due to extravasation of urine from the retroperitoneal space into the thoracic cavity. In our case, the urinoma occurred owing to obstructing urinary tract lesion due to opaque stone. Although rare, urinothorax should be considered when pleural effusion occurs in patients with urinary tract obstruction accompanied by retroperitoneal urinoma.

Out of 267 cases of teratomas diagnosed in the department of pathology, Christian Medical College and Hospital, Ludhiana, Punjab from January 1980 to December 1989, 10 cases occurred in rare sites ie, 3 cases of teratoma in retroperitoneal space, 3 in the floor of the mouth, one each in mediastinum, thyroid, urinary bladder and kidney. A 43-year-old man presented with a retroperitoneal mass, the histological examination of which showed well developed spleen parenchyma with a capsule and fibrous tissue in between normal looking splenic cells. Postoperative 99mTc sulphur colloid scans of his liver and spleen were normal. This is, to our knowledge, the first reported case of an ectopic spleen in the left retroperitoneal space.
