Cough induced stress fracture and arthropathy of the ribs at extreme altitude.

Cough and chest wall pain at high altitude have only received passing mention in the medical literature. Increased minute ventilation of cold dry air at very high altitude is likely to cause airway irritation. This in turn may result in airway drying, mucus production, postnasal drip from vasomotor rhinitis, and bronchospasm acting individually or in combination to stimulate the vagal cough reflex. The cough is exacerbated further at extreme altitudes above 5500 m, and may result in intercostal muscle strain and single or multiple rib fractures. We present a case of multiple cough induced stre…

Sclerosis of a rib associated with a healed fracture of a contiguous thoracic vertebra.

A patient who suffered a fracture of the body of the 10th dorsal vertebra as a result of an automobile accident was examined seven years later. He complained of local pain over the gibbus site. All movements in the back and the joints of the extremities were within normal limits. Radiographs disclosed a healed compressed fracture of the body of the 10th thoracic vertebra with bony bridges contiguous with the 9th and 11th thoracic vertebrae. An unusual finding was the presence of a uniform sclerosis of a single rib at the level of the fracture site. Other bones of the rib cage, as well as the l

Bilateral femoral neck fractures and 11 rib fractures unmask tumour-induced osteomalacia caused by a phosphaturic mesenchymal tumour.

Phosphaturic mesenchymal tumours (PMTs) are rare benign tumours that secrete fibroblast growth factor 23 (FGF23), leading to phosphate wasti … or hypophosphataemia with imaging showing chronic bilateral femoral neck fractures and 11 posterior rib fractures. Copper-64 DOTA-Phe1-Tyr3-Thr8-octreotate positron emission tomography/CT demonstrated increased uptake within the left femur. Venous sampling revealed elevated FGF23. He underwent resection of the femoral tumour with histology consistent with PMT. …

Characterization and influence of ipsilateral scapula fractures among patients who undergo surgical stabilization of sub-scapular rib fractures.

BACKGROUND: Current decision algorithms involving surgical stabilization of rib fractures (SSRF) do not consider either specific fracture locations or other chest wall bony injuries. Our objective was to characterize the impact of scapula fractures on morbidity among patients who underwent fixation of sub-scapular rib fractures. We hypothesized that an ipsilateral scapula fracture was associated with poor acute and long-term outcomes. METHODS: Retrospective review of two institutions' prospectively maintained SSRF databases (October 2010 to January 2019). Patients who underwent repair of ≥ 1 …

Delayed-Onset Hemothorax Following Cough-Induced Rib Fracture.

Hemothorax, the accumulation of blood in the pleural space, is most frequently linked to chest trauma but can occasionally result from nontraumatic causes such as persistent or forceful coughing. Cough-induced rib fractures are rare, with an even less frequent association with hemothorax. We describe a case involving a 57-year-old male who presented with a worsening cough and left-sided pleuritic chest pain despite prior antibiotic and symptomatic treatment. Initial imaging revealed a minimally displaced 10th rib fracture, left-sided atelectasis, and trace pleural effusion. During his hospital…

Resorbable 3D-Printed Osteosynthetic Plates for Rib Fracture Repair.

Rib fractures are common among blunt chest trauma patients and are a hallmark of severe thoracic injury with high morbidity and mortality rates. The standard treatment of most rib fracture cases is limited to pain control and respiratory support, with the surgical stabilization of rib fractures (SSRF) using titanium plates reserved for severely injured patients. Although SSRF has been shown to improve long-term patient outcomes, its expanded use has been limited by the invasiveness of the procedure and a lack of safe and effective resorbable fixation materials. While resorbable metal and polym…

Rib fracture characteristics increasing the risk of hemothorax: a multicenter study.

The precise assessment of hemothorax risk resulting from a rib fracture is not feasible. CT images, patient characteristics, and clinical experience are utilized in daily practice to assess risk intuitively. This study aimed to identify specific markers on CT images that can predict the risk of hemothorax. The study was retrospectively conducted between May 2021 and December 2023 at three different centers. Patients diagnosed with hemothorax at the initial assessment or during follow-up were identified among those being followed for rib fractures. An investigation was carried out to examine th…

A biomechanical study of a polymer material bundled rib fracture fixator.

BACKGROUND: Rib fractures are one of the most common blunt injuries, accounting for approximately 10% of all trauma patients and 60% of thoracic injuries. Multiple rib fractures, especially flail chest, can cause local chest wall softening due to the loss of rib support, leading to paradoxical breathing, severe pain, and a high likelihood of accompanying lung contusions. OBJECTIVE: This study investigates the mechanical properties of a new polymer material rib internal fixator to provide theoretical data for its clinical use. METHODS: We conducted in vitro mechanical tests on 20 fresh caudal…

A method for identifying the learning curve for the surgical stabilization of rib fractures.

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is an accepted efficacious treatment modality for patients with severe chest wall injuries. Despite increased adoption of SSRF, surgical learning curves are unknown. We hypothesized intraoperative duration could define individual SSRF learning curves. METHODS: Consecutive SSRF operations between January 2017 and December 2021 at a single institution were reviewed. Operative time, as measured from incision until skin closure, was evaluated by cumulative sum methodology using a range of acceptable "missteps" to determine the learning cu…

Pacemaker-generated stress fracture of the second rib: a case report.

BACKGROUND: Pocket-related complications following the implantation of cardiovascular implantable electronic devices primarily include pocket hematoma, infection, skin erosion or decubitus, device migration, and Twiddler's syndrome, with other pathologies such as nerve impairment or bone lesions being extremely rarely encountered. We report a case of a 20-year old asthenic, non-athlete female patient presenting with a device-generated fracture of the second rib several months after sub-muscular permanent pacemaker implantation due to repeated bilateral pre-pectoral pocket infections. CASE PRE

Rib fracture after median sternotomy for thymoma: an unreported complication.

A 47-year-old man was admitted to the clinic with histologically diagnosed thymoma of the anterior mediastinum, pT3N0M1a, stage IYA, type B3. He underwent surgery for primary tumor resection through a median sternotomy and left thoracotomy at the 7th intercostal space to remove pleural metastases. On the first postoperative day, massive bleeding occurred, a resternotomy was carried out but failed to save the patient. A fracture of the right first rib, which injured the right vertebral artery, had caused massive bleeding and was diagnosed at autopsy. Surgeons should keep in mind this potentiall

Nontraumatic First Rib Fracture and Empyema Presenting as Shoulder Pain.

Fracture of the first rib is rare because of the anatomic protection provided by surrounding structures. Osseous fatigue caused by repetitive motion and antagonistic muscular contraction is postulated to contribute to nontraumatic first rib fracture, which is most often described in adolescent athletes in sports that involve repetitive overhead motion of the arm. In this report, we describe a 49-year-old man with an occupation involving repetitive overhead motion at the shoulder who was found to have a nontraumatic first rib fracture. The condition resolved with treatment.

[Diagnosis and treatment of rib fracture during spontaneous vaginal delivery].

INTRODUCTION: Progress of labor in multiparous women usually is not accompanied with risk of any kind of birth trauma. CASE OUTLINE: We report a very rare case of rib fracture in a neonate during vaginal delivery in the 39/40 week of gestation. The expulsion started spontaneously without any manipulation from the obstetrician. Live male newborn was delivered 4650 g. in weight, 55 cm long, with head circumference of 39 cm, Apgar score 9. The child was immediately examined by the neonatologist. Crepitations were palpable over the left hemithorax, and auscultatory on the left side inspiratory cr

Rib fracture repair: indications, technical issues, and future directions.

Rib fracture repair has been performed at selected centers around the world for more than 50 years; however, the operative indications have not been established and are considered controversial. The outcome of a strictly nonoperative approach may not be optimal. Potential indications for rib fracture repair include flail chest, painful, movable rib fractures refractory to conventional pain management, chest wall deformity/defect, rib fracture nonunion, and during thoracotomy for other traumatic indication. Rib fracture repair is technically challenging secondary to the human rib's relatively t…

Rib fracture pain and disability: can we do better?

OBJECTIVE: The purpose of this study was to determine the magnitude and duration of pain and disability in patients with rib fractures treated using current standard therapy. This was a prospective case series. METHODS: Injured patients with a chest radiographic diagnosis of one or more rib fractures between June 1, 2001, and October 31, 2001, were asked to participate. Pain levels were assessed at days 1, 5, 30, and 120 after injury using a visual pain scale (0-10). Disability at 30 days was assessed using the SF-36 Health Status Survey, and the total number of days lost from work/usual acti…

Stress fracture of the neck of the seventh and eighth ribs: a case report.

PURPOSE: This article describes a previously unreported site of stress fracture. CASE SUMMARY: A 20-year-old female sculler presented with a 10-day history of posterolateral thoracic pain and marked tenderness maximally over the seventh and eighth costovertebral joints. A radioisotope bone scan showed foci of increased uptake posteriorly in the region of the seventh and eighth ribs. Computed tomographic scan confirmed and diagnosis of stress fracture of the neck of the seventh and eighth ribs. DISCUSSION: Stress fractures of the ribs have previously been reported in rowers. All previous repo
