Action of the isolated canine diaphragm on the lower ribs at high lung volumes.

The normal diaphragm has an inspiratory action on the lower ribs, but subjects with chronic obstructive pulmonary disease commonly have an inward displacement of the lateral portions of the lower rib cage during inspiration. This paradoxical displacement, conventionally called 'Hoover's sign', has traditionally been attributed to the direct action of radially oriented diaphragmatic muscle fibres. In the present study, the inspiratory intercostal muscles in all interspaces in anaesthetized dogs were severed so that the diaphragm was the only muscle active during inspiration. The displacements o

The etiology of pectus carinatum involves overgrowth of costal cartilage and undergrowth of ribs.

PURPOSE: We compared the length of costal cartilage and rib between patients with symmetric pectus carinatum and controls without anterior chest wall protrusion, using a 3-dimensional (3D) computed tomography (CT) to evaluate whether the overgrowth of costal cartilage exists in patients with pectus carinatum. SUBJECTS AND METHODS: Twenty-six patients with symmetric pectus carinatum and matched twenty-six controls without chest wall protrusion were enrolled. We measured the full lengths of the 4th-6th ribs and costal cartilages using 3-D volume rendering CT images and the curved multiplanar re

Blunt liver injury with intact ribs under impacts on the abdomen: a biomechanical investigation.

Abdominal trauma accounts for nearly 20% of all severe traffic injuries and can often result from intentional physical violence, from which blunt liver injury is regarded as the most common result and is associated with a high mortality rate. Liver injury may be caused by a direct impact with a certain velocity and energy on the abdomen, which may result in a lacerated liver by penetration of fractured ribs. However, liver ruptures without rib cage fractures were found in autopsies in a series of cases. All the victims sustained punches on the abdomen by fist. Many studies have been dedicated

Variations of the soft tissue thicknesses external to the ribs in pectus excavatum patients.

BACKGROUND: Surgical repair of pectus excavatum (PE) has become more popular due to improvements in the minimally invasive Nuss procedure. The pre-surgical assessment of PE patients requires Computerized Tomography (CT), as the malformation characteristics vary from patient to patient. OBJECTIVE: This work aims to characterize soft tissue thickness (STT) external to the ribs among PE patients. It also presents a comparative analysis between the anterior chest wall surface before and after surgical correction. METHODS: Through surrounding tissue segmentation in CT data, STT values were calcul

Multiple severe stab wounds to chest with cuts to the ribs. Suicide or homicide?

This paper presents the case of a 36-year-old man found dead in a forest near his home with multiple stab wounds on the chest and abdomen, while the chest wounds penetrated deep into the left pleural cavity and the lung parenchyma, completely cutting through the bone sections of four ribs along the channels of the wounds. In addition, a kitchen knife dug into the ground was found in the immediate vicinity of the corpse. The deceased was wearing a T-shirt with numerous clearly visible holes in places corresponding to the wounds and quite a large tear at the bottom. The post-mortem revealed 19 d

Polyostotic fibrous dysplasia of the ribs: An unusual cause of chest pain and dyspnea.

Fibrous dysplasia is a benign bony abnormality that may be monostotic or polyostotic and is not included in the differential diagnosis of chest pain and dyspnea, since it is typically asymptomatic. A 36-year-old man presented with left-sided chest pain and dsypnea for three months. Chest X-ray and chest computed tomography scan revealed a large solid mass arising from the anterior parts of the left fourth, fifth, and sixth ribs and compressing the adjacent lung parenchyma. The tumor was completely removed surgically and histopathologic examination was consistent with fibrous dysplasia without

Stress fracture of the neck of the seventh and eighth ribs: a case report.

PURPOSE: This article describes a previously unreported site of stress fracture. CASE SUMMARY: A 20-year-old female sculler presented with a 10-day history of posterolateral thoracic pain and marked tenderness maximally over the seventh and eighth costovertebral joints. A radioisotope bone scan showed foci of increased uptake posteriorly in the region of the seventh and eighth ribs. Computed tomographic scan confirmed and diagnosis of stress fracture of the neck of the seventh and eighth ribs. DISCUSSION: Stress fractures of the ribs have previously been reported in rowers. All previous repo

[Scintigraphic diagnosis of sport injuries: multiple fractures of ribs by golf-players (author's transl)].

Three untrained golf-players suffered from pain in the dorsal region of the left thorax after some training hours. Three to four weeks after the pains had begun serial fractures of ribs were diagnosed by 99mTc-MDP bone-scan-examinations. All fractures were localized near the costal angulus (paravertebral) of these ribs. The fractures are causal by manual tensions. The difficulties in timing the bone-scan and in identifying the morphologic substrate are demonstrated. It is recommended, that golf-players suffering from thoracic pain ought to be examined by bone scan.

Asymmetric congenital deformity of the ribs. Collapse of the right side of the chest.

A congenital deformity of the chest is described in which all of the ribs and costal cartilages on the right side are deeply depressed, the apex of the depression being much farther lateral than is usual in pectus excavatum. The sternum is not depressed or scaphoid but is usually rotated to the right, sometimes markedly. The right breast is smaller than the left breast. Four of the patients were female and one was male. A single unsatisfactory experience demonstrated that rigid internal fixation is required. In the other four patients, triple section of the costochondral elements and appropria

[A case of extremely severe chest injury with fracture of several ribs (author's transl)].

A 72-years old man was severely injured when a lorry rolled back and pinned him down, causing contusion of the chest, fractures of ribs 3-10 on the right and haemothorax. Treatment of the chest injuries was by drainage and by positive end-expiratory pressure ventilation because of the development of severe pneumonia with wet lung. Persistent renal insufficiency, a gastro-intestinal haemorrhage and diabetes also required treatment. The patient developed septic endomyocarditis as a late complication, possibly attributable to the central venous catheter. All pulmonary and extrapulmonary injuries

Closed Approach Rib Revision Rhinoplasty.

AIMS AND BACKGROUND: Revision rhinoplasty remains one of the most challenging procedures due to scar tissue, distorted anatomy, and depletion of septal cartilage. Advanced structural and soft-tissue reconstruction strategies are often required in complex revision cases. This study aimed to evaluate the functional and aesthetic outcomes of extended closed rib revision rhinoplasty. ANATOMY: Patients presented with severe anatomical distortions, including overresected dorsum, short nose with contracted skin envelope, septal or retrocolumellar perforation, columellar necrosis, congenital deformit

Analgesia for rib fractures: a narrative review.

PURPOSE: Rib fracture(s) is a common and painful injury often associated with significant morbidity (e.g., respiratory complications) and high mortality rates, especially in the elderly. Risk stratification and prompt implementation of analgesic pathways using a multimodal analgesia approach comprise a primary endpoint of care to reduce morbidity and mortality associated with rib fractures. This narrative review aims to describe the most recent evidence and care pathways currently available, including risk stratification tools and pharmacologic and regional analgesic blocks frequently used as

Robotic resection of a second rib osteochondroma.

A 43-year-old man presented with a several-month history of worsening left shoulder pain. On imaging, he was found to have an osseous mass arising from his left second rib and protruding into the soft tissues of his chest. The mass had radiographic characteristics consistent with those of an osteochondroma. He had point tenderness over the mass, and the area of point tenderness was consistent with his description of the location of his pain over the past several months. Based on his symptoms, he was taken to the operating room for robotic excision of this mass. He was placed in a right lateral

Rib osteochondroma presenting as acute paraparesis.

Osteochondromas are usually osseous outgrowths arising from the metaphyseal region of cortical bone. Moreover, osteochondroma can also arise from flat bones and the spine. However, their origin in the ribs is extremely rare and always near the costochondral junction. We present a 26-year-old male who presented with chief complaints of difficulty in walking for 2 weeks subsequently diagnosed with osteochondroma based on the presence of a cartilage cap on Magnetic resonance imaging.
