Prevalence of antibodies to Legionella species in a series of patients in Israel.

A collection of serum specimens from 77 patients at various hospitals or clinics in Israel was used to determine the usefulness of the enzym … legionella antigens, the great majority (11 of 13) of L. bozemanii-positive sera reacted also with Rickettsia typhi. The data suggest that most, but not all, reactions with L. bozemanii were elicited by a cross-reacting R. typhi antigen. These results were confirmed by cross-absorption tests.

CONTRIBUTIONS TO RICKETTSIOSES RESEARCH IN COLOMBIA (1917-1943), LUIS B. PATIÑO CAMARGO.

Colombian physician Luis Benigno Patiño Camargo was one of the pioneers in the study of rickettsioses in South America, demonstrating for the first time in Colombia the presence of Rickettsia rickettsii as the etiological agent of a highly deadly exanthematic febrile syndrome in the 1930s. However, Patiño-Camargo performed other investigations from 1917-1943, which represent the first descriptions and scientific evidence of the presence of R. prowazekii and R. typhi in Colombia. Almost 60 years after the latest research conducted by Dr. Patiño-Camargo, rickettsioses were again a matter of inte

Murine typhus: an unrecognized suburban vectorborne disease.

Murine typhus, an acute febrile illness caused by Rickettsia typhi, is distributed worldwide. Mainly transmitted by the fleas of rodents, it is associated with cities and ports where urban rats (Rattus rattus and Rattus norvegicus) are abundant. In the United States, cases are concentrated in suburban areas of Texas and California. Contrary to the classic rat-flea-rat cycle, the most important reservoirs of infection in these areas are opossums and cats. The cat flea, Ctenocephalides felis, has been identified as the principal vector. In Texas, murine typhus cases occur in spring and summer, w…

[Fever of intermediate duration after return from the Canary Islands].

A 35 year old patient presented to the emergency room with high fever, headache and a maculopapular rash after returning from the Canary Islands. Elevated levels of LDH and transaminases and thrombopenia developed during the further hospital course. This presentation is common for an infection with Rickettsia typhi. Therapy with doxycycline is usually effective and should be instituted promptly. The patient's fever remitted 48 h after the first dose. Fever of intermediate duration has been described as a separate disease entity in the Mediterranean region and the Canary Islands. It is defined …

Suspected Murine Typhus Associated With Neurologic Symptoms and Posterior Reversible Encephalopathy Syndrome (PRES)-Like MRI Abnormalities: A Case Report.

Murine typhus, caused by Rickettsia typhi, is a flea-borne zoonosis that typically presents with fever, headache, myalgias, and rash. Central nervous system involvement is uncommon, and neuroimaging findings that mimic posterior reversible encephalopathy syndrome (PRES) are rarely reported. This case highlights a rare neuroinfectious presentation of murine typhus with PRES-like imaging findings in a young woman. A previously healthy woman in her 30s presented with persistent retro-orbital headaches, intermittent fevers, tremors, dysarthria, photophobia, myalgias, progressive weakness, and recu…

Development of chimeric monoclonal antibodies as standardised positive controls in rickettsial serodiagnosis.

Rickettsial diseases present ongoing diagnostic challenges due to antigenic diversity, serological cross-reactivity, and the lack of reliable standardised control reagents. Current serodiagnostic assays frequently rely on convalescent human sera as positive controls, which are inherently variable, limited in supply, and poorly suited to assay standardisation and quality assurance. In this study, we developed and validated human-mouse chimeric monoclonal antibodies targeting the conserved outer membrane protein B (OmpB) of Rickettsia typhi as defined, renewable, positive-control calibrators for…

Detection of a typhus group Rickettsia in Amblyomma ticks in the state of Nuevo Leon, Mexico.

The state of Nuevo Leon, Mexico has had outbreaks of typhus group rickettsiosis, most recently recognized in 1997. Evaluation of the sera of 345 patients with a dengue-like illness revealed that 25.5% had antibodies reactive with typhus group rickettsiae and 16% had antibodies to Rickettsia parkeri. Rickettsiae were detected by PCR and shell-vial isolations in the field-collected Amblyomma ticks. Molecular characterization by DNA sequence analysis of the gltA, ompB, and 17-kDa gene identified the organisms to be R. prowazekii.

Clinical and Epidemiological Characteristics of Scrub Typhus and Murine Typhus among Hospitalized Patients with Acute Undifferentiated Fever in Northern Vietnam.

A descriptive study on rickettsiosis was conducted at the largest referral hospital in Hanoi, Vietnam, to identify epidemiological and clinical characteristics of specific rickettsiosis. Between March 2001 and February 2003, we enrolled 579 patients with acute undifferentiated fever (AUF), excluding patients with malaria, dengue fever, and typhoid fever, and serologically tested for Orientia tsutsugamushi and Rickettsia typhi. Of the patients, 237 (40.9%) and 193 (33.3%) had scrub and murine typhus, respectively, and 149 (25.7%) had neither of them (non-scrub and murine typhus [non-ST/MT]). Th…

Proteins of typhus and spotted fever group rickettsiae.

Purified radioactive rickettsiae were obtained from irradiated and cycloheximide-inhibited L cells, and their proteins were analyzed by polyacrylamide gel electrophoresis. Rickettsial species could be distinguished by comparing the relative mobilities of constituent proteins after migration of two differentially labeled preparations in a single gel. Distinct differences were observed in gel patterns of rickettsiae from the typhus and spotted fever groups, as well as with different species within a group. Rickettsial organisms causing murine and epidemic typhus were clearly distinguished, as we

Seropositivity to tick-borne pathogens in nature management workers in the Netherlands.

The incidence of tick-borne infections other than Lyme borreliosis and tick-borne encephalitis is rising in Europe, including the Netherlands. Nature management workers, being highly exposed to ticks, serve as valuable sentinels for seroprevalence studies on tick-borne pathogens (TBPs). This study assessed nature management workers' seropositivity to TBPs including Anaplasma phagocytophilum, Babesia divergens, B. microti, Borrelia burgdorferi s.l., Rickettsia conorii and R. typhi in the Netherlands. In addition, the study examined coexposure to multiple TBPs and identified risk factors for B.

mNGS facilitates the diagnosis of pediatric murine typhus: A case report.

RATIONALE: Murine typhus, caused by Rickettsia typhi, is a globally distributed flea-borne rickettsiosis. Although rarely recognized, it can trigger hemophagocytic lymphohistiocytosis (HLH), a life-threatening hyperinflammatory syndrome. Nonspecific febrile illness and atypical petechial eruptions frequently lead to delayed or missed diagnoses. This report aims to illustrate the diagnostic process and clinical implications of murine typhus-associated HLH in a pediatric patient, and to evaluate the utility of metagenomic next-generation sequencing (mNGS) as an unbiased detection tool for occult…

Unrecognized spotted fever group rickettsiosis masquerading as dengue fever in Mexico.

Although Rocky Mountain spotted fever was documented in northern Mexico during the 1940s, spotted fever group (SFG) rickettsioses have subsequently received little attention in Mexico. In this study, sera collected in 1993 from 50 patients from the Mexican states of Yucatan and Jalisco, who were suspected clinically to have dengue fever but had no antibodies to dengue virus, were examined by indirect immunofluorescence for IgM antibodies reactive with Rickettsia rickettsii, R. akari, and R. typhi. Twenty (40%) of the patients' sera contained IgM antibodies to SFG rickettsiae at a titer of 128

Polymerase chain reaction-based diagnosis of Mediterranean spotted fever in serum and tissue samples.

A nested polymerase chain reaction (PCR) assay has been developed and used in the diagnosis of fatal and benign cases of Mediterranean spotted fever (MSF). The test was based on specific primers derived from a Rickettsia conorii 17-kD protein gene. A positive signal was obtained from spotted fever group (SFG) and typhus group (TG) rickettsiae. Discrimination between SFG and TG rickettsiae was based on a restriction fragment length polymorphism test. Other gram-negative bacterial species tested did not generate a signal, attesting for the specificity of the assay. The SFG-specific DNA fragment
