We present an exceedingly rare case of a Ewing sarcoma metastasis manifesting as a sellar mass mimicking a pituitary adenoma. The differential diagnosis of the young adult with a sellar mass is presented and correlated with a review of available literature, demonstrating this case's unique potential for clinical teaching. More specifically, this case illustrates that in a patient with a clinical history of Ewing sarcoma, a metastasis may involve the sella and suprasellar cistern without apparent osseous involvement.

BACKGROUND: This study used receiver operating characteristic curve to analyze Surveillance, Epidemiology and End RESULTS (SEER) Ewing sarcoma (ES) outcome data. The aim of this study was to identify and optimize ES-specific survival prediction models and sources of survival disparities.
MATERIALS AND METHODS: This study analyzed socio-economic, staging and treatment factors available in the SEER database for ES. 1844 patients diagnosed between 1973-2009 were used for this study. For the risk modeling, each factor was fitted by a Generalized Linear Model to predict the outcome (bone and joint

OBJECTIVE: To explore the imaging features of meningeal Ewing sarcoma/peripheral primitive neuroectodermal tumours (pPNETs).
METHODS: The imaging features and pathological characteristics of eight cases of surgically and pathologically confirmed Ewing sarcoma/pPNET were analysed retrospectively in light of recent literature on the disease.
RESULTS: The peak age was between 10 and 20 years. The lesions tended to be spindle shaped and dural based, usually widely so. CT showed that the lesions had slightly uneven high density in five cases and iso-low mixed density in three cases; marked hetero

BACKGROUND: Unlike osteosarcoma, the Ewing sarcoma family of tumors (ESFT) has rarely been reported as secondary malignant neoplasms after treatment of childhood cancer. ESFT arising as a second cancer was reviewed and characterized at our childhood cancer center.
METHODS: A retrospective review was undertaken of 11,183 patients age <21 years who were treated for a primary cancer between March 1962 and December 2003 at St. Jude Children's Research Hospital. All cases of ESFT were confirmed to have a rearranged EWS gene.
RESULTS: Six cases of ESFT (1.3% of 479 second cancers) were identified

Extraskeletal Ewing sarcoma (EES) represents a rare soft tissue malignant neoplasm histologically similar to skeletal Ewing sarcoma. It occurs mainly in adolescents and young adults and commonly affects the paravertebral regions. The differential diagnosis includes other small, blue round cells tumors. The authors report a case of an EES involving the spinal epidural and paravertebral spaces in an adolescent boy. EES diagnosis was confirmed by features of histologic analysis and immunohistochemistry and by the presence of the t(11;22)(q24;q12) chromosomal translocation by reverse transcriptase

BACKGROUND: The vertebral column is an infrequent site of primary involvement in Ewing sarcoma. Yet when Ewing sarcoma is found in the spine, the urge for decompression is high because of the often symptomatic compression of neural structures. It is unclear in alleviating a neurological deficit whether chemotherapy is preferred over decompressive laminectomy.
OBJECTIVE: To underline, in this case series, the efficiency of initial chemotherapy before upfront surgery in the setting of high-grade spinal cord or cauda equina compression of primary Ewing sarcoma.
METHODS: Fifteen patients with Ew

CBFA2T3::GLIS2-positive myeloid neoplasms occur predominantly in infants and can closely mimic pediatric small round cell sarcomas. We describe a 14-month-old boy who presented with left leg pain, an abnormal gait, and a destructive left ischial mass. Tissue biopsy was initially interpreted as an undifferentiated Ewing-like sarcoma. Bone marrow aspiration contained approximately 2% unclassified large cells, while trephine biopsy demonstrated focal tumor infiltration. Marrow tumor cells showed strong CD56 and weak CD61 expression and were positive for WT1, CD43, ERG, CD117, and BCL2. Lysozyme,

RATIONALE: Ewing-like sarcoma (ELS)/undifferentiated round cell sarcoma (URCS) is a rare type of soft tissue sarcomas (STS), especially in infants, with poor prognosis. It is a so-called "small round cell" sarcoma, and has many features of Ewing sarcoma, but lacks rearrangements in EWSR1. The diagnosis and treatment of this kind of STS remains challenging. BCOR genetic abnormalities have been found in some Ewing-like sarcomas.
PATIENT CONCERNS: This report presents an ELS case of a female infant, who was 2 months old when initially diagnosed, with the clinical stage of IIIA (G2T2N0M0). Histol

INTRODUCTION: Standard Ewing sarcoma treatment includes chemotherapy and local therapy. Previous studies found no average treatment effect of combined local therapy (radiotherapy and surgery) versus surgery alone, leaving it unclear which patients benefit from combined therapy.
METHODS: We analysed patients with localized Ewing sarcoma from the EURO-E.W.I.N.G. 99 and Ewing 2008 trials. Using causal survival forests validated through repeated k-fold cross-validation, we predicted individualized treatment effects (ITEs) for 5-year event-free survival (5y EFS) comparing combined therapy to surge

The balanced reciprocal translocation t(11;22)(q24;q12) is a specific molecular marker for the Ewing's sarcoma family of tumors (ESFT). Based on the detection of this translocation in some olfactory neuroblastomas (ONBs), it has been proposed that ONBs also belong to the ESFT. Others have challenged this hypothesis; however, the rarity of ONBs, as well as the lack of molecular techniques that work reliably on formalin fixed tissue, have precluded the molecular analysis of a significant number of cases. We evaluated the immunophenotypic and molecular features of 17 paranasal small round neuroec

Ewing sarcoma (ES) is a bone and soft tissue sarcoma that is absolutely dependent on the EWS::FLI1 transcription factor for cell survival. No compound has been shown to reverse EWS::FLI1 activity in patients, and outcomes for relapsed patients remain poor. Trabectedin above a threshold concentration reverses the activity of EWS::FLI1 and is potentiated by low-dose irinotecan in vivo. This open-label phase 1/2 trial of trabectedin with irinotecan (SARC037) enrolled 37 relapsed/refractory patients with ES. The primary objectives were to determine the safety, tolerability, recommended phase 2 dos

Ewing sarcoma is characterized by chromosomal translocations fusing the EWS gene with various members of the ETS family of transcription factors, most commonly FLI1. EWS-FLI1 is an aberrant transcription factor driving Ewing sarcoma tumorigenesis by either transcriptionally inducing or repressing specific target genes. Herein, we showed that Sprouty 1 (SPRY1), which is a physiological negative feedback inhibitor downstream of fibroblast growth factor (FGF) receptors (FGFRs) and other RAS-activating receptors, is an EWS-FLI1 repressed gene. EWS-FLI1 knockdown specifically increased the expressi

Ewing's sarcoma is associated with a fusion between the EWS and FLI1 genes, forming an EWS/FLI fusion protein. We developed a system for the identification of cooperative mutations in this tumor through expression of EWS/FLI in primary human fibroblasts. Gene expression profiling demonstrated that this system recapitulates many features of Ewing's sarcoma. EWS/FLI-expressing cells underwent growth arrest, suggesting that growth arrest-abrogating collaborative mutations may be required for tumorigenesis. Expression profiling identified transcriptional upregulation of p53, and the growth arrest

Reporter gene assays allow for examining the influence of regulatory DNA sequences on the transcription of target genes. In Ewing sarcoma, the study of these DNA sequences is paramount because the disease's main driver mutation is a fusion transcription factor that binds distinct motifs compared with its wild-type constituents. Here, we describe a workflow for analyzing the enhancer activity of regulatory DNA sequences using transfection-based dual-luciferase reporter assays in Ewing sarcoma cell lines. To this end, we provide a protocol for cloning genomic sequences into a firefly luciferase-

Ewing Sarcoma of the pelvis has poorer outcomes than other anatomical sites, with complex anatomy often precluding resection with wide margins. The role of postoperative radiotherapy (RT) in improving outcomes remains undefined. A systematic review using Medline, Embase and Cochrane databases (1972-April 2024) evaluated postoperative RT's impact on local recurrence, event-free survival and overall survival. Twenty-nine retrospective studies (21 to 296 patients) met inclusion criteria, with 28 rated good quality. Seven of 14 studies showed improved local control with postoperative RT. Ten studi

Primary intracranial Ewing sarcoma / Ewing-like sarcoma is an exceptionally rare malignant small round cell tumor. Its radiological appearance is often non-specific and may mimic other aggressive intracranial neoplasms, leading to diagnostic delay. We report the case of a young adult presenting with progressive headaches and recurrent generalized tonic-clonic seizures for six months, followed by rapid bilateral visual loss. Neurological examination revealed papilledema and decreased visual acuity, prompting brain imaging. MRI demonstrated a large aggressive frontal mass with heterogeneous enha

Primary Cutaneous Ewing Sarcoma (PCES) is exceptionally rare and represent a notorious diagnostic mimicker of Merkel cell carcinoma (MCC), particularly when it aberrantly expresses neuroendocrine markers. This pitfall can lead to significant diagnostic error and inappropriate management. A 73-year-old woman presented with a progressive, FDG-avid nodule on the left upper arm. An initial punch biopsy revealed a malignant small round cell tumor with a classic neuroendocrine immunophenotype, including dot-like positivity for CK20 and Neurofilament, which did not allow distinction between PCES and

BACKGROUND: Ewing sarcoma is a rare and highly aggressive pediatric bone cancer that is histologically composed of small, round blue cells. These histologic findings can make it difficult to assess intraoperative frozen section bone marrow margins because the bone marrow that regenerates after preoperative chemotherapy has a similar appearance, especially on frozen section analysis. Prior studies have more broadly questioned the utility of intraoperative assessment of bone marrow margins using frozen sections during the resection of bone sarcomas; however, to our knowledge, no prior study has

Ewing's sarcoma is a highly aggressive malignant small round blue cell tumour that typically affects the long bones and axial skeleton in children and adolescents. Sacral involvement is uncommon, often leading to diagnostic delays due to nonspecific symptoms. We report a case of a nine-year-old girl presenting with chronic back pain and weight loss, eventually found to have a large sacral mass with hepatic metastases. Imaging revealed a destructive presacral tumour with soft tissue extension, and histopathology confirmed Ewing's sarcoma with EWSR1 rearrangement. This case highlights the diagno
