The satiety effect of cholecystokinin. Recent progress and current problems.

Since the discovery of the satiety effect of CCK in 1973, progress has been made and problems have been encountered. The progress has included the accumulation of strong, indirect evidence that exogenous CCK acts in the abdomen to activate vagal afferent fibers and that exogenous CCK may be useful in the treatment of bulimia and obesity in humans. The most pressing problem is the current lack of evidence for the hypothesis that the satiety effect of exogenous CCK reveals a physiological function of endogenous CCK released by food entering the small intestine during a meal. Since clarification

C-terminal octapeptide of cholecystokinin decreases food intake in man.

In comparison with a saline infusion, the infusion of the C-terminal octapeptide of cholecystokinin (4 ng/kg/min) decreased food intake by an average of 122 g in a group of 12 lean men without objective evidence of untoward side effects. Shapes of the cumulative intake curves under the two conditions were similar, but subjects ate less and stopped eating sooner when receiving octapeptide than when receiving saline. These results are consistent with the hypothesis that cholecystokinin is an endogenous signal for postprandial satiety. The results offer promise for the possible use of the octapep

Early Satiety in Individuals With Cancer: A Scoping Review of an Orphan Symptom.

BACKGROUND: A common, underrecognized symptom in cancer is early satiety (ES), which is classified as an orphan symptom. No systematic or scoping review focusing on ES has been published previously. OBJECTIVE: This scoping review aimed to detail the available evidence and knowledge gaps surrounding ES in individuals with cancer. METHODS: Five databases (Cochrane, EBSCOhost, Embase, PubMed, and Web of Science) were searched from inception until August 9, 2023. Inclusion criteria were original data on ES in cancer, human studies, peer-reviewed, and full text in English. Extracted data included

Disturbance of gut satiety peptide in purging disorder.

OBJECTIVE: Little is known about biological factors that contribute to purging after normal amounts of food-the central feature of purging disorder (PD). This study comes from a series of nested studies examining ingestive behaviors in bulimic syndromes and specifically evaluated the satiety peptide YY (PYY) and the hunger peptide ghrelin in women with PD (n = 25), bulimia nervosa-purging (BNp) (n = 26), and controls (n = 26). Based on distinct subjective responses to a fixed meal in PD (Keel, Wolfe, Liddle, DeYoung, & Jimerson, ), we tested whether postprandial PYY response was significantly

The clinical uses of leptin.

Leptin is the first of a group of adipocyte-secreted hormones to be used clinically to treat hypoleptinemic states. In children with congenital leptin deficiency and extreme obesity, leptin induces satiety and a dramatic loss of weight. In hypoleptinemic patients with extreme insulin resistance and lipodystrophy, leptin ameliorates insulin resistance, hyperglycemia, hyperinsulinemia, dyslipidemia and hepatic steatosis. In both these leptin-deficient states, leptin therapy restores gonadotrophin secretion, as well as luteinizing hormone and thyroid-stimulating hormone pulsitility.

Sucking behaviour and milk intake of neonates in relation to milk fat content.

The hypothesis that a high concentration of fat in milk acts as a satiety signal for babies was tested by feeding neonates formula milks of different fat concentrations. Babies were tested on 2 consecutive days. On day 1 they were fed a high fat milk followed by a low fat milk, or vice versa, each for 2 min; and on day 2 the same procedure was followed but in the opposite order. Milk intake and 6 parameters of sucking behaviour were recorded. There was no indication that high fat milk acted as a cue to babies to slow or stop feeding. On the contrary babies appeared to feed more actively on the

How dietary amino acids and high protein diets influence insulin secretion.

Glucose homeostasis is the maintenance and regulation of blood glucose concentration within a tight physiological range, essential for the functioning of most tissues and organs. This is primarily achieved by pancreatic secretion of insulin and glucagon. Deficient pancreatic endocrine function, coupled with or without peripheral insulin resistance leads to prolonged hyperglycemia with chronic impairment of glucose homeostasis, most commonly seen in diabetes mellitus. High protein diets (HPDs) are thought to modulate glucose homeostasis through various metabolic pathways. Insulin secretion can

Vagally Mediated Gut-Brain Relationships in Appetite Control-Insights from Porcine Studies.

Signals arising from the upper part of the gut are essential for the regulation of food intake, particularly satiation. This information is supplied to the brain partly by vagal nervous afferents. The porcine model, because of its sizeable gyrencephalic brain, omnivorous regimen, and comparative anatomy of the proximal part of the gut to that of humans, has provided several important insights relating to the relevance of vagally mediated gut-brain relationships to the regulation of food intake. Furthermore, its large size combined with the capacity to become obese while overeating a western di…

Recent Advances in the Definition and Management of Functional Dyspepsia.

In 2016, the Rome criteria were updated as Rome IV, and only minor changes were introduced for functional dyspepsia (FD). The major symptoms of FD now include not only postprandial fullness, but also epigastric pain and burning, and early satiation at above the "bothersome" level. Investigations into the effect of meal ingestion on symptom generation have indicated that not only postprandial fullness and early satiety but also epigastric pain and burning sensation and nausea (not vomiting) may increase after meals. Helicobacter pylori infection is considered to be the cause of dyspepsia if suc…

A Small Potassium Current in AgRP/NPY Neurons Regulates Feeding Behavior and Energy Metabolism.

Neurons that co-express agouti-related peptide (AgRP) and neuropeptide Y (NPY) are indispensable for normal feeding behavior. Firing activities of AgRP/NPY neurons are dynamically regulated by energy status and coordinate appropriate feeding behavior to meet nutritional demands. However, intrinsic mechanisms that regulate AgRP/NPY neural activities during the fed-to-fasted transition are not fully understood. We found that AgRP/NPY neurons in satiated mice express high levels of the small-conductance calcium-activated potassium channel 3 (SK3) and are inhibited by SK3-mediated potassium curren

Postingestive inhibition of food intake by aspartame: importance of interval between aspartame administration and subsequent eating.

Aspartame administered in capsules (i.e., without tasting) 1 h before a meal significantly reduces the amount eaten in that meal. In the present study 36 young men and women were divided into 3 groups of 12 to receive aspartame (400 mg) or placebo (400 mg starch) on separate occasions either 5 min (Group A), 30 min (Group B) or 60 min (Group C) before beginning an ad lib test meal. Compared with placebo, aspartame reduced food intake in Group C (by 18.5%, p < 0.01), but did not reliably affect intake in Groups A or B. There were, in contrast, no significant effects of aspartame on premeal rati

Peripheral factors in the mediation of cholecystokinin-induced satiety as assessed by comparative potencies of cholecystokinin antagonists.

Cholecystokinin COOH-terminal octapeptide (CCK-8) produces a satiating effect in the rat and other animals upon peripheral administration. Although it has been demonstrated that the receptors which mediate this action are located in the periphery and are of the CCK-A subtype, their anatomical location has not been firmly established. A dense population of CCK receptors in the pyloric sphincter has been suggested as a candidate. We here quantify the potency of several CCK antagonists to inhibit the contractile effect of CCK-8 on the rat pyloric sphincter in vitro. The potent and selective antag

Cagrilintide: A Long-Acting Amylin Analog for the Treatment of Obesity.

Despite the worldwide epidemic of obesity, there remain few approved pharmacological treatment options to bridge the gap between lifestyle therapy and bariatric surgery. Cagrilintide is an amylin-analog, now being developed in combination with the GLP-1 agonist semaglutide to achieve sustained weight loss in persons with overweight and obesity. Amylin, released with insulin from beta cells in the pancreas, induces its satiating effect via both the homoeostatic and hedonic regions of the brain. Semaglutide, a GLP-1 receptor agonist, reduces appetite via GLP-1 receptors in the hypothalamus and i

Alteration of regional heterogeneity and functional connectivity for obese undergraduates: evidence from resting-state fMRI.

Obesity was found to be related with the changes of brain functions in human beings. There were several brain areas that were verified to be correlated with the obesity, including the parietal cortex, frontal cortex and so on. However, the cortical regions found from different studies were discrepant due to the different ages, gender distribution and satiation degree of participants. We found that the regional homogeneity of right angular gyrus were smaller in obese undergraduates than that in normal-weight undergraduates. Moreover, functional connectivity of the left middle temporal cortex an…

Behavioral and pharmacological specificity of the feeding elicited by cholinergic stimulation of the substantia nigra in the rat.

Microinjections of the cholinergic agonist carbachol into anterior substantia nigra dose dependently increased food intake in satiated rats. This resulted from a prolongation of the duration of eating. In the absence of food, those doses of carbachol that stimulated food intake (.1 and .5 microgram) had no effect on any other response examined, including gnawing, drinking, locomotion, grooming, sniffing, and rearing. The effect of carbachol depended on the degree of prior food deprivation, but supra-additive effects of carbachol and deprivation were not observed. These results are contrasted w

Management of Gastroparesis.

Gastroparesis is a gastrointestinal motility disorder characterized by nausea, vomiting, early satiation, postprandial fullness, bloating, and upper abdominal pain. The diagnosis requires documented delay in gastric emptying with an optimal test such as scintigraphy or stable isotope gastric emptying breath test in the absence of mechanical obstruction. The pathophysiologic mechanisms of gastroparesis are multifactorial, including antroduodenal hypomotility, pylorospasm, impaired gastric accommodation, and visceral hypersensitivity. The etiologies of gastroparesis are broad, but the most commo…
