Electrocochleography summating potential seen on auditory brainstem response in a case of superior semicircular canal dehiscence.

BACKGROUND: Superior canal dehiscence syndrome (SCDS) is a condition in which an abnormal communication between the superior semicircular canal and the middle cranial fossa causes patients to hear internal noises transmitted loudly to their affected ear as well as to experience vertigo with pressure changes or loud sounds. Patients with SCDS can have an elevated ratio of summating potential (SP) to action potential (AP) as measured by electrocochleography (ECochG). Changes in this ratio have been observed during surgical intervention to correct this abnormal communication. CASE DESCRIPTION: W

Reduction in posterior semicircular canal gain by age in video head impulse testing. Observational study.

INTRODUCTION: and objectives: In-depth assessment of the vestibulo-ocular reflex with the new video systems available provides information on various previously-unidentified characteristics in patients with peripheral vestibular disorder. The aim of this work is to quantify how often pathological gain (uni- or bilateral) in the posterior semicircular canal (PSC) gain appears in video head-impulse testing (vHIT) with preservation of the gain in the rest of the semicircular canals, and to analyse the relation of this frequency with patient age and/or diagnosis. METHODS: This was a prospective o…

Otic capsule dehiscence syndrome: Superior semicircular canal dehiscence syndrome with no radiographically visible dehiscence.

We conducted a prospective longitudinal study of two cohorts of patients who had superior semicircular canal dehiscence syndrome (SSCDS); one group had radiographically confirmed superior canal dehiscence (SCD), and the other exhibited no identified otic capsule dehiscence on imaging (no-iOCD). We compiled data obtained from prospective structured symptomatology interviews; diagnostic studies; three-dimensional, high-resolution, temporal bone computed tomography; and a retrospective case review from our tertiary care referral center. Eleven adults and 1 child with SSCDS were identified, surgic

Surgical treatment of hearing loss when otosclerosis coexists with superior semicircular canal dehiscence syndrome.

OBJECTIVE: Document a case of bilateral otosclerosis with coexisting bilateral superior semicircular canal dehiscence syndrome and the treatment of hearing loss in this setting. PATIENT: A 33-year-old woman presented with bilateral mixed hearing loss; worse in the left ear. This was gradual in onset, and she denied dizziness. Computerized tomographic scan revealed fenestral otosclerosis and a large dehiscence of the superior semicircular canal bilaterally. She declined amplification. INTERVENTION: Sequential laser-assisted stapedotomy with insertion of a Kurz titanium CliP Piston prosthesis.

cAMP-stimulated Cl- secretion is increased by glucocorticoids and inhibited by bumetanide in semicircular canal duct epithelium.

BACKGROUND: The vestibular system controls the ion composition of its luminal fluid through several epithelial cell transport mechanisms under hormonal regulation. The semicircular canal duct (SCCD) epithelium has been shown to secrete Cl- under β2-adrenergic stimulation. In the current study, we sought to determine the ion transporters involved in Cl- secretion and whether secretion is regulated by PKA and glucocorticoids. RESULTS: Short circuit current (Isc) from rat SCCD epithelia demonstrated stimulation by forskolin (EC50: 0.8 μM), 8-Br-cAMP (EC50: 180 μM), 8-pCPT-cAMP (100 μM), IBMX (25

Genetic modification of the inner ear lateral semicircular canal phenotype of the Bmp4 haplo-insufficient mouse.

In the mouse, development of the lateral semicircular canal of the inner ear is sensitive to Bmp4 heterozygosity. In the C57BL6 background 30% of the heterozygotes display circling behavior, 66% have a specific defect in the vestibular part of the inner ear, namely the constriction, interruption or absence of the lateral semicircular canal. Only mice having both ears affected display circling behavior. In the (C57BL6xCBA)N1 background, the penetrance of the canal phenotype is greatly reduced, and bilateral lateral canal defect is not sufficient to induce circling. We found association of the c

Diagnosis and outcomes of middle cranial fossa repair for patients with superior semicircular canal dehiscence syndrome.

The aim of this study was to retrospectively review the clinical presentation, diagnostic features, in particular cervical vestibular evoked myogenic potentials (cVEMPs), and the outcomes of surgical repair for superior semicircular canal dehiscence syndrome (SSCDS). SSCDS is a well-described syndrome of auditory and vestibular symptoms due to a bony dehiscence of the superior semicircular canal in the middle cranial fossa. A series of six procedures on five patients with SSCDS who underwent surgical repair via a middle fossa craniotomy were retrospectively reviewed. Preoperative and postopera

Superior semicircular canal dehiscence: prevalence in a population with clinical suspected otosclerosis-type hearing loss.

Superior semicircular canal dehiscence (SSCD) can present with a variety of symptoms that can be predominantly auditory, predominantly vestibular or both. It can mimic a wide range of otological disorders, in particular otosclerosis-like stapes fixation. Our study revealed that, in 5.3% of our patients with clinically suspected otosclerosis, SSCD was detected in high-resolution multi-detector computed tomography (HRMDCT) of the temporal bone. We therefore emphasise the value of HRMDCT with reconstructions in the plane of the superior semicircular canal and perpendicular to the superior semicir

Glucocorticoids stimulate cation absorption by semicircular canal duct epithelium via epithelial sodium channel.

The semicircular canal duct (SCCD) epithelium is a vestibular epithelial domain that was recently shown to actively contribute to endolymph homeostasis by Cl(-) secretion under control of beta(2)-adrenergic stimulation. By analogy to other Cl(-) secretory epithelia, we hypothesized that SCCD also provides an active absorptive pathway for Na(+) under corticosteroid control. Measurements of short-circuit current (I(sc)) demonstrated stimulation (7-24 h) by the glucocorticoids hydrocortisone (EC(50) 13 nM), corticosterone (33 nM), prednisolone (70 nM), and dexamethasone (13 nM) over physiological

Molecular genetic advances in semicircular canal abnormalities and sensorineural hearing loss: a report of 16 cases.

OBJECTIVES: The study goals were (1) to determine if the degree and pattern of semicircular canal dysmorphology and the presence or absence of a cochlea in patients with congenital sensorineural hearing loss predict audiologic outcome, severity, or the frequencies involved and (2) to review the recent advances in molecular genetics of the semicircular canals and correlate this information with audiologic and anatomic patterns seen in our series of patients. DESIGN AND SETTING: We conducted a retrospective study at a tertiary care center with a large otologic and cochlear implant service. PAT…

Chloride secretion by semicircular canal duct epithelium is stimulated via beta 2-adrenergic receptors.

The ductal epithelium of the semicircular canal forms much of the boundary between the K+-rich luminal fluid and the Na+-rich abluminal fluid. We sought to determine whether the net ion flux producing the apical-to-basal short-circuit current (I(sc)) in primary cultures was due to anion secretion and/or cation absorption and under control of receptor agonists. Net fluxes of 22Na, 86Rb, and 36Cl demonstrated a basal-to-apical Cl- secretion that was stimulated by isoproterenol. Isoproterenol and norepinephrine increased I(sc) with an EC50 of 3 and 15 nM, respectively, and isoproterenol increased

["Inner perilymph fistula" of the anterior semicircular canal. A new disease picture with recurrent attacks of vertigo].

In 1998 Minor et al. described a new variant of perilymphatic fistula: the "superior canal dehiscence syndrome". This syndrome is clinically characterized by recurrent attacks of vertigo and oscillopsia induced by loud noises or stimuli that result in changes in intracranial or middle ear pressure. It is caused by a dehiscence of bone overlying the superior (anterior) semicircular canal. Due to this dehiscence, a third, mobile window (in addition to the round and oval windows) is formed, and changes in pressure are pathologically transduced to the anterior semicircular canal. Although the supe

Long-term results of posterior semicircular canal occlusion for intractable benign paroxysmal positional vertigo.

Benign paroxysmal positional vertigo (BPPV) is a common condition which is usually managed conservatively, surgical intervention being recommended only for those small number of patients in whom it becomes persistent and incapacitating. The results of surgery in 13 patients who underwent posterior semicircular canal occlusion for intractable (> 12 months duration) and incapacitating BPPV are presented with special emphasis on their long-term follow-up. The mean follow-up was 66 months (range, 29-119 months). All patients reported complete and immediate resolution of their positional vertigo, w

Possible Joint Involvement of the Cochlea and Semicircular Canals in the Perception of Low-Frequency Tinnitus, Also Called "The Hum" or "Taos Hum".

INTRODUCTION: "The hum" or "Taos hum" are common terms of a tinnitus with three remarkable features. OBJECTIVE: A possible common background is reevaluated for the three features of the hum: being sensitive on sound, head rotation, and long-distance air travel. MATERIALS AND METHODS: Questionnaires that were originally developed by Frosch were recalculated concerning the question of hum interactions with sounds. The question was split into the two affirmative answers a) "masking", which corresponds to the overrule of the hum by a sound, and b) "beats", which correspond to a nonlinear interac

Bilateral Superior Semicircular Canal Dehiscence Presenting as Progressive Gait Instability in a Patient With Parkinsonism and Autoimmune Features.

Superior semicircular canal dehiscence (SSCD) is a vestibular disorder caused by a defect in the bony roof of the superior semicircular canal, resulting in a third mobile window in the inner ear. This structural abnormality alters vestibular and auditory processing, producing symptoms such as vertigo, imbalance, sound sensitivity, autophony, and oscillopsia. We describe a 65-year-old man with a longstanding diagnosis of atypical Parkinsonism who presented with progressive gait apraxia, disequilibrium, and cognitive slowing. Despite undergoing bilateral deep brain stimulation (DBS) for tremor,

Is CT Still the Gold Standard in Semicircular Canal Dehiscence? Diagnostic Value of MRI in Poschl and Stenver Planes.

BACKGROUND/OBJECTIVES: The primary aim of this study was to investigate whether magnetic resonance imaging (MRI) of the superior and posterior semicircular canals (SCs) in cases with and without dehiscence gives results similar to those of CT. As a novel contribution, the secondary aim was to assess the diagnostic correlation between CT and MRI sequences obtained primarily in Poschl and Stenver planes, instead of reformatted images, for detecting superior and posterior semicircular canal dehiscence. METHODS: A total of 103 patients were retrospectively evaluated based on CT scans, and 27 of t…

Assessment of Semicircular Canal Functioning Using a Video Head Impulse Test in Individuals with Occupational Noise Exposure.

Background: Exposure to noise or loud sound damages the cochlea and can result in hearing loss, which has been well-documented. Research over the years has shown that individuals exposed to noise for a short or extended period may develop vestibular problems. The otolith organs are found to be more susceptible to noise exposure or acoustic trauma than the semicircular canals (SCC). Nonetheless, little is known about the functioning of all three SCC in individuals exposed to noise. A technologically advanced, noninvasive, simple, and fast tool, the video head impulse test (vHIT) helps in evalua…
