Brain-derived neurotrophic factor - a key player in the gastrointestinal system.

Brain-derived neurotrophic factor (BDNF) is highly expressed throughout the gastrointestinal (GI) tract and plays a critical role in the regulation of intestinal motility, secretion, sensation, immunity, and mucosal integrity. Dysregulation of BDNF signalling has been implicated in the pathophysiology of various GI disorders including inflammatory bowel disease, irritable bowel syndrome, functional dyspepsia, and diabetic gastroenteropathy. This review provides a comprehensive overview of BDNF localization, synthesis, receptors, and signalling mechanisms in the gut. In addition, current eviden

Adipsic diabetes insipidus in adult patients.

INTRODUCTION: Adipsic diabetes insipidus (ADI) is a very rare disorder, characterized by hypotonic polyuria due to arginine vasopressin (AVP) deficiency and failure to generate the sensation of thirst in response to hypernatraemia. As the sensation of thirst is the key homeostatic mechanism that prevents hypernatraemic dehydration in patients with untreated diabetes insipidus (DI), adipsia leads to failure to respond to aquaresis with appropriate fluid intake. This predisposes to the development of significant hypernatraemia, which is the typical biochemical manifestation of adipsic DI. METHO

'The body says it': the difficulty of measuring and communicating sensations of breathlessness.

Breathlessness is a sensation affecting those living with chronic respiratory disease, obesity, heart disease and anxiety disorders. The Multidimensional Dyspnoea Profile is a respiratory questionnaire which attempts to measure the incommunicable different sensory qualities (and emotional responses) of breathlessness. Drawing on sensorial anthropology we take as our object of study the process of turning sensations into symptoms. We consider how shared cultural templates of 'what counts as a symptom' evolve, mediate and feed into the process of bodily sensations becoming a symptom. Our contrib

The Insular Cortex Dynamically Maps Changes in Cardiorespiratory Interoception.

Palpitations and dyspnea are fundamental to the human experience of panic anxiety, but it remains unclear how the brain dynamically represents changes in these interoceptive sensations. We used isoproterenol, a rapidly acting peripheral beta-adrenergic agonist similar to adrenaline, to induce sensations of palpitation and dyspnea in healthy individuals (n=23) during arterial spin labeling functional magnetic resonance imaging (fMRI). We hypothesized that the right mid-insular cortex, a central recipient of viscerosensory input, would preferentially respond during the peak period of cardiorespi

Clinico-Cytological Study of Fungal Elements in the Buccal Smears of Prediabetic, Diabetic, and Nondiabetic Individuals in Association with Burning Sensation.

INTRODUCTION: Diabetes mellitus is a metabolic disorder that may have profound effects on oral mucosa being prone to infections, particularly candidiasis. In healthy individuals, the candida is a commensal, but in diabetic patients, it forms severe colonization, even in the absence of any clinically evident lesion and may cause a burning sensation of the mouth. This study aims to identify the presence or absence of fungal elements in buccal smears of nondiabetic, prediabetic, and diabetic individuals and to correlate the presence or absence of burning sensation in the oral cavity. Sensation-seeking: Dopaminergic modulation and risk for psychopathology.

Sensation-seeking (SS) is a personality trait that refers to individual differences in motivation for intense and unusual sensory experiences. It describes a facet of human behaviour that has direct relevance for several psychopathologies associated with high social cost. Here, we first review ways of measuring SS behaviour in both humans and animals. We then present convergent evidence that implicates dopaminergic neurotransmission (particularly via D2-type receptors) in individual differences in SS trait. Both high tonic dopamine levels and hyper-reactive midbrain dopaminergic responses to s

Neurology, child.

Child Neurology concerns the study of the human nervous system during its development, as it is affected by conductivity, embryology and neuro-immunological factors in developmental brain disorders, coagulation disorders and the metabolism of nervous tissue. During a neurological examination the differential diagnosis depends on the localization of the symptoms within vision, strength, coordination, reflexes and sensation. Neurological disorders of the central nervous system may represent the cause of some mental illnesses, making hard to discriminate between the field of application proper of

[Globus sensation : A clinical review].

Globus sensation is a medially felt lump in the throat. It can be associated with a number of different diseases, but this symptom may also occur monosymptomatically as a somatoform disorder or as a secondary somatoform illness behavior. Globus sensation may be persistent or intermittent. Diagnosis requires close interdisciplinary cooperation, since the globus may be a symptom of anxiety, depression or personality disorders. Firstly, patients should undergo a thorough otolaryngological examination (including careful medical history taking), possibly in cooperation with other medical specialist

Clinical, psychopathological and personality correlates of interoceptive awareness in anorexia nervosa, bulimia nervosa and obesity.

OBJECTIVE: To determine the levels of interoceptive awareness (IA), which measures the ability of an individual to discriminate between sensations and feelings, and between the sensations of hunger and satiety, in eating disorder patients and to identify the clinical, psychopathological and personal variables correlated with IA. SAMPLING AND METHODS: Sixty-one restrictor anorectics, 61 binge-purging anorectics, 104 purging bulimics, 49 obese subjects with binge eating disorder (BED) and 47 obese subjects without BED were compared. They were assessed with the Eating Disorder Inventory-2, the T

Broadening the conceptualization of panic disorder to include the fear presentation of avoidant/restrictive food intake disorder: The legacy of panic control therapy.

The presentation of avoidant/restrictive food intake disorder (ARFID) characterized by fear of aversive consequences of eating (fear-ARFID) has both phenomenological and mechanistic similarities to panic disorder. In this narrative review, we propose a shared model of the pathogenesis of the two disorders, centered on interoceptive sensitivity as the key maintenance mechanism. We review the evidence that fear-ARFID, which involves restrictive eating motivated by a desire to avoid aversive events (e.g., choking, vomiting, abdominal pain) related to the gastrointestinal tract, onsets with an une

[Long-term results and complications of uvulopalatopharyngoplasty in snoring without sleep apnea syndrome].

Results and complications were analyzed in 201 patients operated upon by uvulopalatopharyngoplasty for benign or moderately severe rhonchopathy, after elimination of the sleep apnea syndrome as confirmed after follow up for between 15 and 30 months, and showed total recovery from snoring in 60% of cases, an improvement in 26.5% and lack of efficacy in 13.5%. Social and familial relations were improved concomitantly as well as the functional and general disorders usually attributed to snoring (asthenia, somnolence, sleep hypopnea). Various usually minor functional disorders remain, including th

[Gustatory disorders in COVID-19].

Taste disturbances are present in 20.3-88.0% of COVID-19 cases and are the first signs of infection in 11.0-18.1% of cases. They often manifested 3-7 days after the onset of general respiratory symptoms and last 2 to 16 days, followed by recovery. There are also prolonged disturbances of taste sensation (up to 61-76 days or more), which is associated with damage to various types of receptor cells of the mucous membrane of the tongue. More severe taste disturbances are recorded in the elderly. In women, changes in taste sensation are noted more often than in men, and with a longer recovery peri

Nutritional and physiological significance of luminal glutamate-sensing in the gastrointestinal functions.

Recent evidence indicates that free amino acids are nutrients as well as acting as chemical transmitters within the gastrointestinal tract. Gut glutamate research is the most advanced among 20 amino acids. Free glutamate carries the umami taste sensation on the tongue and a visceral sensation in the gut, especially the stomach. In the field of taste physiology, the physiological meaning of the glutamate-derived chemical sense, the umami taste, has been proposed to be a marker of protein intake. Experimental evidence in gut glutamate physiology strongly supports this hypothesis. Free glutamate

Return of human corneal sensitivity after penetrating keratoplasty.

To evaluate the reinnervation of corneal grafts, we studied 91 eyes of 79 patients for the return of sensitivity one month to ten years following penetrating keratoplasty for various corneal disorders including keratoconus, Fuchs' dystrophy, herpes simplex keratitis, aphakic and pseudophakic bullous keratopathy, glaucoma, trauma, and interstitial keratitis. Using the Cochet-Bonnet esthesiometer, a sensation was recorded in the center of the graft no earlier than 18 months. We found a progressive return of sensitivity from the periphery toward the center of the graft. Regression analysis indica

[Alien hand sign observed at the initial stage of a case of Creutzfeldt-Jakob disease].

A 68-year-old man was admitted to our hospital in the mid-October of 2006 because of a one-month history of peculiar movements of the left hand, which had been preceded by one month by awkward motions in the left leg. Upon neurological examination, spontaneous involuntary movement of the left hand was found. His left hand with his index finger stretched moved toward his right side spontaneously. He could not control his left leg freely. Although he showed mild ataxia in his left hand, there was no weakness, no dystonia, and no apraxia. No sensory abnormality was detected except for mild deep-s

Tactile sensitivity in Asperger syndrome.

People with autism and Asperger syndrome are anecdotally said to be hypersensitive to touch. In two experiments, we measured tactile thresholds and suprathreshold tactile sensitivity in a group of adults with Asperger syndrome. In the first experiment, tactile perceptual thresholds were measured. Two frequencies of vibrotactile stimulation were used: 30 and 200 Hz. The results demonstrated significantly lower tactile perceptual thresholds in the Asperger group at 200 Hz but not at 30 Hz, thus confirming tactile hypersensitivity but only for one class of stimulus. A second experiment investigat

Trigeminal nerve injury and repair.

Injuries to the inferior alveolar nerve (IAN) and the lingual nerve are relatively uncommon. However, there are certain routine surgical and non-surgical procedures that have the potential, albeit small, for injuring these neural structures. A review of the diagnosis, classification and management of these injuries is presented.
