Effective implementation of work-hour limits and systemic improvements.

BACKGROUND: Sleep deprivation, ubiquitous among nurses and physicians, recently has been shown to greatly increase rates of serious medical errors and occupational injuries among health care workers in the United States. CURRENT INITIATIVES AND POLICIES: The Accreditation Council for Graduate Medical Education's current work-hour limits for physicians-in-training allow work hours well in excess of those proven safe. No regulations limit the work hours of other groups of health care providers in the United States. Consequently, nursing work shifts exceeding 12 hours remain common. Physician-in

Transient Psychotic Symptoms Induced by Acute Sleep Deprivation in a Factory Worker: A Case Report.

Shift work sleep disorder and acute sleep deprivation are increasingly recognized occupational hazards, particularly among factory workers on rotating schedules, with potential for severe neuropsychiatric manifestations, including transient psychosis. We report the case of a 37-year-old male factory worker with no psychiatric history who developed vivid auditory hallucinations and intense anxiety following consecutive night shifts and daytime overtime, resulting in profound sleep restriction. Presentation to occupational health revealed an exhausted, oriented patient with mild psychomotor agit

Work or Wellness? Examining the Challenges of Low-Wage Workers with Type 2 Diabetes.

Type 2 diabetes disproportionately affects low-income and racially marginalized communities. Several social and economic factors intersect to create and reproduce this unequal burden. This qualitative study explores how low-wage workers experience and navigate diabetes management in the workplace. Our findings highlight how unpredictable work schedules, lack of access to sick leave, and inflexible work environments with limited worker autonomy create significant barriers to diabetes self-management. These challenges are compounded by limited control over work conditions and societal norms that

Physician-level variation exceeds time-of-day variation in diagnostic resource utilization in low-acuity emergency department care.

BACKGROUND: Emergency physicians working prolonged shifts may show temporal variation in clinical practice. We examined whether diagnostic resource utilization differed across scheduled shift periods in a structured low-acuity emergency department setting. METHODS: This retrospective observational study included 11 032 visits for upper respiratory tract infection, myalgia, abdominal pain, or headache across 12 nonconsecutive 24-hour shifts (5 December 2025-19 January 2026) in a high-volume tertiary emergency department in Turkey. Visits were classified as early (0-5 h; 08:30-13:30), mid (5-10

Sleep patterns of shiftworkers.

Work hours affect sleep because the sleep behavior of people on nonworkdays differs from that on workdays. For most workers, workday sleep is shorter than nonworkday sleep. On nonworkdays most workers sleep at night. Work schedule interacts with the effects of work hours on workday sleep. On workdays, people on the nightshift sleep the least, people on the afternoon/evening shift sleep the most, and people on the dayshift sleep less than those on the afternoon/evening shift but more than those on the nightshift. Thus, the infradian sleep strategy elected by a shiftworker is ordinarily related

Sleep Disruption and Shift Work Associate with Increased Risk of Reproductive Endocrine Disorders: Evidence from UK Biobank and Mendelian Randomization.

BACKGROUND: Disrupted sleep and shift work are linked to multiple adverse health outcomes, but their association with reproductive endocrine … s and shift work in RED risk assessment, suggesting that interventions targeting sleep duration and shift work schedules may help reduce RED incidence and improve reproductive health. This study is the first to integrate both prospective and genetic evidence to identify causal pathways.

The association of night shift work with the risk of acute ischemic cerebrovascular syndrome: a longitudinal study of 204,261 UK Biobank participants.

BACKGROUND: Shift work is highly prevalent globally, yet evidence linking it to ischemic cerebrovascular disease remains inconsistent. Existing studies are limited by unsystematic quantification of night shift exposure and outcome definitions failing to capture the clinical continuum of ischemic events. We performed a large prospective cohort study using refined shift pattern classification and acute ischemic cerebrovascular syndrome (AICS) as the primary end point to enable a robust evaluation of the association between shift patterns and ischemic cerebrovascular disease. METHODS: 204,261 pa

Additive interactions of sleep disorders, insufficient sleep, and shift work on road and workplace safety incidents: A sufficient cause approach with overlap weights.

BACKGROUND: Behavioural and biological factors impacting healthy sleep can influence road and workplace safety. This study investigated the additive interactions of sleep-disrupting factors (sleep disorders, insufficient sleep, and shift work) on road and workplace safety in young adults (22 years) from the Raine Study. METHODS: Employed participants (n = 439, median age 22.0 years, IQR 21.6-22.3) were assessed for common sleep disorders (obstructive sleep apnoea, insomnia, restless legs syndrome). They provided information on shift work status, habitual sleep duration (insufficient sleep <7

Impact of social isolation on circadian rhythmicity and clock genes expression in the prefrontal cortex and hippocampus of mice exposed to simulated rotating shiftwork/jetlag.

Circadian rhythms regulate fundamental physiological and behavioral functions, yet are increasingly disrupted by contemporary environmental stressors such as rotating shift work and social isolation. Although each disruptor independently compromises circadian homeostasis, their interactive effects remain insufficiently characterized. Given the role of social cues as non-photic zeitgebers, we investigated whether social isolation exacerbates behavioral and molecular circadian disruption induced by repeated light-dark phase shifts, and whether social interaction confers resilience. Male AKR mice

High body mass index and night shift work are associated with COVID-19 in health care workers.

OBJECTIVE: To assess the magnitude of COVID-19 spread and the associated risk factors among health care workers (HCWs), we conducted an in-hospital survey in a central Italian COVID Hospital. METHODS: Participants underwent nasopharyngeal swab and/or serum collection for SARS-CoV-2 IgG examination. We divided participants according to working status, into rotating-night shift workers (r-NSW) and day-workers. RESULTS: We found 30 cases of COVID-19 infection in a total of 1180 HCWs (2.5%). Most COVID-19-positive hospital employees were r-NSWs with significantly higher BMI than that of individu

Sex and Gender Role Differences in Occupational Exposures and Work Outcomes Among Registered Nurses in Correctional Settings.

BACKGROUND AND CONTEXT: The correctional environment exposes registered nurses to unique occupational health hazards including, but not limited to, an increased risk for workplace violence. Gender role expectations regarding femininity and masculinity may influence occupational exposures and outcomes differently. Risk comparisons between male and female registered nurses working in correctional settings, have been minimally examined. With the proportion of male registered nurses working in corrections higher than that of nurses working in other healthcare sectors, and with the increasing numbe

On call: alert or unsafe? A report of the AORN on-call electronic task force.

Research has linked health care providers' long work hours with patient care errors. Perioperative staff members often work extended hours because of long shifts and being on call. AORN's on-call electronic task force conducted a survey of nurses to determine their work load, call scheduling, and experiences related to fatigue. Of the 1,013 respondents, 67.7% said they had experienced effects of sleep deprivation. A follow-up survey revealed that few call programs are in compliance with AORN's recommended practices. Compliant call programs are detailed for adaptation in other perioperative set

Circadian disruption and circulating TGF-β1 in workers: evidence from Mendelian randomization and observational data.

BACKGROUND: Circadian disruption is increasingly recognized as an important biological stressor underlying chronic disease, yet its direct immunological effects remain unclear. Observational studies are limited by residual confounding and reverse causation. METHODS: We integrated two-sample Mendelian randomization (MR) with an exploratory observational biomarker study. Genetic instruments for genetically proxied shift-work propensity were derived from the UK Biobank GWAS, and outcome data for 46 circulating proteins were obtained from the AGES-Reykjavik Study in Iceland. Vitamin D supply in shift working nurses.

We studied determinants of Vitamin D in serum of 67 female health care workers (aged 25-60 years), including age, body mass index, physical activity, and shift work. Overall, vitamin D levels were low, ranging from 6 to 51 ng/mL (median: 20 ng/mL). Lower serum levels were found in samples drawn in winter and spring and in obese subjects. Shift work had only small effects on vitamin D levels. The high prevalence of vitamin D undersupply is in line with observations from the German general population. Vitamin D supply particularly in winter and spring should be ensured to avoid health problems.

[Emergency physicians and "burn out" syndrome].

Emergency physicians are continuously under stress because of overcrowded departments, severity of cases, and their work schedules. In addition to this work stress, unregular social and family life are the main components of ongoing burn out process in these professionals. Burn out is the end point of the process which is complicated by the loss of professional enthusiasm and satisfaction, and negative behavioral approach to the patients. This review includes detailed statements on specific headlines, such as residency period, shift system, sleep management, risk of physical and psychological

Association of job strain with working hours, shift-dependent perceived workload, sleepiness and recovery.

We explored the relationship of job strain with working hours, shift-dependent perceived workload, sleepiness and recovery. Nurses/nursing assistants (n = 95) were recruited from wards that belonged to either the top (high-strain group, HJS) or the bottom (low-strain group, LJS) job strain quartiles of a Job Content Questionnaire survey of employees in five health care districts and four cities in Finland. Three-week field measurements during naturally occurring shift schedules and a subset of pre-selected shift arrangements consisted of the Karolinska Sleepiness Scale, perceived workload and

Protection of Sudanese irrigation workers from schistosome infections by a shift to earlier working hours.

Although schistosomiasis is an important occupational hazard for irrigation workers in Central Sudan, few measures have been found to protect them, short of abandoning the work. In an attempt to reduce their exposure to the large numbers of schistosome cercariae encountered in the water at midday, a group of Gezira canal cleaners were shifted to early morning working hours after being cured of their infections. They left the water each day at 10.00 hours, working from the canal banks thereafter. At the end of 6 months the prevalence of infections with Schistosoma mansoni was much lower in this
