Dexamethasone is routinely used in anesthesia practice and has been regarded as one of the ideal perioperative agents. It is a synthetic glucocorticoid with potent antiinflammatory action. It reduces postoperative nausea and vomiting, pain, postoperative opioid requirements after general anaesthesia as well as spinal anaesthesia. It has been used via intravenous, epidural and perineural routes. It has been used successfully in fascial blocks. It significantly decreases fatigue, shivering and postoperative sore throat and improves quality of recovery.

Changes in rumen motility and heart rate following injection of 5-HT and PGE2 into the third cerebral ventricle were investigated in conscious goats. The doses used were known to produce predictable changes in thermoregulation in goats. The changes in body temperature, ear temperature (PGE2, 5-HT) and shivering (PGE2) were as reported by others. The i.c.v. injection of PGE2 and 5-HT inhibited rumen motility and slightly decreased the heart rate, probably due to a central action.

Myotonic dystrophy type 1 or Steinert's disease is an autosomal dominant multisystem disease which is characterized by consistent contracture of muscle following stimulation (myotonia). Hypothermia, shivering, mechanical or electric stimulation during surgery can precipitate episodes of myotonia which may complicate the course of anaesthesia. The present case report focuses on successful strategies for providing general anaesthesia for laparoscopic cholecystectomy in a patient affected by this genetic disorder, at a hospital which does not have the facility for postoperative ventilation.

A woman with oculocutaneous Behçet disease developed primary tuberculosis while being treated with infliximab. A latent tuberculosis infection had been excluded before therapy. After more than 80 weeks of treatment, the patient complained of fevers, night sweats, shivering and vigorous cough. The chest x-ray showed miliary shadowing. Mycobacterium tuberculosis was identified. The history revealed recent contact to an individual with smear-positive tuberculosis. This constellation speaks in favor of a de novo tuberculosis infection with a fulminant course.

We report a case of liver amoebic abscesses in a 58-year-old man referred for a FDG PET/CT with an initial diagnosis of secondary hepatic lesions. This patient, coming from Sri Lanka, presented with fever and shivers, pain in the right flank, vomiting, and diarrhea for a month. The FDG PET/CT showed a hepatic heterogeneous uptake with multiple cold lesions surrounded by a faint hypermetabolism. As this result was not typical of hepatic metastases, a hepatic biopsy was performed leading to the diagnosis of amoebic abscess.
