BACKGROUND: Open reduction and internal fixation (ORIF) with a locking plate is a popular surgical treatment for proximal humeral fractures (PHF). This study aimed to assess the occurrence of complications in elderly patients with PHF treated surgically using ORIF with a locking plate and to investigate the potential differences between patients treated by shoulder surgeons and non-shoulder surgeons.
METHODS: A retrospective study was conducted using a single-center database to identify patients aged ≥70 years who underwent ORIF for PHF between January 1, 2011, and December 31, 2021. Data on

CASE: A 32-year-old man sustained a rare and complex injury pattern representing an anterior glenohumeral fracture dislocation with critical intra-articular glenoid bone loss, and a double disruption of the superior shoulder suspensory complex (acromion + coracoid fractures), following a fall. To restore joint stability, surgical reconstruction of the glenoid fossa was performed, while fixation of the acromion and coracoid fractures was necessary to re-establish superior shoulder suspensory complex integrity.
CONCLUSION: Achieving structural stability facilitated fracture healing and rehabili

Scapular spine fractures following reverse shoulder arthroplasty have been associated with complications that include nonunion and fixation failure. This Technical Note presents a surgical approach for enhancing the stability and strength of spine fracture osteosynthesis. The method involves the utilization of double plating in conjunction with fibular allograft reinforcement anchored in the supraspinous fossa to provide support under the acromion. The allograft, offering an enhanced structural integrity, may contribute to an improved rate of bone fusion and clinical outcomes without donor sit

BACKGROUND: The Disabilities of the Arm, Shoulder and Hand (DASH) instrument was developed to assess the disability experienced by patients with any musculoskeletal condition of the upper extremity and to monitor change in symptoms and upper-limb function over time. The 30 items are scored on a 5-point rating scale. The Dutch-language version of the DASH instrument (DASH-DLV) has been examined with the classical test theory in patients with a humeral shaft fracture. This study aimed to examine the DASH-DLV with a more rigorous and extensive analysis by applying the Rasch model.
METHODS: Data

A patient presented for surgery to repair a fractured left shoulder and elbow and requested regional anesthesia. Most upper extremity operations require a single brachial plexus nerve block. The position of the two fractures however required the use of two separate approaches, an interscalene and an axillary approach.

BACKGROUND: Acromion and scapular spine fractures are recognized complications following reverse total shoulder arthroplasty (rTSA). While prior studies have identified select risk factors, formal time-to-event analyses evaluating fracture hazard remain limited.
METHODS: This retrospective database study utilized the TriNetX US Collaborative Network. Data were queried in February 2026 and included data collected from February 2006 to February 2026. Any patient undergoing rTSA with at least 1 year of longitudinal health record data were included in the analysis. Kaplan-Meier survival analysis

The use of locked humeral stems in reverse total shoulder arthroplasty (rTSA) has emerged as an alternative fixation strategy for treating complex proximal humerus fractures (PHFs). This scoping review synthesizes the available literature evaluating clinical outcomes, radiographic findings, and complications associated with interlocking humeral stems in rTSA for PHFs. Nine studies, encompassing 434 patients, were identified, of whom 308 underwent rTSA with a locking humeral stem. The mean patient age was 75.7 ± 5.1 years, with a mean follow-up of 20.0 ± 8.9 months, and at least 54% of patients

BACKGROUND: The use of reverse shoulder arthroplasty (RSA) to treat displaced, unstable 3- and 4-part proximal humerus fractures (PHFs) has traditionally been reserved for patients over 70 years old. However, recent data suggest that nearly one-third of all patients treated with RSA for PHF are between 55-69 years old. The purpose of this study was to compare outcomes for patients younger than 70 versus patients older than 70 years of age treated with RSA for a PHF or fracture sequelae.
METHODS: All patients who underwent primary RSA for acute PHF or fracture sequelae (nonunion, malunion) bet

BACKGROUND: Acromial and scapular spine fractures are common complications after reverse total shoulder arthroplasty (RTSA). There is limited information on the treatment outcome of these fractures. Therefore, the purpose of this study was to compare the clinical outcome of operative and conservative treatment of patients with acromial or scapular spine fractures.
METHODS: A total of 1146 RTSAs were performed in our institution between 1999 and 2016. In 23 patients (2%), we identified an acromial fracture, and in 7 cases (0.6%), a scapular spine fracture in the postoperative course. Of those

INTRODUCTION: Reverse shoulder arthroplasty (RSA) is a common treatment for proximal humeral fractures (PHFs) in the elderly. This study evaluates the influence of tuberosity healing (TH) on functional outcome following a 135° humeral inclination RSA for PHFs.
METHODS: Retrospectively, all patients with an acute PHF treated with a 135° humeral inclination RSA at four centers during a three-year period were followed up. Constant score (CS), TH and glenoid notching were analyzed.
RESULTS: Sixty-four of 100 patients (64%) with a mean age of 76 ± 7 years were available for follow-up at 22 ± 8 mo

BACKGROUND: In the last decade, the biomechanics of reverse shoulder arthroplasty (RSA) for proximal humeral fractures in the elderly have led to more functional outcomes and greater pain relief. However, its use has also introduced a significant rate of complications. The purpose of this study is to assess the complication rate of RSA in these cases as well as the relationship to ASA score and basic disease history.
METHODS: We evaluate patients who underwent RSA due to acute proximal humeral fractures at three hospitals from 2010 to 2016. We analyze previous disease and anticoagulant treatm

Complex proximal humerus fractures in elderly patients represent a difficult problem for orthopaedic surgeons. Classically, treatment methods have included nonoperative management, open reduction and internal fixation, and hemiarthroplasty. Outcomes of nonoperative management for these complex fractures have been poor, and results of traditional operative techniques are variable at best. Over the past several years, reverse total shoulder arthroplasty (RTSA) has been increasingly employed in these injuries with encouraging results. RTSA may represent a valuable treatment option for select pati
