Results after conservative or surgical treatment of comminuted fractures of the head of the humerus are often unsatisfactory and complicated by pain and a severely decreased range of motion. In these patients good results have been obtained by replacement of the humeral head with the Isoelastic Humeral Head Prosthesis. Fourteen cases are reported and their results discussed.

Twenty-one cases of recurrent anterior dislocation of the shoulder joint were operated on by a modification of Bankart's technique between 1975 and 1981. The modification consists in the introduction of two screws with their respective washers in order to fix the lateral flap of the capsule to the neck of the glenoid cavity, after which the medial flap is turned back and secured by a reefing suture. This device overcomes the difficulty encountered with the insertion of the joint capsule in Bankart's original method.

BACKGROUND: Persistent instability and altered scapular mechanics after acromioclavicular joint (ACJ) reconstruction may stem from inadequate replication of native coracoclavicular (CC) ligament function. However, in vivo data on postoperative CC ligament behavior during dynamic shoulder motion remain lacking.
PURPOSE: To assess in vivo functional distance and timing characteristics of the conoid and trapezoid ligaments during active shoulder elevation after ACJ reconstruction, using dynamic stereo x-ray (DSX) imaging and patient-specific 3-dimensional (3D) models.
STUDY DESIGN: Cross-sectio

OBJECTIVE: A common cause of shoulder joint pain in this population is subacromial impingement, often associated with increased thoracic kyphosis. However, the association between thoracic kyphosis angle and shoulder joint pain in competitive swimmers remains uncertain. The aim of this study was to investigate factors associated with shoulder joint pain in male competitive swimmers, considering both previously reported variables and the thoracic kyphosis angle.
DESIGN: Cross-sectional observational study.
SETTING: Poolside at the participants' school.
PARTICIPANTS: A total of 64 male univer…

BACKGROUND: Corticosteroid injections (CSIs) are used for the symptomatic management of osteoarthritis. However, their use may contaminate the joint space and pose an increased risk of periprosthetic joint infection (PJI) following reverse shoulder arthroplasty (RSA). Therefore, the purpose of this study was to assess whether there is any association between the timing of CSI and the incidence of PJI at 90 days, 1 year, and 2 years postoperatively. Specifically, we assessed the risk of PJI in patients who received CSI <1 month, 1-2 months, 2-3 months, and >3 months prior to RSA, as well as ass

BACKGROUND: Excessive general joint laxity, a negative prognostic factor in joint instability, has not been studied to determine its relationship with bipolar bone loss in anterior shoulder instability.
PURPOSE/HYPOTHESIS: This study aimed to investigate the characteristics of bipolar bone defects in the presence of excessive joint laxity and the clinical outcomes based on the on-track/off-track theory. We hypothesized that (1) patients with excessive joint laxity might have less significant bipolar bone defects compared with those without excessive joint laxity and (2) no significant differe

Understanding how individuals distribute mechanical demand imposed on their upper extremity during physically demanding activities provides meaningful insights to preserve function and mitigate detrimental mechanical loading of the shoulder. In this study, we hypothesized that parameterization of the shoulder net joint moment using four functional axes could characterize distribution tendencies about the shoulder during manual wheelchair propulsion and that regardless of demographics, a shoulder flexor dominant NJM distribution would be predominantly used by individuals with paraplegia (n = 13

BACKGROUND: In rugby the tackle is a complex task requiring joint position sense (JPS). Injuries commonly occur during the tackle and these account for significant time lost from training and play. Simulated tackling tasks have previously shown a reduction in shoulder joint position sense and it is possible that this may contribute to injury. There is growing evidence in support of injury prevention programs, but none so far are dedicated specifically to tackling. We postulate that a brief neuromuscular warmup could alter the negative effects of fatigue on shoulder JPS.
METHODS: In this field…

BACKGROUND: Rotator cuff tears in older individuals may result in decreased muscle forces and changes to force distribution across the glenohumeral joint. Reduced muscle forces may impact functional task performance, altering glenohumeral joint contact forces, potentially contributing to instability or joint damage risk. Our objective was to evaluate the influence of rotator cuff muscle force distribution on glenohumeral joint contact force during functional pull and axilla wash tasks using individualized computational models.
METHODS: Fourteen older individuals (age 63.4 yrs. (SD 1.8)) were

OBJECTIVE: Clinical evidence has shown that extremity dysfunction, such as muscle weakness or inhibition, is associated with spinal disorders. Spinal manual therapy is a common therapeutic approach used to address extremity muscle weakness. The purpose of the study was to assess changes in the maximal muscle strength of the shoulder external rotators immediately and at 10, 20, and 30 minutes after cervical joint mobilization at the C5-6 segment.
METHODS: Eighteen participants with existing or a history of neck pain were screened by 2 investigators independently for muscle weakness of shoulder

This paper describes the personal experience of the author with Shoulder Arthro MR. This imaging method is put into perspective with other, more widely used diagnostic methods, which also have their advantages. Indications for Shoulder Arthro MR are discussed and details on the technique and interpretaton are given. Also major shoulder pathology as impingement, rotator cuff tears and instability is discussed. Furthermore a review of the spectrum of pathology in 100 consecutively examined patients is made and treatment and outcome are listed. Lastly a pictorial review of typical pathologies in

Abnormal fibroblast proliferation has been implicated in the pathogenesis of adhesive capsulitis. The aim of the study was to examine the effect of fluid aspirated from the glenohumeral joint of patients with adhesive capsulitis on human fibroblast cells. Shoulder aspirate from 15 patients with stage II adhesive capsulitis was obtained and diluted to concentrations of 10%, 25%, and 50%. Fifteen patients undergoing shoulder arthroscopy for instability comprised the normal control group. Human fibroblast cells were cultured from human volunteers. Cellular proliferation was determined by use of t

Restoration of the soft tissue balance is of great importance during shoulder arthroplasty. We report a novel technique of posterior capsulorrhaphy through an anterior approach for the management of intraoperative posterior instability during implantation of the prosthesis. After the operation, the patients were able to follow a rehabilitation program with some minor restrictions, avoiding the necessity of a shoulder spica or other constrictive measures. This technique is simple, safe and sufficient to address instability due to abundance of the posterior shoulder capsule.

BACKGROUND: Rice body synovitis is a rare subtype of synovitis, often secondary to chronic inflammatory diseases, such as rheumatoid arthritis (RA). Due to its non-specific clinical manifestations, it is prone to misdiagnosis.
METHODS: This report describes a 58-year-old male patient with left shoulder rice body synovitis. The patient had a 25-year history of rheumatoid arthritis and presented with a left shoulder mass for over 2 months, accompanied by pain and limited joint mobility within the previous week. Laboratory findings revealed markedly elevated rheumatoid factor levels (128.0 IU/mL

INTRODUCTION: The etiology of rotator cuff tear (RCT) is multifactorial and includes extrinsic and intrinsic factors. Alterations of the peripheral microcirculation represent a main intrinsic etiological factor; recent evidence demonstrates that at the level of the rotator cuff tendons there is significant reduction in capillary density and microvascular blood flow. On the basis of the alterations of the microcirculation present at the level of the rotator cuff (RC) tendons, the aim of our study was to evaluate the possible presence of alterations of the peripheral nail microcirculation, throu

In the present study, we used a transformer model and a fusion of biosignals to estimate rotational acceleration in elbow and shoulder joints. To achieve our study objectives, we proposed a mechanomyography (MMG) signal isolation technique based on a variational mode decomposition (VMD) algorithm. Our results show that the VMD algorithm delivered excellent performance in MMG signal extraction compared to the commonly used technique of empirical mode decomposition (EMD). In addition, we found that transformer models delivered estimates of joint acceleration that were more precise than those pro…

Hypermobile Ehlers-Danlos syndrome (hEDS) and hypermobility spectrum disorders (HSD) are common causes of chronic musculoskeletal pain. Current practices rely on analgesics, physical therapy, bracing, and assistive devices. Dextrose prolotherapy (DPT) is a regenerative injection modality used to treat chronic painful musculoskeletal conditions through stimulation of tissue proliferation. The effectiveness of DPT for the treatment of chronic shoulder pain in patients with hEDS/HSD has not been established in the literature. Three patients with hEDS or HSD presented with refractory shoulder pain
