[Influence of the sickle cell trait heterozygote on energy abilities].

The sickle cell trait (SCT) is a genetic abnormality of the red blood cell which mainly affects people of African descent. It is due to the mutation of only one parental gene (one glutamic acid of the chain beta of the globin is substituted by one valin). The prevalence of SCT in the black US population is within the range of 8-9%. It is increasing in Europe and in Africa where it may reach up to 40% in some regions. The rate of prevalence of SCT in athletic populations was found to be similar to that of the general sedentary population in west African countries. SCT is usually asymptomatic. H…

Renal papillary necrosis in a patient with sickle cell trait.

A patient with sickle cell trait who presented with gross hematuria and was subsequently found to have renal papillary necrosis is presented. The hematuria resolved with conservative therapy consisting of bed rest and hydration with hypotonic intravenous fluids. The pathophysiology of renal abnormalities associated with sickle cell trait is described. The management of the primary clinical manifestations of this disorder, hematuria and papillary necrosis, are discussed.

Sudden exertional death in a soldier with sickle cell trait.

Most family physicians regard sickle cell trait (SCT) as a benign condition. But in special circumstances SCT can have deadly consequences. SCT affects 8% of the black population. Most of the cases of sudden exertional death (SED) in those with SCT have occurred during military basic training. A retrospective review of all soldiers in basic training found that those with SCT had a 40-fold increased risk of SED. This paper discusses an individual case of SED that occurred at Fort Sill, Oklahoma. A literature review makes comparisons to our index case. The current Army Medical Corps recommendati…

Exertional collapse and sudden death associated with sickle cell trait.

The most serious complication of sickle cell trait (SCT) is sudden death during exertion. SCT often remains unrecognized in the 2.5 million African Americans affected. Exertional collapse and sudden death associated with SCT is characterized by rhabdomyolysis, heat stroke, and cardiac arrhythmia. There is a 40-fold increased risk of sudden death in affected soldiers during military basic training and there are many cases reported in athletes during preseason training. There have been no cases reported in soldiers beyond basic training. In the case presented, a soldier with 3 years of military …

Cilioretinal artery occlusion in sickle cell trait and rheumatoid arthritis.

BACKGROUND: Although once thought to be a benign condition, retinal vascular occlusive disease and proliferative retinopathy can occur with sickle cell trait (hemoglobin AS) when additional systemic diseases or trauma are present. METHODS: The authors discuss the ophthalmologic evaluation and clinical course of a 49-year-old woman with sickle cell trait and rheumatoid arthritis who presented with a cilioretinal artery occlusion. RESULTS: The patient's laboratory evaluation showed both a high rheumatoid factor titer and a mild hypergammaglobulinemia, causing increased serum viscosity. The hig…

Renal medullary carcinoma associated with sickle cell trait: radiologic findings.

PURPOSE: To correlate the radiologic and pathologic findings in patients with renal medullary carcinoma and sickle cell trait. MATERIALS AND METHODS: Radiologic studies of five pathologically proved cases of renal medullary carcinoma were retrospectively correlated with gross pathologic findings. Excretory urograms, computed tomographic (CT) scans, sonograms, photographs of the gross surgical specimens, and an angiogram were available for review. Each case was analyzed for tumor location, pattern of growth, contrast enhancement and echotexture, angiographic pattern, and stage. RESULTS: All t…

Acute exertional rhabdomyolysis and its relationship to sickle cell trait.

Exertional rhabdomyolysis, a syndrome characterized by skeletal muscle degeneration and muscle enzyme leakage, has been shown to occur in normal, healthy individuals following strenuous exercise. In severe cases, this syndrome can result in renal failure and sudden death. Although anyone who performs strenuous exercise may be at risk for developing exertional rhabdomyolysis, some individuals may be more susceptible than others. A number of case reports of exertional rhabdomyolysis involve persons with sickle-cell trait, leading to the theory that these individuals might be at greater risk for

On the protective mechanism of the sickle cell trait (HbAS).

The sickle cell trait has been reported to offer some protection against anaemia in pregnancy. Some attribute this to the break up of the cells which release iron for the synthesis of haemoglobin. The hypothesis presented here is that the 'partial' protective mechanism of the sickle cell trait is due to increased erythropoietic activity.

Splenic sequestration associated with sickle cell trait and hereditary spherocytosis.

Coexistence of sickle cell trait and hereditary spherocytosis (HS) is unusual, and only 16 cases have been reported in the literature. These patients have the same clinical and hematological features as individuals having HS alone. We report a serious complication, acute splenic sequestration crisis (ASSC), occurring in two patients with sickle cell trait and HS. One patient experienced four episodes of ASSC during an 11-year span, while the other had two episodes of this complication during a 4-year period. Red blood cell studies and membrane protein analysis confirmed the diagnosis of HS as …

Proteinuria in sickle cell trait and disease: an electrophoretic analysis.

Using ultrafiltration and SDS-PAGE, abnormal urinary protein excretion was found in 25.4% of 189 persons with sickle cell disease and trait, but none of 72 controls. Based upon molecular weight of urinary proteins, underlying renal lesions were classified as glomerular, tubular, or both. Altered protein excretion appeared at an early age, was more abnormal in older subjects, and was related to the severity of sickle cell disease (SS greater than SC = S/beta Thal greater than AS). Since none of the subjects had yet developed clinically significant renal disease, SDS-PAGE may permit early detect

Superior sagittal sinus thrombosis with infarction in sickle cell trait.

An adolescent female with sickle cell trait presented with acute neurologic deterioration during treatment for pseudotumor cerebri. Cranial computed tomography, initially normal, subsequently revealed multiple hemorrhagic infarctions. Suspected superior sagittal sinus thrombosis was confirmed by cerebral angiography. Superior sagittal sinus thrombosis associated with sickle cell trait is exceedingly rare, and the accompanying increased intracranial pressure may require aggressive management.

Hypothermic cardiopulmonary bypass in a patient with sickle-cell trait.

A male patient, aged 7.5 years, with known sickle cell trait, presented for correction of Fallot's tetralogy. The successful management under hypothermic cardiopulmonary bypass is described and the behaviour of sickle haemoglobin under hypothermic conditions is discussed.

Recurrent hematuria in 4 white patients with sickle cell trait.

Extensive investigations failed to disclose the etiology of recurrent gross hematuria in 4 white patients of Algerian descent. Hemoglobin electrophoresis revealed sickle cell trait in all cases. The hematuria ceased after bed rest and hydration in 3 patients, and following partial nephrectomy after visualization of the bleeding site at operative nephroscopy in 1. We recommend that hemoglobin electrophoresis be considered when evaluating every patient, black or white, presenting with unexplained hematuria.
