The integration of sleep medicine into hospital care is critical for addressing sleep-disordered breathing (SDB), a condition highly prevalent among hospitalized patients. Effective management of SDB is linked to improved patient outcomes, reduced recovery times, and decreased hospital readmission rates. Advancements in technology, such as wearable devices and noninvasive monitoring tools, have enhanced real-time assessment and management of SDB. Early detection and intervention through structured hospital sleep medicine programs have shown improvements in patient care and overall health care

Inpatient sleep testing (IST) provides a unique although underused opportunity to improve access to care by identifying patients with sleep-disordered breathing (SDB) who may benefit from timely initiation of positive airway pressure (PAP) therapy. SDB identified in hospitalized patients with cardiovascular and pulmonary comorbidities is associated with worse clinical outcomes, but initiation of PAP therapy based on inpatient testing may improve patient outcomes. Various types of IST can be performed, with portable monitoring being the most common given its accuracy, convenience, and cost-effe

Obstructive sleep apnea (OSA) is a very common and underdiagnosed condition across the world. It affects both pediatric and adult populations in unique but important ways. Long-term health risks associated with OSA include cardiovascular conditions, metabolic disorders, depression as well as poor work performance, and increased risk of motor vehicle accidents. Accurate and precise testing is vital to ensure accurate treatment, and specific testing methods are reviewed. Treatment options are discussed in detail for both adult and pediatric populations.

The depth, rate, and regularity of breathing change following transition from wakefulness to sleep. Interactions between sleep and breathing involve direct effects of the central mechanisms that generate sleep states exerted at multiple respiratory regulatory sites, such as the central respiratory pattern generator, respiratory premotor pathways, and motoneurons that innervate the respiratory pump and upper airway muscles, as well as effects secondary to sleep-related changes in metabolism. This chapter discusses respiratory effects of sleep as they occur under physiologic conditions. Breathin

Central sleep apnea (CSA) is characterized by intermittent repetitive cessation and/or decreased breathing without effort caused by an abnormal ventilatory drive. Although less prevalent than obstructive sleep apnea, it is frequently encountered. CSA can be primary (idiopathic) or secondary in association with Cheyne-Stokes respiration, drug-induced, medical conditions such as chronic renal failure, or high-altitude periodic breathing. Risk factors have been proposed, including gender, age, heart failure, opioid use, stroke, and other chronic medical conditions. This article discusses the prev

Obstructive sleep apnoea (OSA) is increasing in prevalence due to rising obesity. While OSA is a disorder primarily of the upper airway during sleep, its pathophysiological impact on other body systems is increasingly recognised. There has been interest in the prevalence of OSA in different ophthalmic conditions and possible causation has been postulated. As OSA is common, it can be expected that people with co-existent OSA will be found in any ophthalmic disease population studied. To determine with confidence the significance of finding patients with OSA in a particular cohort requires a wel

PURPOSE OF REVIEW: Sleep-disordered breathing encompasses a broad spectrum of sleep-related breathing disorders, including obstructive sleep apnea (OSA), central sleep apnea, as well as sleep-related hypoventilation and hypoxemia. Diagnostic criteria have been updated in the International Classification of Sleep Disorders, Third Edition and the American Academy of Sleep Medicine Manual for Scoring Sleep and Associated Events. Neurologic providers should have basic knowledge and skills to identify at-risk patients, as these disorders are associated with substantial morbidity, the treatment of w

Sleep has a strong influence on interictal epileptiform discharges and on epileptic seizures. Interictal epileptiform discharges are activated by sleep deprivation and sleep, and some epilepsies occur almost exclusively during sleep. Treatment of sleep-related epilepsy should take in account the type of epileptic syndrome, the type of seizures, the patient characteristics, and also the pharmacokinetics of the drug. Proper characterization of the epilepsy is essential to choose appropriate antiepileptic drugs. Drugs effective in focal epilepsy may be used to treat benign genetic focal epilepsie

Most people spend a third of their lives sleeping, and thus, sleep has a major impact on all of us. As sleep is a function and not a structure, it is challenging to treat and prevent its complications. Sleep apnoea is one such complication, with serious and potentially life-threatening consequences. Local studies estimate that about 15% of Singapore's population is afflicted with sleep apnoea. The resulting sleep fragmentation may result in poor quality of sleep, leading to daytime sleepiness. Sleep apnoea may also be the underlying cause of high blood pressure, memory loss, poor concentration

Primary sleep disorders include those not attributable to another medical or psychiatric condition: insomnia disorder, hypersomnolence disorder, narcolepsy, obstructive sleep apnea hypopnea syndrome, central sleep apnea syndrome, and the parasomnias. They are commonly encountered and are comorbid with many psychiatric disorders. It is important to recognize these disorders and be comfortable treating them or to know when to refer to a sleep disorders center and sleep specialist. Treatment of a comorbid sleep disorder can improve the overall quality of life, symptoms in mood disorders, and symp

Neurophysiologically, central apnea is due to a temporary failure in the pontomedullary pacemaker generating breathing rhythm. As a polysomnographic finding, central apneas occur in many pathophysiological conditions. Depending on the cause or mechanism, central apneas may not be clinically significant, for example, those that occur normally at sleep onset. In contrast, central apneas occur in a number of disorders and result in pathophysiological consequences. Central apneas occur commonly in high-altitude sojourn, disrupt sleep, and cause desaturation. Central sleep apnea also occurs in numb

Irrespective of diagnosis, chronic daily, morning, or "awakening" headache patterns are soft signs of a sleep disorder. Sleep apnea headache may emerge de novo or may present as an exacerbation of cluster, migraine, tension-type, or other headache. Insomnia is the most prevalent sleep disorder in chronic migraine and tension-type headache, and increases risk for depression and anxiety. Sleep disturbance (e.g., sleep loss, oversleeping, schedule shift) is an acute headache trigger for migraine and tension-type headache. Snoring and sleep disturbance are independent risk factors for progression

Obstructive sleep apnea (OSA) is a common disorder characterized by repetitive collapse of the pharyngeal airway during sleep. Control of pharyngeal patency is a complex process relating primarily to basic anatomy and the activity of many pharyngeal dilator muscles. The control of these muscles is regulated by a number of processes including respiratory drive, negative pressure reflexes, and state (sleep) effects. In general, patients with OSA have an anatomically small airway the patency of which is maintained during wakefulness by reflex-driven augmented dilator muscle activation. At sleep o

Obesity is a critical factor in the development of sleep-disordered breathing (SDB). Snoring is the most frequent nocturnal symptom suggesting a diagnosis of SDB. Other common nighttime symptoms include snorting, gasping, choking, coughing, and witnessed apneas. The most frequent diurnal symptom in SDB is excessive daytime sleepiness. Patients suspected of having SDB should undergo a full night of in-laboratory polysomnography or in-home oligosomnography. SDB includes a spectrum of disorders; the most common are obstructive sleep apnea and central sleep apnea.

The common feature that characterized humoral theories of sleep is the production and accumulation of certain substances, usually end-products of metabolism, either in the tissues, in general, or in certain organs, such as the brain. When a definite concentration of such substances has been reached, the activity of the brain is depressed either directly, or through indirect influences, like vasoconstriction or asphyxia. The gradual removal of these substances during sleep leads to a return to the waking state. Humoral sleep theories were popular in the twentieth century. However, none of them
