BACKGROUND: Direct access to the sphenoid lateral recess offers the best chance of sealing spontaneous cerebrospinal fluid (CSF) rhinorrhea caused by lateral sphenoid encephaloceles of the Sternberg canal defect.
METHOD: We present a case of spontaneous left-sided sphenoid lateral recess CSF leak after previous unsuccessful transcranial surgery managed with an endoscopic endonasal transpterygoid approach (EETA). An anatomical-based step-by-step illustration of the EETA was presented in the surgical video.
CONCLUSION: This case demonstrates the value of endoscopic endonasal transpterygoid cor

INTRODUCTION: Pneumatization of the sphenoid sinus (SS) varies widely among different ethnic groups. Information regarding the prevalence and significance of SS variants among Hispanic groups is limited. This study aims to describe and analyze pneumatization and septation patterns of the SS in a Hispanic population.
METHODS: A total of 160 paranasal sinus computed tomographies were reviewed by a head and neck-specialized radiologist and 2 otolaryngologists.
RESULTS: The postsellar and sellar types were the most frequent patterns of pneumatization observed, with a prevalence of 52.5 and 40%,

BACKGROUND: The variant pneumatization patterns of the sphenoid sinus have significant surgical implications due to their associated inconsistent neurovascular relations. This study aimed at evaluating the pneumatization patterns of the sphenoid sinus in adult Nigerians.
METHODS: This was a retrospective study conducted at the Radiology Department of a Tertiary Hospital in Nigeria after obtaining institutional ethical approval. Brain Computed Tomography images of 336 patients (137 females, 199 males) aged ≥20 years were studied for the variant pneumatization patterns of the sphenoid sinus. St

The anatomy of the skull base is complex with multiple neurovascular structures in a small space. Understanding all of the intricate relationships begins with understanding the anatomy of the sphenoid bone. The cavernous sinus contains the carotid artery and some of its branches; cranial nerves III, IV, VI, and V1; and transmits venous blood from multiple sources. The anterior skull base extends to the frontal sinus and is important to understand for sinus surgery and sinonasal malignancies. The clivus protects the brainstem and posterior cranial fossa. A thorough appreciation of the anatomy o…

Intracranial transtemporal orbital decompressions in one case of massive fibrous dysplasia of temporal, parietal and maxillary bones and in two cases of meningioma of the sphenoidal region are described. Surgery for fibrous dysplasia, even if extensive, must always be modified according to cosmetic and functional aspects. In bone-invading meningioma, large decompressing resections should be made. They may save the eye and arrest the progress of the disease for long periods of time.

A 66-year-old woman with no prior medical history presented to the emergency department with diplopia and parosmia. The neurological examination identified an isolated left abducens nerve (CN VI) palsy. A head CT scan, followed by a brain MRI, showed a large, locally advanced tumour in the left sphenoid sinus with extensive skull base involvement and perineural extension into the left orbit. The histopathological analysis of the sphenoid sinus biopsy revealed high-grade diffuse large B-cell lymphoma. A PET scan confirmed this was a primary bone lymphoma. MRI of the pituitary, without contrast,

BACKGROUND: Fibro-osseous lesions include a variety of bone lesions with different clinical and histopathological features.
CASE DESCRIPTION: We report a case of cemento-ossifying lesion involving the left ethmoid and sphenoid sinus in a 17-year-old male patient. Computed tomography showed an expansile hyperdense lesion with sclerosed peripheral mantle epicentered on the left ethmoid and sphenoid sinus and extending into the left nasal cavity. Magnetic resonance imaging was done for preoperative planning. Transnasal endoscopic resection was performed and histopathological examination confirme

Reports of congenital anomalies of the Eustachian tube are scare. These anomalies are usually associated with chromosomal abnormalities, most often in oculoauriculovertebral spectrum. We present a case of completely bony widened Eustachian tube, entering the cells of sphenoid sinus lateral recess. No wall defect between the sphenoid sinus and the tube was found, nevertheless the tube and middle ear had normal pneumatisation. The anatomy of the outer ear, otoscopy and hearing thresholds were normal on the ipsilateral side. At the same time, there were microtia, atresia of the external auditory

Herein we report a 30-year-old man presenting with fevers, headaches and weight loss. On admission he was disorientated and demonstrated no focal signs of neurological deficit. Magnetic resonance imaging revealed a large area of abnormal bone marrow signal centred within the clivus with extension into the sphenoid sinus and signs of associated basal meningitis. A sphenoid sinus biopsy was performed and proved non diagnostic. The patient was treated empirically with antitubercular therapy (ATT). Lumbar puncture provided cerebrospinal fluid from which Mycobacterium tuberculosis (MTB) was isolate

A 66-year-old male patient was admitted due to high fever, severe headaches and disturbance of consciousness. Meningitis was confirmed by lumbar puncture and intravenous antimicrobial therapy was started. Since he had undergone radical tympanomastoidectomy 15 years before, otogenic meningitis was suspected, so the patient was referred to our department. Clinically, the patient manifested watery discharge from the right nostril. Microbiological analysis verified Staphylococcus aureus in a cerebrospinal fluid (CSF) sample acquired by lumbar puncture. Radiological work-up, including computed tomo

Cemento-ossifying fibromas are benign lesions of the jaw, which arise from the periodontal membrane. Histopathologically these are composed of fibrous tissues with calcified structures resembling bone and cementum. Surgical resection is the treatment of choice. They have rarely been reported in the ethmoid sinus, maxillary sinus and sphenoid sinus Mastoid bone is an extremely rare site of such tumors. Only one case of cemento-ossifying fibroma of petromastoid bone has been reported before. We present a case of cementoossifying fibroma involving the petromastoid bone, with the large intracrania

We report a case of a patient with traumatic carotid cavernous fistula (CCF) caused by transnasal-transsphenoidal surgery, who was successfully treated using detachable coils. A 47-year-old man was admitted to our hospital because of severe headache. He was confirmed to have a nonfunctioning pituitary adenoma with presellar-type sphenoid sinus. Cerebral angiography initially disclosed no vascular lesions. A transnasal-transsphenoidal adenomectomy was performed. When the anterior wall of the sphenoid sinus was dissected with a chisel, the chisel deeply stuck into the posterolateral part of the
