Gallstone ileus following endoscopic retrograde cholangiopancreatography and endoscopic sphincterotomy.

Endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic sphincterotomy (ES) is the treatment of choice for common bile duct (CBD) stones. Complication rates of 5-15%, and mortality rates of 1.0% have been reported. We report a case of gallstone ileus presenting 12 days after ERCP and ES.

[Access to the biliary tract through trans-pancreatic sphincterotomy].

OBJECTIVE: Several pre-cut procedures have been used in patients with inaccessible bile ducts. Trans-pancreatic sphincterotomy (TS) has been demonstrated to be a valid, but little used technique. We describe our experience with TS with a prospective analysis of its results. PATIENTS AND METHODS: Twenty patients who underwent TS due to the impossibility of cannulating the biliary tract using routine methods between November 2004 and October 2005 were included. CT-25 sphincteromes (Cook) were used, introducing the tip into the common bile duct or pancreatic duct, performing the cut toward the b…

[Needle-knife sphincterotomy for biliary access: a prospective study].

OBJECTIVE: Needle-knife sphincterotomy (NKS) is used to achieve biliary access when routine cannulation methods have been unsuccessful. The aim of this study was to analyze the results of NNS in patients with an inaccessible common bile duct (difficult cannulation) using standard techniques in endoscopic retrograde cholangiopancreatography and the factors associated with the success of this procedure and complications. MATERIAL AND METHODS: We performed a prospective study of 72 patients who underwent NKS performed by the same endoscopist (J.E.) because of difficult cannulation of the common …

Effectiveness of endoscopic sphincterotomy in complicated hepatic hydatid disease.

BACKGROUND: The aim of this study was to evaluate the results of endoscopic sphincterotomy in patients with hepatic hydatid cysts that have ruptured into the biliary tract. METHODS: Over a 10-year period, 25 patients (11 men, 14 women, mean age 60 years) underwent treatment; 13 with no prior surgery were treated for biliary obstruction. Postoperative treatment was undertaken in 12 cases because of persistent drainage (8), duct obstruction (3), and postoperative pancreatitis (1). RESULTS: In patients who had not undergone previous surgery, cholangiographic findings were hydatid vesicles in th…

Randomized controlled trial of lateral internal sphincterotomy with haemorrhoidectomy.

A prospective trial was conducted to compare haemorrhoidectomy alone (control, group 1) with haemorrhoidectomy plus lateral internal sphincterotomy (group 2) for prolapsed piles. Some 33 patients (18 men, 15 women) of mean(s.e.m.) age 40(2.3) years were randomized, 16 to group 1 and 17 to group 2. There were no significant differences in postoperative pain scores. Mean resting and maximum anal squeeze pressures, studied 6 weeks and 3 months after operation, were generally lower in group 2, but were not significantly different. Two patients in group 2 were distressed by incontinence to liquid f…

Longterm effects of endoscopic sphincterotomy on gall bladder motility.

BACKGROUND: Some of patients with an intact gall bladder develop acute cholecystitis or have gall bladder stone formation after endoscopic sphincterotomy. Endoscopic sphincterotomy may affect gall bladder motility. AIMS: To prospectively evaluate longterm effect of endoscopic sphincterotomy on gall bladder motility. PATIENTS: Thirty two patients with an intact gall bladder (15 with and 17 without gall bladder stones) who underwent endoscopic sphincterotomy for choledocholithiasis. METHODS: Gall bladder function was examined before and at from seven days to five years after sphincterotomy. G…

Variations of common bile duct diameter after endoscopic sphincterotomy.

Diameters of the common bile duct (CBD) and common hepatic duct (CHD) were measured before and after endoscopic sphincterotomy (EPT) in 100 patients. There was a significant reduction of the lumen diameter in both the CBD and CHD (p less than 0.01) after EPT. A comparison between 32 patients without symptoms (such as pain, residual stones, and elevated levels of liver enzymes) and 19 patients with remaining symptoms after EPT, showed a statistically significant reduction in both CBD and CHD diameters (p less than 0.001) in the former group, and no statistically proven changes in the diameters …

[Uses of endoscopic high frequency diathermy: polypectomy--sphincterotomy (papillotomy)].

Several complications may arise as a consequence of monopolar high frequency diathermy applied for control of hemorrhage, polypectomy and papillotomy because of the specific conditions of current flow. Such complications can be burns, thermic organ damage or dysfunction of implanted cardiac pacemakers. Results and experience obtained during the last decade however show, that this endoscopic method of surgery is efficient and of low risk, if appropriate precautions are taken.

[Results of endoscopic sphincterotomy in common bile duct lithiasis].

Endoscopic sphincterotomy (ES) was attempted in 409 patients with common bile duct stone(s) (CBDS). The mean age of patients was 72.0 +/- 0.8 years (m +/- SEM); 47 p. 100 presented risk factors; 57 p.100 had previously been cholecystectomized while 43 p. 100 had not. On an average, patients in the former group were older (80 +/- 0.7 years) than in the latter 65.4 +/- 1.0 years, p less than 0.001). The procedure was successful in 98 p. 100 of the patients, after a standard ES in 78.5 p. 100 or after different technical artifices in 21.5 p. 100. The vacuity of the CBD was obtained in 96.5 p. 100…

Lateral subcutaneous internal sphincterotomy in treatment of anal fissure.

One hundred and three patients with anal fissures have been treated by lateral internal sphincterotomy performed by a subcutaneous myotomy technique, and 99 have been successfully followed up for from 3 to 24 months (mean 11 months). The operation was free from significant complications, apart from slight reactionary bleeding in one case and a perianal abscess in another. The fissure persisted or recurred in three cases, in one of which a further lateral sphincterotomy was performed, with cure. Various minor defects of anal continence were noted in 12 cases. These results have been found to be…

Non-standardized surgery lateral internal sphincterotomy: Is there a consensus?

OBJECTIVE: Lateral internal sphincterotomy (LIS) is considered the gold standard surgical treatment for chronic anal fissures. However, substantial variation exists in the surgical techniques applied. This study aims to evaluate practice differences among surgeons performing LIS and to assess whether a consensus has been established. MATERIAL AND METHODS: An anonymous online survey was conducted to assess surgeons' technical approaches to LIS. Data were collected using a 24-question survey targeting surgeons from various countries. RESULTS: A total of 207 surgeons (131 from Türkiye, 76 from …
