Nine patients (5 male and 4 female) are described with mild pain in the lumbar and thoracolumbar spine, early morning pain, morning stiffnes … B27. On the basis of these observations it is considered that 7 of these 9 patients had ankylosing spondylitis, the erosive lesions of the vertebral bodies being the first visible signs of the disease.

Two cases of Salmonella spondylitis with unusual features appeared in patients who have no sickle cell disease and no history of typhoid fever. One case presented with paraparesis, was misdiagnosed and originally treated for Pott's disease. The other case was diagnosed correctly by specific laboratory tests, but failed to respond to the conventionally accepted conservative antibiotic treatment and required surgical excision and drainage. Salmonella spondylitis with paraparesis may not have been reported heretofore.

1. Concomitant cervical and thoracic spine fractures are reported in a patient with advanced ankylosing spondylitis (second reported case). 2. The thoracic T9-10 interspace fracture-dislocation was grossly unstable, as well as the C7-T1 fracture-dislocation. 3. The higher incidence of cord injury following spinal fracture-dislocations in these patients is discussed. 4. The dangers of inadequate postinjury immobilization in those patients fortunate enough to be neurologically intact following spinal fracture are stressed.

BACKGROUND: Brucella spondylitis (BS) and tuberculous spondylitis (TS), caused initially by bacteremia, are the two leading types of granulomatous spinal infections. BS is easy to miss or may be misdiagnosed as TS. Our purpose aims to differentiate BS from TS in conventional MR imaging and MR T2 mapping.
METHODS: We performed on 26 BS and 27 TS patients conventional MR imaging and MR T2 mapping. We analyzed the features in conventional MR imaging and measured T2 values of the lesion vertebrae (LV) and unaffected adjacent vertebrae (UAV) in BS and TS patients, respectively.
RESULTS: There wer…

OBJECTIVE: Familial Mediterranean Fever (FMF) is an autoinflammatory disease that is commonly present with recurrent episodes of fever, peritonitis, pleuritis or arthritis. Enthesitis and sacroilitis can also be seen in FMF. Spondylitis is a less common manifestation of joint involvement in FMF and there are controversial publications about whether this involvement is FMF-related or coincidentally. The aim of this study was to provide a comparison between ankylosing spondylitis (AS) patients and FMF patients with AS.
METHODS: A total of 404 patients who 360 of them was AS and 44 was FMF patie…

OBJECTIVE: To investigate the distribution of subtypes between HLA-B*27 (+) patients with ankylosing spondylitis (AS) and carriers.
METHODS: This case–control study recruited Chinese Han patients with HLA-B*27 (+) AS from six hospitals in Zhejiang Province, China between 2013 and 2018. Patients who were examined for HLA-B*27 because of back pain or arthralgia but who did not have AS or arthritis were recruited as controls. HLA-B*27 target DNA was amplified by amplification refractory mutation systems and HLA-B*27 subtypes were determined by sequencing.
RESULTS: The positive rate of HLA-B*27 …

BACKGROUND: Ankylosing spondylitis (AS) is characterised by immune-mediated arthritis and osteoproliferation, ultimately leading to joint ankylosis. Whether inflammation is necessary for osteoproliferation is controversial, fuelled by the unclear efficacy of anti-inflammatory treatments on radiographic progression. In proteoglycan-induced spondylitis (PGISp), a mouse model of AS, inflammation is the prerequisite for osteoproliferation as osteoproliferation was only observed following inflammation-driven intervertebral disc (IVD) destruction. We hypothesised that early intervention with a poten…

BACKGROUND: Preference-based measures of health-related quality of life (HRQoL), such as the EQ-5D or the SF-6D, are essential for health economic evaluation. However, they are rarely included in clinical trials of ankylosing spondylitis (AS). This study aims to develop mapping algorithms to predict EQ-5D-3L and EQ-5D-5L health utility scores from the Bath Ankylosing Disease Activity Index (BASDAI) and the Bath Ankylosing Spondylitis Functional Index (BASFI).
METHODS: Patients with AS were recruited from the largest tertiary hospital in Shandong province, China, between December 2019 and Octo…

PURPOSE: This study aimed to develop and validate a deep learning radiomics nomogram (DLRN) to differentiate between tuberculous spondylitis (TS) and pyogenic spondylitis (PS) using contrast-enhanced MRI (CE-MRI).
METHODS: A retrospective approach was employed, enrolling patients diagnosed with TS or PS based on pathological examination at two centers. Clinical features were evaluated to establish a clinical model. Radiomics and deep learning (DL) features were extracted from contrast-enhanced T1-weighted images and subsequently fused. Following feature selection, radiomics, DL, combined DL-r…

BACKGROUND: Self-management programs have demonstrated significant health benefits in people with musculoskeletal diseases.
AIM: To examine the benefits of a tailored ankylosing spondylitis self-management program (ASSMP) delivered by trained health professionals for people with ankylosing spondylitis (AS) relative to health status, quality of life and disease activity.
METHODS: ASSMP was developed within a continuous quality improvement framework following a needs assessment and focus group discussions. Formal feedback from the group after each 6 week program cycle group by questionnaire he…

Axial spondyloarthritis (axSpA) is an autoimmune disease primarily affecting the axial skeleton, with associated extra-musculoskeletal manifestations. Treatment strategies targeting cytokines tumor necrosis factor-alpha (TNF-α) and interleukin 17 (IL-17) have proven effective. However, paradoxical reactions, including paradoxical psoriasis and arthritis, have been reported in axSpA patients receiving TNF-α inhibitors. IL-17 inhibitors have been used as an alternative treatment option, but paradoxical reactions have also been rarely observed. This case report presents a 45-year-old man with axS

BACKGROUND: Pyogenic spondylitis (PS) and tuberculous spondylitis (TS) present with significant clinical overlap, posing a major diagnostic challenge. We aimed to develop and validate an imaging-based nomogram integrating CT and MRI features to accurately differentiate PS from TS.
METHOD: We conducted a multicenter retrospective study including 539 patients with spinal infections (251 PS, 288 TS) diagnosed between June 2021 and May 2025. Patients were divided into training (n = 427) and external validation (n = 112) cohorts. Imaging features were screened using univariate logistic regression.…

OBJECTIVE: Opioid analgesics may be prescribed to ankylosing spondylitis (AS) patients with pain that is unresponsive to antirheumatic treatment. Our study assessed factors associated with opioid usage in AS.
METHODS: A prospective cohort of 706 patients with AS meeting modified New York criteria followed at least 2 years underwent comprehensive clinical evaluation of disease activity and functional impairment. These were assessed by the Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) and Bath Ankylosing Spondylitis Functional Index (BASFI). Radiographic severity was assessed by t…

PURPOSE: Differentiating between pyogenic (PS) and brucellar (BS) spondylitis is clinically challenging due to their similar clinical symptoms, with delayed diagnosis or misdiagnosis common, causing trouble for surgeons in selecting appropriate treatment strategies. Currently, radiology-based diagnostic models for PS and BS are lacking. This study aimed to combine magnetic resonance (MR) and radiographic imaging to elucidate the differences between PS and BS and develop a novel diagnostic model for differential diagnosis.
METHODS: We collected and analyzed the differences between MR and radio…
