BACKGROUND AND OBJECTIVE: To report the results of combined cataract and strabismus surgery using an adjustable suture technique in patients of different ages.

The incidence of amblyopia in the normal population is 2-6%, whereas among patients with congenital ptosis it can be as high as 50%. We reviewed 146 cases of congenital ptosis in patients aged between 5 months and 15 years and compared them to a control group of 34 age- and sex-matched patients. In 78 children (156 eyes) reliable optotype visual acuity could be obtained. Fifty-three eyes (34%) were amblyopic. We studied all cases of strabismus surgery performed at the Manhattan Eye, Ear and Throat Hospital between January 1, 1981 and June 30, 1986. Of the procedures, 106 (5.4%) were performed on patients over the age of 60 years. Strabismus in patients over age 60 represents a category of diseases which are different from those of childhood. We performed 9 to 12 mm of recession of the superior oblique tendon for A-pattern strabismus in 10 patients. The average preoperative A-pattern measured 29.4 prism dioptres (PD), and the average pattern correction was 29.3 PD. All patients had a residual pattern of 6 PD or less (average 2.3 PD). Examination of the oculomotor apparatus in 130 patients after scleroplastic operations and in 65 patients with vertical squint, forced position of the head, by means of the method of coordimetry and a proposed Parks' (in Hardesty's modification) test "three steps" has shown this test to be valuable as a simple, requiring no technical equipment and accessible to practitioners, method of diagnosing vertical squint in children and adults. It is shown that this test can be computerized and assessed by means of microcalculators.

A series of 83 patients had vertical displacement of the horizontal rectus muscle insertions monocularly along with recession-resection surgery to correct comitant, vertical strabismus coexisting with a horizontal deviation. We reviewed retrospectively the records of 17 patients with incomitant vertical diplopia secondary to an underacting inferior rectus muscle who had been treated by surgically weakening the contralateral inferior rectus muscle with a posterior fixation suture (fadenoperation). Inasmuch as surgery is often suggested as the primary treatment for intermittent exotropia, we undertook an extensive literature search to ascertain the outcome of this treatment. Surprisingly, only 22 papers were located which gave presurgical and postsurgical results for intermittent exotropia using reasonably clear success criteria.
