BACKGROUND: It is documented that children who stutter, as well as children with autism, may experience diminished well-being relative to children without these conditions. In contrast, almost nothing is documented about well-being of children living with both conditions, most likely because such children are relatively rare and thus challenging to study as a group. The study's aim was to begin documenting well-being factors related to quality of life in children who both stutter and have autism by evaluating how parents/guardians characterized their children on a brief questionnaire administe

INTRODUCTION: Stuttering, a speech disorder marked by disruptions in fluency, can be developmental or acquired. Acquired stuttering often stems from neurological causes, psychological trauma or certain medications, with antipsychotics such as clozapine implicated in several cases. Clozapine, particularly at higher doses, has been associated with dose-dependent stuttering although the precise mechanisms remain uncertain. While clozapine's effects dopamine pathways and seizure thresholds are proposed mechanisms, movement disorders such as focal dystonia and prior extrapyramidal symptoms (EPSEs) …

PURPOSE: Difficulty discarding irrelevant negative stimuli activated in working memory has been linked to increased rumination. Adults who stutter (AWS) report (a) rumination about communication events, and (b) difficulties during tasks that require manipulation of information held in working memory compared to non-stuttering adults (AWNS). Given the link between rumination and working memory, this study examined whether AWS discard negative stimuli faster or slower compared to AWNS, and whether speed of discarding negative information is linked to generalized rumination.
METHOD: To assess th…

PURPOSE: Stuttering may disrupt the speech of individuals with Down syndrome (DS), but standard stuttering therapies may be less adapted to these clients' needs. This study examined if their strength in gesture use can lead to the development of a new stuttering therapy.
METHOD: Eighteen individuals with DS who stutter participated in an experimental task. During this task, they produced sentences in three different conditions: once without the ability to use gestures, once while moving the mouth of a hand puppet synchronous with their speech, and once while making beat gestures along their s…

BACKGROUND: Evidence shows that neurotypical individuals who stutter use fewer gestures than those who do not stutter. Presently, no research exists about the interaction of stuttering and gestures in individuals with Down syndrome.
METHOD: Twenty-nine individuals with Down syndrome (7-19 years) of whom 16 stuttered and 13 spoke fluently and 20 neurotypical children (3-10 years) of whom 8 stuttered and 12 spoke fluently participated in this study. In spontaneous speech transcriptions, stuttering events and gestures were coded.
RESULTS: Comparisons of gesture frequency during stuttered and fl…

Cerebellar-cortical loops comprise critical neural circuitry that supports self-initiated movements and motor adjustments in response to perceived errors, functions that are affected in stuttering. It is unknown whether structural aspects of cerebellar circuitry are affected in stuttering, particularly in children close to symptom onset. Here we examined white matter diffusivity characteristics of the three cerebellar peduncles (CPs) based on diffusion MRI (dMRI) data collected from 41 children who stutter (CWS) and 42 controls in the 3-11 years range. We hypothesized that CWS would exhibit de…

BACKGROUND: Awareness of stuttering is likely to depend upon the development of the metalinguistic skill to discriminate between fluent speech and stuttering and the ability to identify one's own speech as fluent or stuttered. Presently, little is known about these abilities in individuals with Down syndrome (DS).
PURPOSE: This study investigates whether individuals with DS and typically developing (TD) children who stutter and who do not stutter differ in their ability to discriminate between fluent speech and stuttering. The second purpose of this study is to discover if this ability is cor…

PURPOSE: Stuttering during adulthood is associated with a heightened rate of anxiety disorders, especially social anxiety disorder. Given the early onset of both anxiety and stuttering, this comorbidity could be present among stuttering children.
METHOD: Participants were 75 stuttering children 7-12 years and 150 matched non-stuttering control children. Multinomial and binary logistic regression models were used to estimate odds ratios for anxiety disorders, and two-sample t-tests compared scores on measures of anxiety and psycho-social difficulties.
RESULTS: Compared to non-stuttering contr…

PURPOSE: Despite the greatly increased risk of social anxiety disorder in adults who stutter, there is no clear indication of the time of on … school-age girls, 41 adolescent boys and nine adolescent girls who were seeking treatment for their stuttering. Participants also rated the severity of their own stuttering.
RESULTS: All mean scaled scores on the four RCMAS subscales and Total Anxiety scores were within normal limits. However, for both groups of boys, scores on the Lie Scale were significan …

In the past two decades, neuroimaging investigations of stuttering have led to important discoveries of structural and functional brain differences in people who stutter, providing significant clues to the neurological basis of stuttering. One major limitation, however, has been that most studies so far have only examined adults who stutter, whose brain and behavior likely would have adopted compensatory reactions to their stuttering; these confounding factors have made interpretations of the findings difficult. Developmental stuttering is a neurodevelopmental condition, and like many other ne…

PURPOSE: Those with anxiety use safety behaviors when attempting to prevent negative outcomes. There is evidence that these behaviors contribute to the persistence of anxiety disorders. Safety behaviors have been prominent in the cognitive behavior therapy literature during the last decade, particularly with social phobia management. However, nothing is known of safety behavior use by those who stutter. This is surprising given the high prevalence of social phobia in the stuttering population who seek clinical help.
METHOD: Clinical psychologists and speech-language pathologists (SLPs) create…

BACKGROUND: Treatment of adolescents who stutter is an under-researched area that would benefit from greater attention.
AIMS: To investigate whether an intensive treatment programme for older teenagers who stutter, aged over 16 years of age, is effective in reducing overt and covert aspects of stuttering.
METHODS & PROCEDURES: A repeated-measures, single-subject experimental design was replicated across participants. The study consisted of a 5-week baseline phase, 2-week intensive treatment phase, 5-week consolidation phase and 10-month follow-up phase. Participants were asked to make ten vi…

This article explores the effect that words from different lexical categories have on disfluency in 12 Persian children, ten boys and two girls, who stutter. They were aged 7 years 5 months to 10 years 6 months. Words from the participants' narrative and reading samples (sub-tests of the Reading and Dyslexia Test validated for Persian school-aged children) were categorized as content, function, or content-function, and stuttering-like disfluencies were coded in each speech sample. Content and content-function words were significantly more likely to show stuttering-like disfluencies than functi…

The aim of the study was to evaluate the role of steady-state and dynamic visual gestures of vowels in stuttering inhibition. Eight adults who stuttered recited sentences from memory while watching video presentations of the following visual speech gestures: (a) a steady-state /u/, (b) dynamic production of /a-i-u/, (c) steady-state /u/ with an accompanying audible 1 kHz pure tone, and (d) dynamic production of /a-i-u/ with an accompanying audible 1 kHz pure tone. A 1 kHz pure tone and a no-external signal condition served as control conditions. Results revealed a significant main effect of au…

OBJECTIVE: To report the first known case of a lithium-exacerbated stutter in a pediatric patient.
CASE SUMMARY: A 10-year-old male with a history of developmental stuttering, bipolar disorder not otherwise specified (NOS), attention-deficit/hyperactivity disorder, and conduct disorder was admitted to the psychiatric hospital because of recurrent suicidal ideations and increased physical aggression toward staff at his residential facility. The patient was being treated with lithium at initial dose of 150 mg/day at bedtime for bipolar disorder NOS. When the lithium dose was increased to 900 mg…

School-based speech-language pathologists are often called upon to treat children who stutter, though many clinicians have reported that they feel uncomfortable working with this population. Fortunately, there is much that speech-language pathologists can do to help children who stutter speak more easily and minimize the adverse impact of stuttering in both academic and social settings. The purpose of this article is to provide clinicians with a guide to some of the key issues they should consider when working with school-aged children who stutter. The goal is to encourage clinicians to develo…

OBJECTIVE: The validity of a simple and not time-consuming self-assessment (SA) Scale was tested to establish progress after or during stuttering therapy.
PATIENTS AND METHOD: The scores on the SA scale were related to (1) objective measures (percentage of stuttered syllables, and syllables per minute) and (2) (self-)evaluation tests (self-evaluation questionnaires and perceptual evaluations or judgments of disfluency, naturalness and comfort by naïve listeners). Data were collected from two groups of stutterers at four measurement times: pretherapy, posttherapy, 12 months after therapy and 2…

It is known that the speech of people who stutter improves when the speaker's own vocalization is changed while the participant is speaking. One explanation of these effects is the disruptive rhythm hypothesis (DRH). The DRH maintains that the manipulated sound only needs to disturb timing to affect speech control. The experiment investigated whether speech that was gated on and off (interrupted) affected the speech control of speakers who stutter. Eight children who stutter read a passage when they heard their voice normally and when the speech was gated. Fluency was enhanced (fewer errors we

Though stuttering is manifest in its motor characteristics, the cause of stuttering may not relate purely to impairments in the motor system as stuttering frequency is increased by linguistic factors, such as syntactic complexity and length of utterance, and decreased by changes in perception, such as masking or altering auditory feedback. Using functional and diffusion imaging, we examined brain structure and function in the motor and language areas in a group of young people who stutter. During speech production, irrespective of fluency or auditory feedback, the people who stuttered showed o…
