Blood as tracer for CSF circulation after subarachnoidal hemorrhage.

The amount and distribution of blood in the cerebrospinal fluid following subarachnoid hemorrhage can be monitored with CT. An investigation of 110 patients was performed retrospectively in 100 patients with a total of 132 CT examinations, and prospectively in 10 patients with 40 CT examinations. During the days following the hemorrhage, the blood was redistributed within the subarachnoid space and eventually reabsorbed. The blood in the basal cisterns was cleared first, whereas the amount of blood in the cortical sulci appeared to increase after a few days, later to be cleared. It is conclude…

Cerebrospinal Fluid Pressure Measurement and Infusion Studies Using Lumbar Puncture.

Lumbar puncture provides an easy way of accessing the subarachnoid space. Measuring of the opening cerebrospinal fluid pressure is the most commonly used method of evaluating intracranial pressure but provides basic snapshot information only. Further insights into cerebrospinal fluid dynamics can be obtained through infusion studies, which rely on measurement of the degree of pressure change in response to addition of fluid volume into the subarachnoid space. The authors describe applications of these 2 techniques pertinent to a practicing neuroradiologist, who may be asked to assist with fluo…

Computerized axial tomography for the diagnosis of cerebral cysticercosis.

Computerized Axial Tomography (CAT) has proved extremely useful for the diagnosis of cerebral cysticercosis. The calcified small, multiple, and scattered cysts provide a typical image on CAT. The collection of non-calcified cysts in the subarachnoid spaces (racemose form) or in the ventricles may produce areas of low density similar to that of the cerebrospinal fluid. The dilatation of the ventricular system, extreme degrees of hydrocephalus, areas of cerebral atrophy, and other related changes induced by the cysts in the subarachnoid spaces are also clearly shown in the CAT. Four personal cas…

Rebalancing translaminar pressure difference: a novel glaucoma surgery-a pilot trial in non-human primates.

AIM: To propose a novel glaucoma surgery for rebalancing translaminar pressure difference. METHODS: Three non-human primates with normal eyes and two with laser-induced glaucoma underwent the novel surgical procedure. Cannulation of the subarachnoid space was performed after completion of routine vitrectomy steps. An XEN 45 implant was inserted into the created puncture to communicate between the vitreous body and subarachnoid space. Intraocular pressure (IOP), fundus photography, and spectral-domain optical coherence tomography were assessed at baseline and regular intervals during follow-up…

Early prediction of clinical outcomes in patients with aneurysmal subarachnoid hemorrhage using computed tomography texture analysis.

Radiological evaluation of subarachnoid hemorrhage (SAH) is often subject to interobserver variability. The aim of this study was to retrosp … vailable software program (Synapse Vincent). The following texture parameters of blood clots in the subarachnoid space and cerebral edema were assessed: mean CT value, entropy, skewness, and kurtosis. The mean CT value of blood clots in the subarachnoid space was significantly associated with cerebral vasospasm, DCI, and functional outcome. The mean CT value ≥ 49.64 …

Spinal neurocysticercosis.

Worldwide, cysticercosis is the most common parasitic infection of the central nervous system. In endemic regions, the incidence of neurocys … e spinal canal, the mass effect of these lesions is poorly tolerated. Most spinal NCC occurs in the subarachnoid space where mass effect can cause spinal cord compression, although obstruction of cerebrospinal fluid pathways due to scarring of the subarachnoid space can also cause symptoms. The authors treated six patients with spinal NCC. In five cases the lesions …

Acute Multidisciplinary Management of Aneurysmal Subarachnoid Hemorrhage (aSAH).

Aneurysmal subarachnoid hemorrhage is a life-threatening, neurological emergency characterized by accumulation of blood in the subarachnoid space due to a ruptured aneurysm. Over the past several decades, improvements in the clinical management of aneurysmal subarachnoid hemorrhage have led to better patient outcomes. However, aneurysmal subarachnoid hemorrhage is still associated with high morbidity and mortality. During the acute phase of aneurysmal subarachnoid hemorrhage and prior to the definitive management of the aneurysm, numerous medical emergencies, such as elevated intracranial pres…

The influence of dexamethasone on subarachnoid fibrosis after subarachnoid haemorrhage. Scanning electron microscopic study in the dog.

The author used the scanning electron microscope (SEM) to study the influence of dexamethasone on subarachnoid fibrosis after experimental subarachnoid haemorrhage (SAH). In the subchronic group of dogs, which were sacrificed three weeks after SAH, an average of 2.82 mg/kg intrathecal dexamethasone seemed to delay the fibrosis somewhat, but statistically showed no significant difference from the control group. In the chronic group, sacrificed three months after SAH, an average of 0.95 mg/kg intrathecal dexamethasone did not prevent the fibrosis, and no significant difference was found from the

Diffusion-weighted magnetic resonance imaging of cerebrospinal fluid in patients with and without communicating hydrocephalus.

BACKGROUND: Recent concepts about cerebrospinal fluid (CSF) circulation in communicating hydrocephalus (CoHy), which is also termed "restricted arterial pulsation hydrocephalus," suggest reduced arterial pulsations of subarachnoid vessels with a smaller amount of CSF shifted in subarachnoid spaces during the early systole. The postulated restriction of subarachnoid arterial pulsations in CoHy should induce a smaller motion artifact and reduced local stream effects in CSF in magnetic resonance (MR) diffusion-weighted imaging (DWI). PURPOSE: To investigate the maximum diffusivity in CSF in pati…

Leptomeningeal melanomatosis with multiple cutaneous pigmented nevi: tumor cell proliferation and malignant transformation in an autopsy case.

We experienced a rare case of leptomeningeal melanomatosis. The proliferative activity and nuclear accumulation of p53 in this tumor were examined, since the relationship between this tumor type and growth has not yet been elucidated. A 33-year-old Japanese man was shown to have leptomeningeal melanomatosis with multiple cutaneous pigmented nevi. The autopsy findings showed the presence not only of benign diffuse melanosis of the leptomeninges but also of leptomeningeal melanomatosis in the subarachnoid space and brain parenchyma. In the brain parenchyma, the direct invasion of tumor cells fro…

Cerebrospinal fluid pathways from cisterns to ventricles in N-butyl cyanoacrylate-induced hydrocephalic rats.

OBJECT: Cerebrospinal fluid typically enters the subarachnoid space from the ventricles via the fourth ventricular foramina. However, there is clinical evidence that CSF also flows in the opposite direction. Ventricular reflux of CSF from a cistern is a well-known phenomenon in radioisotope studies in patients with normal-pressure hydrocephalus. Additionally, the presence of ventricular blood in acute subarachnoid hemorrhage is frequently observed. The goal of this investigation was to examine the potential CSF pathways from cisterns to ventricles. The authors examined pathways in rat models i…

The oil myelogram after operation for lumbar disc lesions.

Post-operative myelograms in patients with renewed or persistant pain in the back or sciatica after lumbar disc surgery were reviewed in an attempt to differentiate between recurrent disc prolapse and other changes, particularly scarring and inflammation of the meninges. The most useful sign of recurrent disc prolapse was anterior indentation of the iophendylate (Myodil) column opposite a disc space. Inflammatory changes in the meninges were indicated by loculation of the subarachnoid space, matting or marked swelling of the nerve roots, irregularity of the theca, and by the absence of a poste…

Changes in the volumes of the brain and cerebrospinal fluid spaces after shunt surgery in idiopathic normal-pressure hydrocephalus.

OBJECTIVES: To investigate volumetric changes of the brain and cerebrospinal fluid (CSF) spaces after shunt surgery in shunt-responsive idiopathic normal-pressure hydrocephalus (iNPH), and correlations between the changes and postoperative clinical improvements. METHODS: Twenty-one patients with shunt-responsive iNPH were studied. Magnetic resonance imaging (MRI) of the brain was performed before and 1year after surgery, and clinical symptoms were assessed by the iNPH Grading Scale, a validated assessment tool of the triad of iNPH, the Modified Rankin Scale, the Timed Up and Go Test, and neur

Contrast extravasation after bilateral inferior petrosal sinus sampling masquerading as venous subarachnoid haemorrhage.

When performed properly, bilateral inferior petrosal sinus sampling (BIPSS) for adrenocorticotropic hormone (ACTH) is rarely associated with complications. Major complications reported to date include thromboembolism, brain stem infarction, pontine haemorrhage, isolated sixth nerve palsy and venous subarachnoid haemorrhage. We describe a rare case where a predominant contrast extravasation into the subarachnoid space, admixed with a small quantity of venous blood, occurring during BIPSS in a 58-year-old woman with ACTH-dependent Cushing's syndrome, was misinterpreted as venous subarachnoid hae…

External hydrocephalus: radiologic spectrum and differentiation from cerebral atrophy.

External hydrocephalus (EH) is a condition in which infants with rapidly enlarging heads are found to have a CT scan that shows widening of the subarachnoid space with mild or no ventricular dilation. In this study, 74 infants with EH associated with a variety of conditions were examined clinically and with CT scans to identify the radiologic features of the condition as well as to understand its clinical and radiologic evolution. Some of these CT scans were compared with those of patients with cerebral atrophy. The characteristic CT findings in EH were bifrontal widening of the subarachnoid s…

[Infratentorial superficial siderosis : a diagnostic challenge from the general practice to the hospital].

Superficial siderosis is a rare neurological evolutive condition that encompasses two distinct entities classified according to their cortical or infratentorial localisation. The disease is caused by the neuronal destruction of the central nervous system linked with hemosiderin deposits. The aetiology is a sustained low-intensity bleeding located at the supratentorial subarachnoid space for the cortical superficial siderosis or from the subarachnoid space for the infratentorial superficial siderosis. The triad sensorineural hearing loss, cerebellar ataxia and pyramidal signs should suggest inf…

Cerebral subarachnoid blood migration consecutive to a lumbar haematoma after spinal anaesthesia.

We report the case of a woman who received spinal anaesthesia for peripheral vascular surgery of the lower limbs and subsequently developed a spinal subarachnoid haematoma. Interestingly, low back pain was the only symptom of this spinal subarachnoid haemorrhage. During the following days, blood migrated from the spinal haematoma towards the cerebral subarachnoid spaces. The patient presented with stupor, nausea and vomiting that resolved within 2 weeks with conservative treatment.

Giant aneurysms: surgical treatment.

We have reviewed our experience with the surgical management of giant aneurysms of the cerebral circulation. Patients with aneurysms at the base of the skull, below the subarachnoid space, were treated by proximal ligation and arterial bypass with trapping intracranially if necessary. Patients harboring giant aneurysms located on arteries within the subarachnoid space were treated by direct attack on the aneurysm aided by hypothermia and elective circulatory arrest. Elective circulatory arrest allows the surgeon more control in dissecting out and obliterating giant aneurysms. We were able to e…
