Synthetic mesh versus biological mesh to prevent incisional hernia after loop-ileostomy closure: a randomized feasibility trial.

BACKGROUND: Incisional hernia is a frequent complication after loop-ileostomy closure, rationalizing hernia prevention. Biological meshes have been widely used in contaminated surgical sites instead of synthetic meshes in fear of mesh related complications. However, previous studies on meshes does not support this practice. The aim of Preloop trial was to study the safety and efficacy of synthetic mesh compared to a biological mesh in incisional hernia prevention after loop-ileostomy closure. METHODS: The Preloop randomized, feasibility trial was conducted from April 2018 until November 2021

Chronic Foreign Body Inflammatory Response in Textiloma Formation: Case Report and Analysis of Current Evidence.

A gossypiboma (GPB), also referred to as a textiloma (TXT), is a mass formed as a result of the body's inflammatory and granulomatous response to retained surgical textile material (RSTM), most commonly gauze or mesh fibers. Its reported incidence ranges from 1 in 1,000 to 1 in 10,000 surgical procedures, although the true frequency is believed to be significantly higher due to underreporting related to medicolegal concerns. This entity is clinically important because it may mimic more common postoperative complications such as abscess (ABS), seroma (SER), or mesh infection (MI), frequently le

Duramesh registry study: short-term outcomes using mesh suture for abdominal wall closure.

INTRODUCTION: Sutures are flexible linear elements that join tissue and maintain their hold with a surgeon-created knot. Tension at the suture/tissue interface can cut the very tissues that sutures are designed to hold, leading to dehiscence and incisional hernia formation. A new suture design (Duramesh, Mesh Suture Inc., Chicago, IL) was approved for marketing by the United States Food and Drug Administration in September 2022. The multiple filaments of the mesh suture are designed to diffuse tension at the suture/tissue interface thereby limiting pull-through. The macroporosity and hollow co

Host-biomaterial interactions in mesh complications after pelvic floor reconstructive surgery.

Polypropylene (PPL) mesh is widely used in pelvic floor reconstructive surgery for prolapse and stress urinary incontinence. However, some women, particularly those treated using transvaginal PPL mesh placement for prolapse, experience intractable pain and mesh exposure or extrusion. Explanted tissue from patients with complications following transvaginal implantation of mesh is typified by a dense fibrous capsule with an immune cell-rich infiltrate, suggesting that the host immune response has a role in transvaginal PPL mesh complications through the separate contributions of the host (patien

Robotic transabdominal preperitoneal Morgagni hernia repair: surgical technique.

A Morgagni hernia is a rare congenital anterior diaphragmatic hernia that is often diagnosed in adults and requires surgical repair in order to avoid complications. A preoperative computerized tomography (CT) scan is recommended to assess the size of the defect and the contents of the hernia. There are various transabdominal and transthoracic surgical options for repairing these hernias but minimally invasive transabdominal approaches are usually recommended, with robotic surgery gaining popularity. The wristed instruments of the robotic platform facilitate the surgery, by allowing increased d

Transvaginal repair of genital prolapse: preliminary results of a new tension-free vaginal mesh (Prolift technique)--a case series multicentric study.

Our goal was to report the preliminary results of a transvaginal mesh repair of genital prolapse using the Prolift system. This retrospective multicentric study includes 110 patients. All patients had a stage 3 (at the hymen) or stage 4 (beyond the hymen) prolapse. Total mesh was used in 59 patients (53.6%), an isolated anterior mesh in 22 patients (20%) and an isolated posterior mesh in 29 patients (26.4%). We report one bladder injury sutured at surgery and two haematomas requiring secondary surgical management. At 3 months, 106 patients were available for follow-up. Mesh exposure occurred i

Biomechanical Modeling of Prosthetic Mesh and Human Tissue Surrogate Interaction.

Surgical repair of hernia and prolapse with prosthetic meshes are well-known to cause pain, infection, hernia recurrence, and mesh contraction and failures. In literature, mesh failure mechanics have been studied with uniaxial, biaxial, and cyclic load testing of dry and wet meshes. Also, extensive experimental studies have been conducted on surrogates, such as non-human primates and rodents, to understand the effect of mesh stiffness, pore size, and knitting patterns on mesh biocompatibility. However, the mechanical properties of such animal tissue surrogates are widely different from human t

Comparative outcomes of biologic versus synthetic mesh in inguinal hernia repair: A systematic review and meta-analysis.

BACKGROUND: The comparative effectiveness of biologic versus synthetic mesh in inguinal hernia repair remains uncertain, with existing studies reporting inconsistent findings. We aimed to report a meta-analysis of randomized controlled trials comparing the outcomes of synthetic and biologic mesh in open and laparoscopic inguinal hernia repair. METHODS: A literature search of PubMed, Embase, Web of Science, and EBSCO databases was performed to identify randomized controlled trials comparing biologic and synthetic mesh in adult patients undergoing open or laparoscopic inguinal hernia repair. Mesh augmentation during pelvic-floor reconstructive surgery: risks and benefits.

PURPOSE OF REVIEW: Synthetic meshes are increasingly used in the surgical management of stress urinary incontinence and pelvic-organ prolapse in an attempt to improve success rates and increase the longevity of repairs. This review describes and analyses complications following pelvic-floor procedures employing synthetic meshes. RECENT FINDINGS: Type I monofilament polypropylene mesh with a large pore size is currently the mesh of choice. Chronic inflammation is a typical host response, whereas acute inflammation and predominant CD20+ lymphocyte infiltration represent an adverse host reaction

Laparoscopic removal of mesh used in pelvic floor surgery.

Various meshes are being used widely in clinical practice for pelvic reconstructive surgery despite the lack of evidence of their long-term safety and efficacy. Management of complications such as mesh erosion and dyspareunia can be challenging. Most mesh-related complications can probably be managed successfully via the transvaginal route; however, this may be impossible if surgical access is poor. This case report demonstrates the successful laparoscopic removal of mesh after several failed attempts via the vaginal route.

Laparoscopic transabdominal preperitoneal approach for umbilical hernia with rectus diastasis.

INTRODUCTION: Rectus diastasis, when coexistent with umbilical hernia, can benefit from mesh-based repair of the midline. Laparoscopic correction of an umbilical hernia involves the placement of a mesh in the peritoneal cavity, but this comes with the risk of bowel complications. However, newly developed dual-sided composite meshes have helped to reduce this risk. MATERIALS AND SURGICAL TECHNIQUE: Four men and three women with umbilical hernia and rectus diastasis were treated with laparoscopic transabdominal preperitoneal repair. Composite mesh with a hydrophilic 3-D polyester textile on the

Scar endometriosis developing after an umbilical hernia repair with mesh.

A 44-year-old female was initially evaluated for a 3-cm umbilical hernia, which developed after a laparoscopic myomectomy performed seven years prior. The umbilical hernia was repaired using a synthetic mesh. Eight months after the umbilical hernia repair, the patient returned with chronic pain in a 3-cm raised mass originating from the umbilical hernia repair incision. The mass and mesh were surgically removed. The umbilical fascial defect was repaired with a primary fascia-to-fascia closure and the umbilicus was reconstructed from adjacent skin. The mass was found histologically to be endome

Management of Synthetic Mesh Infection With Open Resection and Laparoscopic Ventral Hernia Repair Using Biosynthetic Prosthetic Material.

Ventral hernia repair (VHR) is a common surgical operation with the prevailing complication of hernia recurrence. The development of diverse mesh materials has led to a lower hernia recurrence rate but poses concern for mesh infection. Featured is the case study of a patient who underwent laparoscopic recurrent VHR with utilization of biosynthetic mesh and removal of prior permanent synthetic mesh. During the post-operative course, the patient developed a seroma, cellulitis, and wound dehiscence. Despite these complications, the biosynthetic mesh was salvaged using conservative measures. The p

Absorbable mesh and topical negative pressure therapy for closure of abdominal dehiscence with exposed bowel.

Permacol mesh is a porcine dermis-derived biomaterial used for the repair of abdominal incisional and inguinal hernia. It has been found to be a safe and effective alternative to non-absorbable mesh for application over exposed bowel. This mesh has also been successfully applied over contaminated abdominal wound beds and around stomas. Topical negative pressure therapy has been used for the management of wound complications after surgical implantation of Permacol mesh. We describe our experience with the combined use of Permacol mesh and simultaneous application of topical negative pressure th

Mesh infection in cases of polypropylene mesh hernioplasty.

BACKGROUND: Mesh hernioplasty is the most common surgery for hernia repair with Polypropylene mesh as the preferred one. But the use of synthetic prosthesis is associated with the inherited risk of developing a prosthetic infection that may manifest even after a very long time as subtle symptoms in the form of fever, myalgia, erythema of overlying skin and in some cases as chronic discharging sinus. With this in view a systematic prospective observational study has been carried out to estimate the incidence of mesh infections, to identify the type of associated organism and to study the outcom
