Extensive damage of the tibialis anterior tendon is rare and mainly caused by trauma. Surgical treatment of these injuries can become challenging owing to the limited availability of autogenous graft resources for reconstruction of the defect. In the present case report, we describe a large defect in the midfoot soft tissue after a traffic injury, which included complete loss of the tibialis anterior tendon. The tendon was reconstructed by split tendon transfer of the tibialis posterior tendon without sacrificing function, which was confirmed by the follow-up examination at 6 years after injur

BACKGROUND: Rotator cuff tears are common and not well-understood shoulder problems. Structural and mechanical inhomogeneity of the supraspinatus tendon complicates accurate prediction of risk of tear propagation and may affect appropriate clinical treatment.
HYPOTHESIS: We propose that interactions between the supraspinatus and infraspinatus tendons are critical to load bearing at the glenohumeral joint and warrant investigation.
STUDY DESIGN: Controlled laboratory study.
METHODS: Principal strains in the infraspinatus tendon of cadaveric human shoulders were evaluated with increasing ante…

Repair of chronic Achilles tendon rupture is a surgical challenge. We describe the use of a free turndown tendon flap augmentation raised from the proximal gastrocnemius aponeurosis. To control optimal tension or the reconstructed Achilles tendon length, we used an original method by referring to the gravity planter flexion ankle angle of the contralateral limb. Key aspects of the technique are described. A retrospective analysis of the short-term outcomes achieved in a case series (n = 56) is presented. The postoperative anthropometric findings are also presented to indicate the successful ou

BACKGROUND: There is a need for a valid, reliable, and easily applicable clinical measure of the length of the Achilles tendon (AT) after rupture. This study examines the reliability of a new ruler based measurement, the Achilles Tendon Length Measure (ATLM) in comparison with the goniometer-based Achilles Tendon Resting Angle (ATRA).
METHODS: Measurements were performed by two independent physiotherapists eight weeks after AT rupture on 28 patients treated non-operatively.
RESULTS: The mean (SD) injured ATLM was 56.5 (2.3)cm, ICC2.1 0.91(CI [0.72-0.97]), SEM 0.7cm (SEM% 1.2), MDC 1.9cm (MDC

Although pre- and postoperative imaging of Achilles tendon rupture (ATR) has been well documented, radiographic evaluations of postoperative … nt measurements were not significantly different in the proximal and distal regions of the ruptured tendons compared with the healthy tendons. The present study used DTI and fiber tracking to demonstrate the radiologic properties of postoperative Achilles tendons with respect to trajectory and tendinous fiber continuity. BACKGROUND: Patients with Achilles tendinopathy and Achilles tendon rupture experience functional challenges during and beyond rehabilitation. The objective of this study was to combine advanced methods including shear wave tensiometry and ultrasound imaging to assess symptoms and tendon loading during gait to better understand the root cause of these injury-specific impacts.
METHODS: 15 individuals (five with Achilles tendinopathy, five with a previous Achilles tendon rupture, five uninjured) participated. Patient reported outcome measures were used to assess injury severity, pain, function,

This review describes a case of chronic post-traumatic foot drop managed with posterior tibial tendon transfer through the interosseous membrane and percutaneous Achilles tendon lengthening. Key technical considerations and postoperative rehabilitation principles are highlighted. This approach provides a durable solution for restoring ankle dorsiflexion and improving ambulation in patients with irreversible peroneal nerve injury.

PURPOSE: To highlight the spectrum of pathology and patterns of gluteus minimus tendon tearing observed on MR imaging of the hip.
METHODS AND MATERIALS: Retrospective review of consecutive hip MRI exams with findings of gluteus minimus tendon (GMin) pathology. A total of 194 exams in 178 patients (148 female, mean age 61) were reviewed. MRI exams are assessed for GMin: tendinopathy, partial, or complete tendon tears. GMin muscular fatty atrophy, enthesopathic cortical irregularities of the greater trochanter (GT), and peri-trochanteric edema or bursal fluid collections were assessed in all ca

BACKGROUND: To summarize available evidence and determine if tendon allograft is an effective treatment for chronic Achilles tendon rupture.
METHODS: A search was performed in the PubMed, Web of Science, Embase and Cochrane Database from 1960 to April 2017 to identify relevant articles. Predefined inclusion and exclusion criteria were applied to identify all eligible articles.
RESULTS: Total 186 articles were identified through our systematic search. Of these, 9 publications met the inclusion criteria. Five studies were case reports; three were case series; and one were expert opinion. Of a

BACKGROUND: Flexor hallucis longus (FHL) tendon rupture is a challenging injury to lead with clawing of the great toe when the FHL tendon is repaired too tight. When the diagnosis is delayed, the tendon ends may not be opposable because of contracture or poor tendon tissue.
METHODS: A technique to reconstruct FHL tendon rupture without a free tendon graft is described. A split tendon lengthening is performed at the midfoot around the knot of Henry. Ankle block anesthesia is used to allow the patient's active movement of the interphalangeal (IP) joint to determine the appropriate length of the

Combined flexor hallucis longus tendon transfer and bone-tendon allograft transplantation is a reasonable option for advanced distal-segment Achilles tendinopathy. This procedure provides anatomic restoration and improved function of the posterior muscle group without sacrificing the regional anatomy. Allograft transplantation is safe and does not require immunosuppressive therapy. The soft tissue envelope should be healthy because wound complications can be an issue. This procedure is especially helpful in patients with significant disability.

BACKGROUND: Chronic rupture of the Achilles tendon (delayed diagnosis of more than 4 weeks) can result in retraction of the tendon and inadequate healing. Direct repair may not be possible and augmentation methods are challenging when the defect exceeds 5-6 cm, especially if the distal stump is grossly tendinopathic.
METHODS: We describe our method of Achilles tendon reconstruction with ipsilateral semitendinosis autograft and interference screw fixation in a patient with chronic rupture, a 9 cm defect and gross distal tendinopathy.
RESULTS: Patient reported outcome measures consistently dem

Early flexor tendon healing is characterized by peak cellular apoptosis of both inflammatory and tendon cells in the first week, followed by progressively greater tenocyte proliferation in the second and third weeks. Tenocyte apoptosis is a predominant event, but proliferation of tenocytes is minimal in the middle and late healing periods. Edematous subcutaneous tissues, edema of the tendon, the intact annular pulleys, and extensor tendons all greatly contribute to the resistance. Careful consideration of the contributing factors and dynamics offers insight into strategies to reduce repair rup…

BACKGROUND: Chronic tears of the Achilles tendon with a tendon gap exceeding 6 cm are a surgical challenge. The purpose of this study is to report the long-term results of reconstruction of such chronic Achilles tendon ruptures with use of a free autologous gracilis tendon graft.
METHODS: Twenty-one patients underwent reconstruction of a chronic rupture of the Achilles tendon. Fifteen patients were available for clinical and functional assessment on the basis of anthropometric measurements, isometric strength testing, and the Achilles Tendon Total Rupture Score after a mean duration of follow
