We report a case of an immunocompetent individual who was diagnosed with fungal tenosynovitis of the wrist caused by Cyberlindnera rhodanensis, a yeast in the Saccharomycetales order that has never been reported to cause infections in humans. The yeast grew in routine culture and was sent to a reference laboratory for identification and susceptibility testing. The patient ultimately received six surgeries and five months of antifungal therapy, including micafungin and voriconazole, before clinical resolution.

Data on the efficacy of treatment modalities in De Quervain's tenosynovitis (DQT) is limited. Paraffin therapy is routinely used in treating hand osteoarthritis and neuropathies. However, there are not enough studies investigating paraffin effectiveness. This study evaluates the effectiveness of paraffin bath therapy on pain, functional status, muscle strength, and quality of life in patients with DQT. This prospective, single-blind, randomized, controlled study enrolled 51 DQT patients. Group 1 (n = 26) received paraffin bath + splint + exercise; Group 2 (n = 25) received splint + exercise th…

BACKGROUND: Nonsurgical management of de Quervain disease relies mainly on the use of oral nonsteroidal antiinflammatory drug administration, splint therapy, and corticosteroid injections. Although the latter is most effective, with documented success rates of 61 to 83 percent, there exists no clear consensus pertaining to conservative treatment protocols conferring the best outcomes. This article reports on all present conservative treatment modalities in use for the management of de Quervain disease and highlights specific treatment- and patient-related factors associated with the best outco

We present a case of carpal tunnel syndrome involving wrist trigger caused by a hypertrophied lumbrical muscle with flexor synovitis. The case was a 40-year-old male heavy manual worker complaining of numbness and pain in the median nerve area. On active flexion of the fingers, snapping was observed at the carpal area, and forceful full grip was impossible. Tinel's sign was positive and an electromyographic study revealed conduction disturbance of the median nerve at the carpal tunnel. Magnetic resonance imaging revealed edematous lumbrical muscle with synovial proliferation around the flexor

Tenosynovitis with psammomatous calcification, described in 1983 by Gravanis and Gaffney, is a distinctive clinicopathologic variant of "idiopathic calcifying tenosynovitis" or "calcific tendonitis." However, tenosynovitis with psammomatous calcification is poorly recognized by pathologists and for this reason has not been adopted widely as a distinct entity. We present the clinicopathologic features of 6 cases of tenosynovitis with psammomatous calcification. Cases involved the tendons, peritendinous soft tissues, and adjacent synovium of the distal extremities (3 fingers, 2 feet, and 1 carpa…

OBJECTIVE: To describe and evaluate a technique for radical resection of the entire intrathecal component of the common digital extensor tendon (CDET) in horses.
STUDY DESIGN: Prospective clinical study.
ANIMALS OR SAMPLE POPULATION: Seven client-owned horses.
METHODS: Seven horses with wounds that resulted in septic tenosynovitis of the CDET sheath were treated by complete surgical resection of the affected tendon and ablation of the tendon sheath; 5 had previous surgery that was unsuccessful. Time from initial injury to complete resection was 1-22 weeks. In 6 horses, the wound was closed …

Trigger finger due to stenosing flexor tenosynovitis distal to the first annular pulley is not a common finding; however, when present, it introduces a diagnostic and therapeutic challenge. Resection of the second annular pulley for release of triggering at this level may leave the patient with impaired function. The pulley can be preserved by removing an elliptical portion of the center of the tendon through a lateral incision.

A 63-year-old man developed symmetrical polyarthritis, subcutaneous nodules at the elbows, olecranon bursitis, and recurrent tenosynovitis. He was later discovered to have idiopathic hemochromatosis. Staining of the subcutaneous nodule revealed iron deposits. These manifestations which are common to rheumatoid arthritis may be seen in hemochromatotic arthropathy.

PURPOSE: Trapeziometacarpal joint replacement is an effective treatment for trapeziometacarpal osteoarthritis, but postoperative de Quervain tenosynovitis remains a concern, with reported incidences of 4-17%. Thumb lengthening has been suggested as contributory. This study evaluated the association between postoperative thumb lengthening and de Quervain tenosynovitis using the M1-M2 arch described by Duché and Trabelsi.
METHODS: A retrospective cohort study included 49 patients (53 procedures) who underwent primary trapeziometacarpal joint replacement with a dual-mobility ball-and-socket impl…

INTRODUCTION: Femoral fractures in the transtrochanteric region and avulsion of the greater trochanter in skeletally immature individuals occur due to high-energy trauma. Due to the unique anatomy and blood supply of the proximal femur in growing children, these fractures are notorious for high rates of complications despite appropriate management. Classification of these fractures is done according to the Delbet system and AO classification, which not only guides management but also provide prognostic clues. Multiple fixation methods have been described, and there is no consensus on what cons

INTRODUCTION: Conservative treatment options, such as rest, massage, cold and heat packs, wrist splints, braces, physical therapy, thumb spica casts, and local steroid injections, have been used with mixed results to treat De Quervain's tenosynovitis. Surgical treatment, like releasing the first dorsal wrist compartment, is the last resort for resistant cases of De Quervain's tenosynovitis, exhibiting an efficacy of 91%. However, complications and increased expenses have limited the use of surgical interventions.
OBJECTIVES: This study aims to determine the efficacy, measured by the reduction…
