Best practice in end of life care and the use of anticipatory medicines.

Anticipatory prescribing is a vital aspect of end of life care and involves prescribing medicines in advance of a patient's clinical need. However, many nurses are not confident in identifying when a patient has entered the final stages of life or in prescribing and administering anticipatory medicines, with a lack of education potentially responsible. This article describes the rationale for anticipatory prescribing, particularly given the current context of an ageing population with increasing rates of comorbidities. The author outlines some of the distressing symptoms experienced by people

A Scoping Review of Australian Hospitals' Policies on the Deactivation of Implantable Cardioverter Defibrillators.

AIM: More older adults are living with an implantable cardioverter defibrillator (ICD). Deactivation rates for those approaching end-of-life are low, risking undue distress and an undignified death. This scoping review aimed to determine the availability and content of hospital guidance documents regarding ICD deactivation towards end-of-life. METHOD: Guidelines' databases from two Australian states (Victoria and South Australia) were systematically searched between September 2022 to February 2023, to identify all documents that specified guidance for ICD deactivation at end-of-life. [Guidelines for the Management of Venous Thromboembolism in End-of-Life Cancer Patients: National Consensus].

Venous thromboembolism is a common complication in cancer patients, particularly in advanced stages, where the presence of metastatic disease can increase the risk of thrombosis by approximately 20 times compared to early stages, with this risk being even higher in end-of-life patients. Despite its high prevalence, there are significant gaps in the evidence guiding venous thromboembolism management during this phase of illness. It presents a therapeutic challenge due to the high rates of thrombotic recurrence and hemorrhagic complications. Additionally, diagnosis is complicated by the nonspeci

Diabetes management during the end of life.

For individuals with diabetes, maintaining optimal glycaemic control is essential to reduce the risk of long-term complications. However, as patients approach the end of life, the emphasis on tight glycaemic targets becomes less relevant. Instead, the primary goal shifts to maintaining blood glucose levels within a range that minimises the risk of hypoglycaemia and prevents symptomatic hyperglycaemia, thereby ensuring comfort and quality of life. This article explores the transition from disease-focused management to a person-centred approach grounded in comfort, dignity and ethical responsibi

Factors related to good death from the perspectives of informal caregivers of cancer patients: a systematic review.

INTRODUCTION: Ensuring a good death in cancer patients is a multifaceted phenomenon that depends on various factors from the perspective of the patient and their informal caregivers and is based on clinical, cultural and ethical conditions. The objective of the present systematic review is to investigate factors of good death from the perspective of informal caregivers of cancer patients. METHODS: An advanced English electronic search was conducted with no time limit in four online databases of PubMed, Scopus, Web of Science, ProQuest and using related keywords on April 29, 2024. The articles

The role of emotional intelligence in end-of-life care: a scoping review of studies involving healthcare professionals.

BACKGROUND: The capacity to recognize, label, comprehend, manage, and apply emotions, known as Emotional Intelligence (EI), is pivotal for health outcomes, especially within the emotionally charged context of end-of-life (EOL) care. Although a growing body of research examines EI within the realm of EOL care, a comprehensive synthesis of the results has yet to be conducted. AIM: This scoping review synthesizes evidence to explore the role of EI in EOL care by healthcare professionals, focusing on its associated factors and the effectiveness of interventions aimed at enhancing EI. OBJECTIVES:

Friends and neighbours as non-kin caregivers of seriously ill patients at the end of life: a scoping review of experiences, individual capacities, support needs and services.

BACKGROUND: The number of older individuals in need of care is projected to increase in the coming years. As support from family members is becoming increasingly limited, often due to geographical distance, non-kin caregivers, such as neighbours, friends, and acquaintances, are assuming an increasingly significant role in providing support. The present scoping review aimed to identify, describe and synthesise existing evidence on the experiences, individual capacities and support needs of informal non-kin caregivers of patients at the end of life, as well as the support services available to t

Nausea and vomiting in end-of-life care: managing this debilitating symptom in the community.

Nausea and vomiting (N&V) are common, debilitating and distressing symptoms for patients with advanced cancer, precipitating admission to hospital for intravenous antiemetic and re-hydration (Glare et al, 2011). The causes of N&V in end-of-life care (EOLC) are multifaceted, with appropriate therapy guided by thorough assessment (Walsh et al, 2017; Watson et al, 2019). Cyclizine and levomepromazine can, depending on aetiology, be cited as effective antiemetic agents for patients with advanced cancer (Ingleton and Larkin, 2015; Watson et al, 2019). Conversely, careful consideration of the use of

The spiritual concerns of people experiencing homelessness at the end of life.

BACKGROUND:: Spiritual care is a fundamental component of holistic end-of-life (EoL) care. AIM:: To explore what is known about the spiritual concerns of people experiencing homelessness towards the EoL. METHODS:: A narrative literature review was conducted from 1997 to June 2018 using CINAHL Complete, MEDLINE and PubMed. This identified just 11 relevant papers; eight papers report on studies based in the US, one paper reports on a study based in the Republic of Ireland (ROI), and two of the papers are literature reviews. RESULTS:: Both the ROI and US studies report the primacy of religious

Organ donation and imminent death: pro position.

PURPOSE OF REVIEW: Donation after cardiac death is associated with many problems including ischemic injury, high rates of delayed allograft function, prolonged time to asystole, and frequent organ discard. Imminent death donation (IDD) has been proposed as a separate category of organ donation: distinct from living donation and donation after cardiac death. RECENT FINDINGS: A protocol for IDD was developed at Rhode Island Hospital and published in the ethics literature. The United Network for Organ Sharing (UNOS) Ethics Committee reviewed the protocol and stated that IDD was ethically appropr

Care for dying patients with primary malignant brain tumor.

Patients with primary malignant brain tumor experience deterioration of multi-focal neurological deficits such as hemiparesis, aphasia, visual field defects, dysphagia, and disturbance of recent memory at the advanced stage of disease. With these advancing neurological deficits, many patients will inevitably prepare for death and may experience psychological and spiritual distress. Active listening is an important skill to explore the fears of patients with a terminal illness but in the advanced stage of a primary brain tumor, patients usually have great difficulty with verbal expression. Reframing palliative care by embracing early integration.

Around 600 000 people die each year in the UK, with the number of deaths set to increase to 736 000 within the next 10 years because of the ageing population. Although most of these deaths are predictable, people do not always die in their preferred place of care and may not experience a good death. Palliative and end-of-life care can vary and be inequitable because of socioeconomic and demographic factors, as well as the person's medical condition. Robust systems must be developed to meet the requirements of people who need palliative care and to avoid inappropriate treatment that may impact

Nursing interventions for patients in last days of life and their family caregivers at home: a protocol for a mixed methods systematic review.

BACKGROUND: The demand for end-of-life care will rise significantly in coming decades due to increasing life expectancy and the growing burden of chronic diseases. Palliative care aims to support patients' preferences to remain at home (both for ongoing care and, when possible, to die). The feasibility of dying at home is shaped by a complex interplay of patient factors, the capacity of family caregivers, and the availability of home care teams and community resources. Healthcare professionals, especially nurses, are pivotal in accommodating patient preferences, and are uniquely positioned to

End-of-Life Care in the Cardiovascular Intensive Care Unit.

The cardiovascular intensive care unit (CVICU) presents unique opportunities and challenges for integrating palliative care. A growing population of patients living longer with advanced cardiac disease, persistent symptoms, and functional limitations may benefit from early palliative involvement. With increasing use of advanced therapies such as mechanical circulatory support, many patients die in the CVICU, while survivors often experience long-term physical and psychosocial burdens. Despite evidence that early palliative care improves quality of life and reduces nonbeneficial interventions,

Advance Care Planning Challenges in Adult Congenital Heart Disease.

Due to advances in congenital heart disease management, there are now more adults than children living with congenital heart disease in the United States. This population of over 1.4 million people is expected to continue to grow rapidly as a result of improving survival. However, patients with adult congenital heart disease continue to face increased rates of morbidity, hospitalizations, and medical interventions compared to the general population, in addition to elevated mortality. Therefore, comprehensive advance care planning is integral to caring for this patient population. Yet despite r
