Tranexamic acid, an inhibitor of plasminogen activation, aggravates staphylococcal septic arthritis and sepsis.

Haemostatic balance shifts towards pro-coagulation during infection. Plasminogen, a key molecule of fibrinolysis, may play an important role in the pathogenesis of staphylococcal infections. In the present study, we assessed the impact of inhibition of plasminogen activation by tranexamic acid on the course of staphylococcal sepsis and septic arthritis in mice. We found significantly down-regulated plasmin activity and increased D-dimer levels in the blood from the mice with staphylococcal sepsis. Treatment with tranexamic acid significantly increased the severity and mortality of staphylococc…

Tranexamic acid protects against rotenone-induced apoptosis in human neuroblastoma SH-SY5Y cells.

Rotenone is a pesticide that has been shown to induce the pathological symptoms of Parkinson's disease (PD) in animal models. In this study, we investigated the protective effects of tranexamic acid (TA) on rotenone-induced apoptosis in human dopaminergic SH-SY5Y cells. TA blocked the rotenone-induced phosphorylation of JNK and P38, the downregulation of BCL2 and the upregulation of BAX. Furthermore, TA not only decreased the rotenone-induced cleavage of caspase 9, PARP, and caspase 3, but also increased caspase 3 enzymatic activity. Our findings indicate that TA is able to protect neuronal ce…

Prophylactic tranexamic acid in elective, primary coronary artery bypass surgery using cardiopulmonary bypass.

OBJECTIVE: Perioperative use of tranexamic acid (TA), a synthetic antifibrinolytic drug, decreases perioperative blood loss, and the proportion of patients receiving blood transfusion in cardiac surgery, but the results may vary in different clinical settings. The primary objective of the present study was to determine the efficacy of TA to decrease chest tube drainage and the proportion of patients requiring perioperative allogeneic transfusions following primary, elective, on-pump coronary artery bypass grafting (CABG) in patients with a low baseline risk of postoperative bleeding. METHODS:…

Tranexamic acid (AMCA) and late hyphaema. A double blind study in cataract surgery.

A double blind study of the effect of tranexamic acid (AMCA) on late hyphaema after cataract surgery is reported. A total of 244 patients were included in the study after strict selection. It was shown that the incidence of late hyphaema in the treated group was significantly lower at the 5% level than that in the placebo group. The conclusion is made that the fibrinolytic inhibitor tranexamic acid can be used therapeutically or prophylactically to decrease the risk for re-bleeding after ocular surgery.

Tranexamic acid for the prevention of blood loss after cesarean among women with twins: a secondary analysis of the TRAnexamic Acid for Preventing Postpartum Hemorrhage Following a Cesarean Delivery randomized clinical trial.

BACKGROUND: Although prophylactic tranexamic acid administration after cesarean delivery resulted in a lower incidence of calculated estimated blood loss of >1000 mL or red cell transfusion by day 2, its failure to reduce the incidence of hemorrhage-related secondary clinical outcomes (TRAnexamic Acid for Preventing Postpartum Hemorrhage Following a Cesarean Delivery trial) makes its use questionable. The magnitude of its effect may differ in women at higher risk of blood loss, including those with multiple pregnancies. OBJECTIVE: This study aimed to compare the effect of tranexamic acid vs p…

Tranexamic Acid With Acid Suppression Versus Acid Suppression Alone as Therapy for Upper Gastrointestinal Bleeding: A Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Studies have reported the benefits of tranexamic acid (TXA) in controlling bleeding across various conditions. However, the effect of TXA in upper gastrointestinal bleeding (UGIB) remains controversial, and its therapeutic impact when combined with acid suppression, particularly proton pump inhibitors (PPIs), which are considered first-line therapy for bleeding peptic ulcers, has not been reported. METHODS: We systematically searched PubMed, Embase, and Google Scholar from January 1987 to June 2024 using predefined keywords to identify RCTs meeting our inclusion criteria, includin…

Tranexamic acid in patients with traumatic brain injury: Is there benefit in patients with fibrinolytic shutdown?

BACKGROUND: Tranexamic acid (TXA) is widely used in patients with traumatic brain injury (TBI). However, its pharmacology suggests differences in effectiveness should exist based on fibrinolytic phenotype, particularly fibrinolytic shutdown. This study examined whether fibrinolytic shutdown is associated with increased mortality in TBI and whether TXA retains its benefit in this subgroup. METHODS: This retrospective cohort study included TBI patients who had thromboelastography (TEG) upon presentation at three affiliated trauma centers. Patients were excluded if they received TXA before TEG, …

Tranexamic acid in spinal surgery: a stratified protocol from the surgeon's point of view.

INTRODUCTION: Intraoperative bleeding in spinal surgery remains a major concern, given its association with increased morbidity, prolonged hospitalization, and greater transfusion requirements. Tranexamic acid (TXA), a synthetic antifibrinolytic, has shown consistent efficacy in minimizing blood loss across diverse surgical contexts. Nevertheless, its application in spinal surgery must be individualized, taking into account bleeding risk, surgical complexity, and patient comorbidities. RESEARCH QUESTION: Following the creation of a dedicated Spine Unit, our objective was to design and validat…

Sterility of Tranexamic Acid Syringes Prefilled by Tactical Combat Medics in a Garrison Setting.

BACKGROUND: Tranexamic acid (TXA) is an antifibrinolytic agent that confers a significant mortality benefit in the treatment of hemorrhagic trauma. The benefit of TXA is timedependent; Special Operations Tactical Medics (SOTM) and Special Operations Independent Duty Corpsmen (SOIDC) commonly prefill TXA syringes prior to a mission. However, the sterility of prefilled TXA syringes remains unclear. METHODS: SOTMs (n=40) and SOIDCs (n=11) each filled two syringes with TXA in a garrison-type training setting using their customary preparation technique. As controls, a trained clinical research coo…

Stopping the flow: Tranexamic acid as an adjunct to anterior epistaxis management - An invited commentary.

Anterior epistaxis is one of the most common emergencies encountered by otolaryngologists and is usually managed with anterior nasal pressure, topical vasoconstrictors, anesthetics, cautery, and nasal packing. While traditional methods are often effective, they can cause discomfort and can result in recurrent bleeding and repeat emergency department visits. Tranexamic acid (TXA) is a synthetic lysine analog developed in the 1960s as an antifibrinolytic agent and has become an emerging adjunct in the acute management of epistaxis. By inhibiting fibrinolysis locally at the bleeding site, topical…

Evaluating the Safety of Tranexamic Acid in Cancer Surgery: Clinical Evidence and Biological Rationale.

Tranexamic acid (TXA) is widely used to reduce blood loss and red cell transfusion in major surgery. Its safety in oncologic surgery remains uncertain, given theoretical risks of venous thromboembolism (VTE) and tumor progression. We conducted an analytic review of clinical and laboratory evidence evaluating TXA in cancer surgery or cancer-relevant models. We performed a clinically focused review of experimental and clinical evidence examining TXA use in oncologic settings. Key influential studies were prioritized and supplemented by a structured literature search of MEDLINE, Embase, CENTRAL, …

The efficacy of tranexamic acid in prevention and treatment of post-inflammatory hyperpigmentation: a pilot study.

Post-inflammatory hyperpigmentation (PIH) is a common complication following trauma or cutaneous inflammation, particularly affecting individuals with skin of colour. Tranexamic acid (TXA) is a lysine analogue with antifibrinolytic properties and has been used as an off-label agent for melasma and procedural PIH. This pilot study evaluated the efficacy of oral TXA in the prevention and treatment of PIH using a validated in vivo model. A baseline site was treated with 30% trichloroacetic acid to induce PIH (control). Once PIH developed, another site was induced and patients started oral TXA 650…

Is Tranexamic Acid Associated With a Reduced Need for Hypotensive Anesthesia During Orthognathic Surgery?

BACKGROUND: No prior study in orthognathic surgery has isolated the effects of tranexamic acid (TXA) from deliberate hypotensive anesthesia (HA). Due to hypoperfusion risks with HA, it is valuable to evaluate the efficacy of TXA in the absence of HA as the utility of TXA to decrease blood loss may potentially make HA an unnecessary risk. PURPOSE: The study purpose was to assess surgical site visualization and measure blood loss among subjects exposed to TXA during bimaxillary orthognathic surgery without deliberate HA. STUDY DESIGN, SETTING, SAMPLE: A prospective cohort study was performed a…
