[Tubulointerstitial nephritis syndrome and uveitis].

Intermediate uveitis is described as inflammation in the anterior vitreous, ciliary body and the peripheral retina. It is a subset of uveitis where the vitreous is the major site of damage. It has been reported to be associated with many local and systemic inflammatory and infectious diseases. An infrequent cause is the tubulointerstitial nephritis and uveitis syndrome. We report a case of an acute visual acuity loss related with renal failure in a 64 years old woman with Hashimoto disease. It was an acute tubulointerstitial nephritis and uveitis syndrome case.

Uveitis in juvenile idiopathic arthritis.

BACKGROUND: Juvenile idiopathic arthritis (JIA) is the most common systemic disease causing uveitis in childhood, with a prevalence of 10 per 100 000 persons. JIA often takes a severe inflammatory course, and its complications often endanger vision. METHODS: This review is based on pertinent articles retrieved by a selective literature search up to 18 August 2014 and on the current interdisciplinary S2k guideline on the diagnostic evaluation and anti-inflammatory treatment of juvenile idiopathic uveitis. RESULTS: Uveitis arises in roughly 1 in 10 patients with JIA. Regular eye check-ups shou…

Magnetic resonance imaging of uveitis.

INTRODUCTION: Uveitis is a term used to describe inflammation of the choroid, iris, or ciliary body, which make up the uveal tract. It can be idiopathic or associated with a systemic disease which may be infectious or noninfectious. With the exception of B-scan ultrasonography, current imaging methods for diagnosing and monitoring uveitis are predominately non-radiologic. Although MRI has been anecdotally shown to detect various inflammatory conditions of the globe, such as posterior scleritis, endophthalmitis, and posterior uveitis secondary to Vogt-Koyanagi-Harada disease, a more comprehensi…

Rapidly blinding posterior tubercular uveitis.

BACKGROUND: A 21-year-old female patient had chorioretinitis in the left eye which relapsed while being on anti-tubercular treatment and oral corticosteroids leading to blindness and the loss of the left eye. FINDINGS: Mycobacterium tuberculosis causing chorioretinitis showed a poor response, and the lung lesions showed a good response to the same anti-tubercular treatment. CONCLUSIONS: Mycobacterium tubercle bacilli in the eye may show a poor response to the anti-tubercular drugs due to poor ocular penetration of the drugs secondary to early ocular hypoxia.

Uveitis: the collaborative diagnostic evaluation.

Uveitis, or inflammation of the uveal tract (i.e., iris, ciliary body, and choroid), results from a heterogeneous collection of disorders of varying etiologies and pathogenic mechanisms. Uveitis is caused by a systemic disease in 30% to 45% of patients. Primary care physicians may be asked to evaluate patients with uveitis when an underlying systemic diagnosis is suspected but not apparent from eye examination or history. If the history, physical examination, and basic laboratory studies do not suggest an underlying cause, serologic tests for syphilis and chest radiography for sarcoidosis and …

Diagnostic vitrectomy for infectious uveitis.

The identification of an infectious or noninfectious uveitis syndrome is important to determine the range of therapeutic and prognostic implications of that disease entity. Diagnostic dilemmas arise with atypical history, atypical clinical presentations, inconclusive diagnostic workup, and persistent or worsened inflammation despite appropriate immunosuppression. More invasive intraocular testing is indicated in these situations particularly in infectious uveitis where a delay in treatment may result in worsening of the patient’s disease and a poor visual outcome. Laboratory analysis of vitreo…

Differential diagnosis of behçet uveitis.

The diagnosis of Behçet disease is clinical and based on the presence of characteristic ocular and systemic inflammatory manifestations. Patients may present with anterior, posterior, or panuveitis in one or both eyes. The differential diagnosis includes a variety of infectious and noninfectious causes of acute nongranulomatous anterior uveitis, intermediate uveitis, occlusive retinal vasculitis, focal or multifocal retinitis, and necrotizing retinitis. A course characterized by sudden onset with improvement followed by recurrence of inflammatory signs is most typical for Behçet uveitis.

[Two cases of tuberculous uveitis].

Uveitis has many etiologies, but tuberculous uveitis is rare. We herein report 2 cases of uveitis due to tuberculosis infection. The first case was a 28-year-old man who was showed abnormal shadows in the chest radiographic examination performed in search of the etiology of uveitis. Computed tomography (CT) of the chest revealed hilar and mediastinal lymphadenopathy, small nodules, and consolidation, with a small cavity in the right upper lobe. An ulcerated nodule in the truncus intermedius and stenosis of the right middle lobe bronchus were found on bronchoscopy. The biopsy of the nodule in t…

Acute anterior uveitis in Sydney.

PURPOSE: To evaluate the patterns of acute and recurrent acute anterior uveitis (AAU) in a metropolitan tertiary referral center. METHODS: A retrospective medical record review of a total of 241 patients with AAU presenting to Sydney Eye Hospital between June 2009 and June 2011 was performed. Patients who underwent typing for the HLA-B27 antigen were included in this study. Ninety-five patients were HLA-B27-positive and 146 were HLA-B27-negative. RESULTS: Seronegative spondyloarthropathy (n = 26, 11%) was the most common systemic disease association in the HLA-B27 positive patients. Posterio…

[Syphilis-associated uveitis in Switzerland].

BACKGROUND: In spite of the worldwide declining number of syphilis cases, there has been stable rate of syphilis infections in Switzerland from 2006 - 2008. In the year 2008 672 case were reported. Uveitis is the most common ocular finding in syphilis. It occurs in 2.5 %-5 % of all cases. HISTORY AND SIGNS: Six patients (5 male, mean age 44.2 years, 1 female, age 41 year) who were examined in our facility from April 2007 to October 2009. All patients presented with posterior uveitis, with a multitude of clinical signs. All patients tested positive for syphilis-specific serologic testing (VDRL…

[Recent advances in tuberculous uveitis].

With an increasing number of new cases of tuberculosis every year, the incidence of tuberculous uveitis presents a rising trend. The disease is associated with a wide spectrum of clinical manifestations. The common clinical presentations appear to be anterior uveitis, choroidal tubercles, multifocal choroiditis, rarely serpiginous-like choroiditis, subretinal abscesses or suspected ocular tumors. Macular edema can be the only ocular manifestation of tuberculosis. With absence of proper diagnostic standard, it usually leads to missed diagnosis, misdiagnosis, and delayed diagnosis of tuberculous…

[Systemic voclosporin for uveitis treatment].

To date cyclosporine is the only immunosuppressive drug approved for the treatment of uveitis in Germany. However, side effects often limit its use in daily practice. Voclosporin (Luveniq) represents a next generation calcineurin inhibitor which had shown efficacy in an animal model of experimental autoimmune uveitis as well as in clinical trials for the prevention of renal allograft rejection and for plaque psoriasis. The LUMINATE program included three prospective, randomized, double-masked, placebo-controlled multicenter trials which demonstrated the efficacy and tolerability of voclosporin…

[Uveitis in juvenile idiopathic arthritis].

Anterior uveitis usually occurs in the context of juvenile idiopathic arthritis in about 10% of patients. Frequency is dependent on JIA subtype. Uveitis is most commonly found in patients with extended oligoarthritis (up to 25%) and early-age onset of arthritis. As the uveitis is usually without externally recognisable signs and often leads to ocular complications, all JIA patients should undergo regular ophthalmological examinations to ensure the promptest possible diagnosis and therapy. About 25% of uveitis patients have a complicated clinical course and require systemic immunosuppression. I…

Juvenile idiopathic arthritis-associated uveitis.

PURPOSE OF REVIEW: To describe recent evidence from the literature pertaining to juvenile idiopathic arthritis (JIA)-associated uveitis. RECENT FINDINGS: Uveitis is most common in extended oligoarticular JIA. A significant number of patients already have ocular complications at time of diagnosis of uveitis. Risk factors for complications include either abnormally high or low intraocular pressure, posterior synechiae, male sex, temporal proximity to diagnosis of arthritis and topical corticosteroid use. Use of immunosuppressive agents significantly reduces ocular complications. Aggressive peri…

[Lyme uveitis: 2 case reports].

Lyme borreliosis is a zoonosis characterized by great clinical polymorphism. We report 2 cases in patients who presented ocular manifestations: one anterior uveitis in an adult, which resolved under treatment, and one posterior uveitis in a child, whose initial ophthalmologic examination already showed retinal fibrous scars including the macular area. Lyme disease is on the long list of causes that must be discussed in cases of uveitis. The diagnosis is based on a series of epidemiological, clinical, and biological arguments with Western Blot serological profile analysis. Treatment is based es…
