Evaluating the social return on investment of nature-based social prescribing to alleviate loneliness: a randomized controlled trial in Barcelona.

INTRODUCTION: Loneliness is associated with adverse health outcomes, including cardiovascular diseases, depression, and premature mortality, and is unequally distributed according to social determinants. Prescribing individuals to community-based interventions in nature, often described as nature-based social prescribing (NBSP), has emerged as a promising intervention to address loneliness. This study aims to quantify the social return of investment (SROI) of the "Friends in Nature" (FiN) intervention designed to alleviate loneliness. METHODS: Following key guidelines, we performed a SROI stu

Addressing barriers to dental care: a Tūranga Health and Trinity Koha Dental initiative.

AIM: There is a high level of unmet need for dental care in Aotearoa New Zealand due to the inability of people facing socio-economic disadvantage to afford optimal and timely care. This includes many Māori. METHOD: This paper describes a joint initiative between a philanthropic organisation, Trinity Koha Dental Clinic, and Tūranga Health, an iwi-based health provider based in Gisborne. The service was designed for those with high needs and involved provision of dental care at no cost to the patients (addressing affordability as a barrier). It was also designed to address other barriers to ac

Integrating Oral Health Into Kidney Care: A Policy Imperative for Chronic Kidney Disease, the US Experience.

Emerging evidence underscores the bidirectional relationship between oral health, diabetes mellitus (DM) and chronic kidney disease (CKD), highlighting oral inflammation as both a possible cause and consequence of systemic disease. Periodontal therapy has been shown to lower HbA1c by 0.43%-0.50%, reduce systemic inflammation and slow CKD progression, effects comparable to adding a second hypoglycaemic agent. Recent policy directives, including the American Society of Nephrology's (ASN) 2025 nominations to the Centers for Medicare & Medicaid Services (CMS) and the 2026 Medicare Physician Fee Sc

A modelled cost-utility analysis comparing early referral to specialist palliative care versus standard care for older people with acute myeloid leukaemia.

AIM: To investigate whether early referral to specialist palliative care (SPC) is cost-effective compared to standard care for people ≥ 60 years of age with newly diagnosed acute myeloid leukaemia (AML). METHODS: A modelled cost-utility analysis, from an Australian public healthcare system perspective, over a 5-year time horizon. A systematic review of studies reporting health state utility values (HSUVs), or convertible health-related quality of life measures, provided values for the decision tree model. Cost and effectiveness data were sourced from a broad review of the literature. All cost

Tuberculin skin test and isoniazid prophylaxis among health care workers in high tuberculosis prevalence areas.

The tuberculin skin test (TST) is an important tool for the detection of latent tuberculosis (TB) and the identification of health care workers (HCWs) who require chemoprophylaxis. Although TST is inexpensive, easily available and the preferred test in most TB-prevalent settings, it has recognised limitations, including subjective interpretation, false positivity, cross reactivity with non-tuberculous mycobacteria, administration errors and the requirement for two visits. Given these limitations and the unavailability of better screening tests in resource-limited settings, the acceptance rate

Artificial Intelligence as a Catalyst for Value-Based Health Insurance in the United States: Narrative Review and Policy Perspective.

The United States health insurance system is at a critical crossroads. Inflating costs, fragmented care, and administrative inefficiencies have revealed the limitations of the Fee-for-Service (FFS) model. This long-standing structure, while once effective in expanding access, now struggles to deliver efficiency and value. Value-based care (VBC) aims to realign incentives toward outcomes, quality, and efficiency. This article explores how artificial intelligence (AI) can serve as the digital backbone to accelerate the transition from FFS to VBC. The article reviews evidence from bundled payment

Accuracy of the Enhanced Liver Fibrosis Test vs FibroTest, Elastography, and Indirect Markers in Detection of Advanced Fibrosis in Patients With Alcoholic Liver Disease.

BACKGROUND & AIMS: Alcohol is the leading cause of cirrhosis and liver-related mortality, but we lack serum markers to detect compensated disease. We compared the accuracy of the Enhanced Liver Fibrosis test (ELF), the FibroTest, liver stiffness measurements (made by transient elastography and 2-dimensional shear-wave elastography), and 6 indirect marker tests in detection of advanced liver fibrosis (Kleiner stage ≥F3). METHODS: We performed a prospective study of 10 liver fibrosis markers (patented and not), all performed on the same day. Patients were recruited from primary centers (municip

Midwifery-led continuity-of-care education affects health literacy, empowerment and childbirth fear: a quasi-experimental study.

BACKGROUND: According to the World Health Organisation, antenatal care should ensure continuity, respect and evidence-based education. Fear of childbirth (FOC), affecting 20-40% of women globally, contributes to prolonged labour, higher caesarean rates and negative birth experiences. In Türkiye, where midwifery-led support remains limited, this study examined how pregnant women's health literacy influences empowerment and FOC within antenatal care and evaluated the effectiveness of the midwifery-led continuity-of-care (MLCC) model. METHODS: A quasi-experimental study with a comparative group

An evidence-based approach to spine surgery.

Health care reform will emphasize evidence-based medicine to provide the highest quality care. Recent literature has emerged in spinal surgery that has profoundly increased the evidence base for several spinal procedures. There is now good evidence from randomized controlled trials that surgical treatment of symptomatic lumbar disc herniation, decompression for spinal stenosis, and decompression and fusion for degenerative spondylolisthesis all offer significant clinical benefit in the face of serious back and radicular pain when compared with nonsurgical care. Studies of nonsurgical and surgi

[The Nursing Experience Using Pedal Exercise to Prevent Hypotension in Hemodialysis Patients].

This is a case report on the nursing care of an elderly patient with end-stage renal disease experiencing recurrent intradialytic hypotension resulting in physical discomfort and psychological anxiety during hemodialysis. The period of care spanned from August 1-10, 2024. After conducting a comprehensive assessment based on Gordon's Eleven Functional Health Patterns, three primary health problems were identified: deficient fluid volume, impaired physical mobility, and anxiety. The corresponding interventions employed were 1) implementing low-intensity pedal exercise during dialysis to promote

A dynamic customized electronic health record rule based clinical decision support tool for standardized adult intensive care metrics.

OBJECTIVES: Many routine patient care items should be reviewed at least daily for intensive care unit (ICU) patients. These items are often incompletely performed, and dynamic clinical decision support tools (CDSTs) may improve attention to these daily items. We sought to evaluate the accuracy of institutionalized electronic health record (EHR) based custom dynamic CDST to support 22 ICU rounding quality metrics across 7 categories (hypoglycemia, venothromboembolism prophylaxis, stress ulcer prophylaxis, mechanical ventilation, sedation, nutrition, and catheter removal). DESIGN: The dynamic C

Metric properties of the appropriateness evaluation protocol and predictors of inappropriate hospital use in Germany: an approach using longitudinal patient data.

BACKGROUND: The German health care system, renowned for its unrestricted access, high quality care, and comprehensive coverage, is challenged by increasing health care costs. This has been attributed partly to inefficiencies in the in-patient sector, but has been studied little. Attempts at quality improvement need to relate costs to outcomes. Until now, there has been no standardized methodology to evaluate the appropriateness of hospital care. OBJECTIVE: To develop and evaluate the metric properties of a method to assess inappropriate hospital care in Germany based on a widely used measure,

International Classification of Functioning, Disability and Health: Catalyst for interprofessional education and collaborative practice.

Patient-centred and community-based care is required for promotion of health equity. To enhance patient-centred interprofessional care, the World Health Organization recommends using the framework of the International Classification of Functioning, Disability and Health (ICF). Stellenbosch University's Interprofessional Education and Collaborative Practice (IPECP) strategy has promoted using ICF since 2010. Undergraduate medical students on rural clinical placements are expected to use ICF in approaching and managing patients. Students' ability to develop interprofessional care plans using ICF

Moving from Evidence to Implementation of Breakthrough Therapies for Diabetic Kidney Disease.

Diabetic kidney disease is the most frequent cause of kidney failure, accounting for half of all cases worldwide. Moreover, deaths from diabetic kidney disease increased 106% between 1990 and 2013, with most attributed to cardiovascular disease. Recommended screening and monitoring for diabetic kidney disease are conducted in less than half of patients with diabetes. Standard-of-care treatment with an angiotensin-converting enzyme inhibitor or an angiotensin receptor blocker is correspondingly low.
