Background: Venous vascular access with higher sheath size is common in interventional electrophysiology. In contrast to arterial vascular access, no dedicated closure devices exist for closure after venous access with higher sheath sizes. The Figure-of-8-Suture, an easy to apply suture, may be as a feasible approach for closure venous puncture. Our aim was to evaluate the feasibility of closure of femoral venous access. Methods: From November 2016 to February 2018, patients undergoing electrophysiological procedures, closure of left atrial appendage or patent foramen ovale were included. Unti

The current narrative review provides an update of available knowledge on venous access techniques for cardiac implantable electronic device implantation, with a focus on axillary vein puncture. Lower procedure-related and lead-related complications have been reported with extrathoracic vein puncture techniques compared with intrathoracic accesses. In particular, extrathoracic lead access through the axillary vein seems to be associated with lower complication incidence than subclavian vein puncture and higher success rate than cephalic vein cutdown. In literature, many techniques have been de

BACKGROUND: Vascular access-site complications are among the most frequent adverse events of endovascular treatment (EVT) for acute ischemic stroke (AIS). Suture-based closure devices (SBD) are widely used in peripheral and cardiac procedures, but their performance has not been specifically assessed in stroke EVT. We aimed to compare SBD with the more commonly used bioabsorbable plug devices (BPD) in transfemoral stroke thrombectomy.
METHODS: We conducted a multicenter retrospective cohort study across seven French tertiary stroke centers. We included consecutive patients undergoing transfemo

BACKGROUND: Leadless pacemakers traditionally rely on femoral venous access, which may be limited in patients with unfavorable IVC anatomy, prior interventions, or situations where preserving femoral access is preferred. Internal jugular (IJ) access offers an alternative route, but real-world data on IJ implantation of single- and dual-chamber leadless devices remain limited.
OBJECTIVE: To describe patient characteristics, procedural details, and outcomes of pacemaker implantation via IJ access.
METHODS: We performed a retrospective case series of five patients who underwent Abbott Aveir lea

Data comparing MANTA device with Perclose device for large bore arterial access closure is limited. We performed meta-analysis to compare safety and efficacy of the two devices in large (⩾14 Fr sheath) arteriotomy closure post-TAVR. Relevant studies were identified via PubMed, Cochrane, and EMBASE databases until June, 2022. Data was analyzed using random effect model to calculate relative odds of VARC-2 defined access-site complications and short-term (in-hospital or 30-day) mortality. A total of 12 studies (2 RCT and 10 observational studies) comprising 2339 patients were included. The odds

BACKGROUND: Cerebral embolism remains a concern during transcatheter aortic valve replacement (TAVR). Cerebral embolic protection (CEP) devices have been developed to mitigate this risk, but their clinical benefit remains unclear.
METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials comparing TAVR with and without CEP devices. Primary endpoint was overall stroke; secondary endpoints included disabling stroke, all-cause mortality, life-treating bleeding, vascular complications related to CEP access and acute kidney injury.
RESULTS: Eight trials comprisin

Peripheral intravenous catheters (PIVCs) are the preferred vascular access for apheresis; however, veins that initially appear suitable may develop venous spasms, resulting in poor inlet flow and frequent pressure alarms. Ports used for apheresis can also experience inlet pressure alarms due to catheter or venous occlusion. When conservative measures failed, we implemented a novel double-draw technique in which a second draw line was connected via a four-way stopcock and male-to-male adapter to the initial draw access (either a PIVC or port) to improve inlet flow. The double-draw technique was

Vascular access devices (VADs), including both peripheral intravenous (IV) catheters and central VADs, are used across the continuum of health care to administer IV fluids, medications, and diagnostics. Needleless connectors (NCs) are a critical component of the infusion system providing needle-free access to VADs yet many nurses are unaware of their significant impact on VAD-related complications, including infection, thrombotic occlusion, and catheter malfunction. This article provides a review of the critical role of the nurse in identifying risks and applying best practices in NC care, inc…

The majority of patients with kidney failure requiring replacement therapy will need the support of hemodialysis during their journey with kidney failure. A reliable functioning vascular access is required to provide hemodialysis. This Core Curriculum reviews the major forms of vascular access (arteriovenous fistula, arteriovenous graft, and central venous catheter) as well as the planning, preparation, creation, use, and maintenance of vascular access, requiring a P-L-A-N (Patient ESKD Life-Plan first then Access Needs) for each patient. The end-stage kidney disease Patient Life-Plan focuses

INTRODUCTION: More than 20 years have passed since the first clinical application of continuous renal replacement therapy (CRRT) in children. In that revolutionary era, before roller pumps and dialysis monitors for intensive care units were readily available, continuous arteriovenous hemofiltration was the most common treatment for critically ill children.
MAJOR FINDINGS: Those steps were the basis for current knowledge about modern CRRT. Research on circuit rheology and filter materials allowed for the improvement of materials, and the optimization of patency and session life spans. Hemofilt

We describe a case of an elderly patient who presented with right-sided ophthalmoplegia, proptosis, chemosis, and increased intraocular pressure. An angiogram showed feeding vessels from the bilateral internal and external carotid arteries. Our initial attempt to blindly probe the inferior petrosal sinus was unsuccessful. This was followed by a right anterior orbitotomy exposing the superior ophthalmic vein which was directly cannulated with an 18 gauge angiocatheter. However, a proximal third of the superior ophthalmic vein within the orbit which was thrombosed was probed blindly. The thrombo
